ABSTRACT

BACKGROUND: Melasma is a prevalent skin pigmentation disorder that is difficult to treat. Tranexamic acid (TA) is a potential agent, but there are few studies on its effectiveness under the transdermal route (drug delivery-Dd). One of these Dd pathways is through microneedling, which seems to be effective itself. OBJECTIVE: To evaluate the efficacy of tranexamic acid when applied in the form of drug delivery through microneedling in the treatment of facial melasma. METHODS: A randomized controlled double-blind split-face trial with 3 monthly sessions in 20 melasma patients: microneedling was performed in the entire face, and then TA solution was applied to one hemiface and placebo to the other. The effectiveness was measured using Hemi-MASI (Melasma Area and Severity Index), images pixels, and perceptions of experts and patients. RESULTS: Hemi-MASI regressed 22% in control and 29% in TA side. A good/better improvement was found in 37.5% of the control and 42.5% of TA by the experts and 60% of the patients for both sides. Pixels increased by 5 and 7, respectively. In none of these criteria, there was a significant difference between the sides. CONCLUSION: Tranexamic acid in drug delivery through microneedling did not bring additional benefit to the treatment of melisma.

ABSTRACT

A criocirurgia é uma técnica de vasto conhecimento e uso entre dermatologistas. A aplicação intralesional já é utilizada no tratamento de queloides e tem como vantagem restringir a zona de ação das baixas temperaturas do nitrogênio líquido. O uso da criocirurgia de contato em lesões vasculares é relatado, mas não há publicações sobre a técnica intralesional nesses tipos de lesões. Aqui, relatamos dois casos: 1- Hamartomas sobre mancha vinho do Porto em um paciente de 61 anos; e 2 - Angioma rubi em um paciente de 70 anos, ambos tratados de forma satisfatória e segura com criocirurgia intralesional.


Cryosurgery is a technique widely known and used among dermatologists. Intralesional application is already used to treat keloids and has the advantage of restricting the zone of action of low temperatures of liquid nitrogen. The use of contact cryosurgery in vascular lesions is well reported in the literature, but there are no intralesional technique publications on these types of lesions. Here, we report two cases, one of port-wine stain hamartomas in a 61-year-old patient and another of a cherry angioma in a 70-year-old patient, safely and satisfactorily treated with intralesional cryosurgery

A Artrite reumatoide (AR) é uma doença associada com aumento de risco cardiovascular em relação à população em geral. A leflunomida, utilizada para seu tratamento, tem um efeito hipouricemiante mas não se sabe se este fato influi na redução do risco cardiovascular. Objetivo: Estudar a influência do uso da leflunomida em níveis de ácido úrico sérico e na espessura da camada médio-intimal da carótida. Métodos: Sessenta pacientes com AR foram estudados: 30 usando leflunomida e 30 sem o seu uso. Dados acerca de fatores de risco cardiovascular, ácido úrico sérico e espessura da camada médio-intimal da carótida por ecografia foram obtidos. Resultado: A leflunomida mostrou-se capaz de reduzir os níveis de ácido úrico sérico (p=0.03) mas não influenciou os valores da espessura médio-intimal da carótida (p=0.51) Conclusão: Não foi possível demonstrar redução dos valores de espessura da média-íntima da carótida em pacientes usuários de leflunomida.


Background: Rheumatoid Arthritis (RA) is a disease associated with increased cardiovascular risk when compared to the general population. Leflunomide, used for its treatment, has a hypouricemiante effect but it is not known whether this fact influences the reduction of cardiovascular risk. Objective: To study the influence of the use of leflunomide in serum uric acid levels and the intimal middle layer of carotid. Methods: Sixty patients with RA were studied: 30 using leflunomide and 30 without it. Data on cardiovascular risk factors, serum uric acid and thickness of the middle-intimal layer of carotid by ultrasound were obtained. Result: Leflunomide was able to reduce serum uric acid levels (p=0.03) but did not influence the values of the medium-intimal thickness of carotid (p=0.51). Conclusion: It was not possible to demonstrate a reduction in the thickness values of the media-intima of carotid in patients using leflunomide.
