ABSTRACT
This paper presents a few theoretical considerations about the posttraumatic posterior luxation of the nucleus. We discuss the evolution and surgical treatment of a clinical case, and we remark once more that the posterior luxation of the nucleus is considered to be an delayed surgical emergency.

Cataract surgery of glaucoma patients treated with Pilocarpine several years, with a small and miotic pupil, with synechiae is an important problem of management. The self-retaining flexible iris retractor is a microsurgical innovation that allows adequate enlargement of the pupil without iris incision. Each retractor is inserted through a separate self-sealing beveled incision made as far posteriorly in the cornea as possible. The retractors are spaced 90 degree apart and can easily be removed following implant insertion. The conclusion is that if we obtain an adequate enlargement of the pupil we can choose any cataract surgery technique, even phacoemulsification or EEC.

In the beginning, the paper presents some general words about the main intraoperative complications of the phacoemulsification and some practical attitudes patient in front of the rupture of the posterior capsule. The second part presents the case of a 63 years old patient who was intended to be operated for cataract by phacoemulsification and who suffered a complication during this procedure--the rupture of the posterior capsule with the luxation of the majority of the nucleus into the vitreous body.

Starting from a clinical case, the paper presents a rare complication of the strabismus and retinal detachment surgery: the anterior segment ischemia syndrome. In this context, we discuss the clinical and pathogenetic circumstances of apparition, underlining the great importance of primary prophylaxis.

Seven patients with open angle glaucoma were treated using Dorsolamida twice daily together Betablocant treat. Five patients with secondary glaucoma were treated with Dorsolamida three time daily as well three patients with edematous postimplant keratopathy. Intraocular pressure decreased with an average of 3-4 mm Hg and at the patients with corneal edema that decreased.
