ABSTRACT

Attitudes to animal welfare are not understood well in China, the country with the highest output of farm animals in the world. We surveyed attitudes of the public around China using a team of researchers to conduct individual interviews, with 1301 respondents in total. Contrary to results obtained in several other countries, high school leavers were more concerned about animal welfare than those who had obtained a university degree. We speculate that this may reflect the labour market currently existing in China, with limited opportunities for graduates. Scientists were less supportive, and artists more supportive, of good animal welfare. Urban dwellers were more concerned about animal welfare than rural residents, with village residents in the middle, which confirmed our theory that such a difference prevails in developing countries, where a large proportion of the rural population are involved in agriculture. It is concluded that education level, occupation and living place all have pronounced influences on attitudes to animal welfare in China, some of which follow international trends.

ABSTRACT

The cardiac operating room (OR) is a high-risk, high-stakes environment inserted into a complex socio-technical healthcare system. During cardiopulmonary bypass (CPB), the most critical phase of cardiac surgery, the perfusionist has a crucial role within the interprofessional OR team, being responsible for optimizing patient perfusion while coordinating other tasks with the surgeon, anesthesiologist, and nurses. The aim of this study was to investigate objective digital biomarkers of perfusionists' workload and stress derived from heart rate variability (HRV) metrics captured via a wearable physiological sensor in a real cardiac OR. We explored the relationships between several HRV parameters and validated self-report measures of surgical task workload (SURG-TLX) and acute stress (STAI-SF), as well as surgical processes and outcome measures. We found that the frequency-domain HRV parameter HF relative power - FFT (%) presented the strongest association with task workload (correlation coefficient: -0.491, p-value: 0.003). We also found that the time-domain HRV parameter RMSSD (ms) presented the strongest correlation with perfusionists' acute stress (correlation coefficient: -0.489, p-value: 0.005). A few workload and stress biomarkers were also associated with bypass time and patient length of stay in the hospital. The findings from this study will inform future research regarding which HRV-based biomarkers are best suited for the development of cognitive support systems capable of monitoring surgical workload and stress in real time.

ABSTRACT

BACKGROUND: Transition of dairy cows from a tied to a loose housing system may affect their behaviour, health and production. Such housing system changes have become more frequent in Estonia but knowledge is lacking on how cows adapt to a new system. The aim of this study was to evaluate how cows' behaviour, milk production and composition, and different aspects of their health changed after transition from tied to loose housing. RESULTS: A herd of 400 dairy cows was moved to a new system on the same farm, so that effects of transport were not confounding factors. Behavioural observations were made for approximately 4 months following transition. Milk production data were recorded from 12 months before to 12 months after transition. Examination for skin alterations and cleanliness, as well as body condition scoring were carried out before transition, and thereafter monthly throughout the study. Significant effects on behaviour were observed just after the transition, with increases in the behaviour indicative of poor welfare, such as vocalisation and aggression, and decreases in those indicative of a good state of welfare, such as ruminating, resting and grooming. These effects were of short duration, with most returning to a steady state after the first week. Milk production declined already before the transition but fell significantly after transition, and this fall lasted longer in older cows. Likewise, somatic cell counts were higher in all cows following transition, but older cows were affected significantly more than cows in the first lactation. The frequency of lameness and skin alterations increased on average after transition. Body condition scores fell after transition but recovered by the second month. Therefore, there were adverse effects on the behaviour, health and production of the dairy cows transferred, although, apart from older cows, of short duration. CONCLUSION: The transition from tied to loose housing first had negative impacts on the welfare of the cows, although by the tenth day the behavioural indicators had returned to normal values. Impacts were more severe in higher parity cows, indicating that the change was more of a challenge for older cows. The findings of this study suggest that animals' behaviour and health should be more carefully observed within about 2 weeks after transition. It is quite likely that more and more farmers in Estonia and elsewhere will recognize the benefits of keeping their dairy cattle in loose housing, aimed at improving animal welfare and the value of the production chain.

ABSTRACT

It is widely assumed that the quality of human-animal interactions may have a strong impact on animals' living conditions and is fundamental to improving farm animal welfare. This work aims to evaluate the effectiveness of methods for assessing and monitoring the welfare of lactating goats. In particular, attention was paid to the methods regarding the assessment of the human-animal relationship. The experimental protocol included three tests and observations in the milking parlour, namely: avoidance distance in the pen, avoidance distance at the manger, approach test within 2 min and behavioural observations of stockmen and animals during milking. Fourteen dairy goat farms located in the province of Potenza (Southern Italy) with flocks ranging in size from 67 to 450 lactating goats were used. All farms raised the Rossa Mediterranean goat breed. The percentage of goats that permitted themselves to be touched at the manger (mean ± SD: 12.36 ± 9.50) was higher than that found in the pen (9.67 ± 11.86) and within 2 min inside the pen (8.19 ± 13.78). The avoidance distance at the manger (0.63 ± 0.28 m) was shorter than that recorded in the pen (1.19 ± 0.58 m). Avoidance distance at the manger was positively correlated with that in the pen (Spearman correlation test (rs) = 0.607; p < 0.01), as was the percentage of goats touched at the manger and in the pen (rs = 0.647; p < 0.01). Approximately 60% of the stockman interactions observed during milking were neutral, while positive and negative interactions had similar values to each other, approximately 20%. The number of neutral stockman interactions was positively correlated with the percentage of goats moving when approached at a distance >1 m (rs = 0.799; p < 0.001) and was negatively correlated with the animals touched at the manger (rs = -0.607; p < 0.05), in the pen (rs = -0.613; p < 0.05) and within 2 min (rs = -0.669; p < 0.01). As regards the degree of association between the behaviour of the milker and the animals during the milking routine, the percentage of neutral interactions tended to be positively correlated only with the number of kicks performed by the animals (rs = 0.476; p < 0.10). It is concluded that avoidance distance at the manger and in the pen, as for other farm animals, are valid tests to evaluate the human-animal relationship in goats. In addition, avoidance distance at the manger may reliably replace the test performed in the pen.

ABSTRACT

Clinical alarm and decision support systems that lack clinical context may create non-actionable nuisance alarms that are not clinically relevant and can cause distractions during the most difficult moments of a surgery. We present a novel, interoperable, real-time system for adding contextual awareness to clinical systems by monitoring the heart-rate variability (HRV) of clinical team members. We designed an architecture for real-time capture, analysis, and presentation of HRV data from multiple clinicians and implemented this architecture as an application and device interfaces on the open-source OpenICE interoperability platform. In this work, we extend OpenICE with new capabilities to support the needs of the context-aware OR including a modularized data pipeline for simultaneously processing real-time electrocardiographic (ECG) waveforms from multiple clinicians to create estimates of their individual cognitive load. The system is built with standardized interfaces that allow for free interchange of software and hardware components including sensor devices, ECG filtering and beat detection algorithms, HRV metric calculations, and individual and team alerts based on changes in metrics. By integrating contextual cues and team member state into a unified process model, we believe future clinical applications will be able to emulate some of these behaviors to provide context-aware information to improve the safety and quality of surgical interventions.

ABSTRACT

Excessive intra-operative noise in cardiac surgery has the potential to serve as source of distraction and additional cognitive workload for the surgical team, and may interfere with optimal performance. The separation from bypass phase is a technically complex phase of surgery, making it highly susceptible to communication breakdowns due to high cognitive demands and requiring tightly coupled team coordination. The objective of this study was to investigate team cognitive workload levels and communication in relation to intra-operative time periods representative of infrequent vs. frequent peaks in ambient noise. Compared to 5-minute segments with no peaks in noise at all, segments with the highest percentage of noise peaks (≥10%) were significantly associated with higher team members' heart rate before, during, and after noise segments analyzed. These noisier segments were also associated with a significantly higher level of case-irrelevant communication events. These data suggest that case-irrelevant conversations associated with a greater degree of excessive peaks in noise may be associated with team workload levels, warranting further investigation into efforts to standardize communication during critical surgical phases.

ABSTRACT

A person's gender and age can influence their attitudes towards animal welfare, with more benign attitudes generally ascribed to women. Given that attitudes influence consumer behaviour and the rapid recent social development in China (globally the biggest livestock producer), we surveyed over 1300 individuals across China to elucidate the role of gender and age in determining attitudes towards animals. Respondents self-identified their gender as male, female, other or not revealed. There were interactions between age and gender for many of the survey items, demonstrating that the effects of gender were dependent on the respondents' age. Women aged 18-24 reported more benign attitudes towards animals than older men (aged between 25 and 54 years, depending on the survey question) and more empathetic responses were found in young respondents generally, although this did not necessarily translate into a willingness to pay more for higher-welfare animal products. We propose, drawing on Social Identity Theory, that women see animals as part of their social group, whereas men tend not to do this. Those responding as neither male nor female, i.e., as another gender, and those not revealing their gender appeared to have different relationships to animals than those responding as men or women. It is concluded that within Chinese culture, attitudes towards animals and their welfare are complex and influenced by an interaction between gender and age.

ABSTRACT

Time lags between the initiation of a continuous drug infusion and achievement of a steady state delivery rate present an important safety concern. At least 3 factors contribute to these time lags: (1) dead volume size, (2) the ratio between total system flow and dead volume, and (3) startup delay. While clinicians employ both peristaltic pumps and syringe pumps to propel infusions, there has been no head-to-head comparison of drug delivery between commercially available infusion pumps with these distinct propulsion mechanisms. We quantified the delivery of a model drug by peristaltic and syringe pumps at clinically relevant flow rates using spectrophotometric absorbance. Delivery curves were modeled and compared, and the time required to reach 5% (T5), 50% (T50), and 95% (T95) of the intended delivery rate was reported. The ability to overcome the combined effects of startup delay and dead volume differed between syringe and peristaltic pumps. T5, T50, and T95 were shorter for the peristaltic pump at higher flow rates. T50 and T95 were shorter for the syringe pump at lower flow rates. The ability to overcome the effects of dead volume was overall similar between the syringe and peristaltic pumps, as was the response to consecutive changes in drug infusion rates. Startup delay and dead volume in carrier-based infusion systems cause substantial time lags to reaching intended delivery rates. Peristaltic and syringe pumps are similarly susceptible to dead volume effects. Startup performance differed between peristaltic and syringe pumps; their relative performance may be dependent on flow rate.

Factors influencing grazing behavior in species-rich grasslands have been little studied. Methodologies have mostly had a primary focus on grasslands with lower floristic diversity.We test the hypothesis that grazing behavior is influenced by both animal and plant factors and investigate the relative importance of these factors, using a novel combination of video technology and vegetation classification to analyze bite and step rates.In a semi-natural, partially wooded grassland in northern Estonia, images of the vegetation being grazed and records of steps and bites were obtained from four video cameras, each mounted on the sternum of a sheep, during 41 animal-hours of observation over five days. Plant species lists for the immediate field of view were compiled. Images were partnered by direct observation of the nearest-neighbor relationships of the sheep. TWINSPAN, a standard vegetation classification technique allocating species lists to objectively defined classes by a principal components procedure, was applied to the species lists and 25 vegetation classes (15 open pasture and 10 woodland) were identified from the images.Taking bite and step rates as dependent variables, relative importance of animal factors (sheep identity), relative importance of day, and relative importance of plant factors (vegetation class) were investigated. The strongest effect on bite rates was of vegetation class. Sheep identity was less influential. When the data from woodland were excluded, sheep identity was more important than vegetation class as a source of variability in bite rate on open pasture.The original hypothesis is therefore supported, and we further propose that, at least with sheep in species-rich open pastures, animal factors will be more important in determining grazing behavior than plant factors. We predict quantifiable within-breed and between-breed differences, which could be exploited to optimize conservation grazing practices and contribute to the sustainability of extensive grazing systems.

Low temperatures can provide a risk to the welfare of sheep during transport because of increased ventilation chilling the sheep, and we examined the importance of three factors-covering the vehicle, duration of transport, and feeding prior to transport-on the welfare of sixty transported 4-month-old Dorper × Mongolian female sheep in a cold climate. Sheep in a covered vehicle had greater increases in head and ear temperatures than those in an open vehicle. Sheep transported for 2 h increased their leg temperatures, whereas those transported for 1 h had reduced leg temperatures. Increases in non-esterified fatty acids (NEFA) and lactate dehydrogenase (LDH) in the blood samples during the long transport suggested that sheep had more muscular and metabolic activity, compromising their well-being. Feeding prior to transport did not affect body temperatures, but those not fed prior to transport had reduced alanine transferase, HSP and cortisol in their blood, whereas those that were fed had reduced NEFAs, LDH and creatine kinase. Prior feeding had no effect on the sheep temperature indices over a two-hour transport period. Thus, the sheep most at risk of the adverse effects of cold temperatures were those transported in open vehicles, those transported for a longer time, and those not fed before transport.

Food-producing animals make up the majority of animals that humans manage globally, and China has been a major producer and exporter of animal products since the late 1990s. The opinions of the population in China regarding animal welfare are not as well understood as those in Europe. In China, animal welfare as a societal concern is still at an early stage of development. This survey of Chinese attitudes aimed to understand consumer knowledge of and behaviour towards animal welfare, and to determine whether harnessing consumer interests may be a potential future influence on the development of high-welfare agricultural production. Most participants were not aware of the meaning of animal welfare, but the number of those that were aware was higher than reported previously. The welfare of wild animals was rated particularly important compared to other animals. The links between welfare and the taste and/or safety of food were considered to be important, and Chinese consumers reported a willingness to pay more for food from animals produced in good welfare conditions, although the quality of the food was considered more important than the animal suffering. A large majority of the respondents reported that there should be legislation protecting animals and certification of welfare on farms, that animals on farms should be provided with enjoyable experiences and that transportation times should be minimised. Furthermore, most respondents reported that animals should be stunned before slaughter. We conclude that animal welfare is of importance to the Chinese consumer, in particular because of its connection to food quality.

Regrouping dairy cows is a common feature of dairy farm management. Cows are grouped based on lactation stage, age, milk yield and other factors. Regrouping cows during the dry period (from far-off area to close up area and from close up area to the main herd) brings new challenges. This is especially true for heifers who, after being confirmed gravid, may be grouped into a new pen with dried off cows. The aims of this study were to determine how grouping affects activity, nearest neighbour relationships and aggression, and how heifers' acclimatization to a new group differs from cows. Therefore, the hypotheses were that regrouping cows has less of an effect on older cows compared to heifers, and cows' individuality affects acclimatization to a new group. Aggression data were recorded using a video camera that was directed at the feed bunk, and activity was recorded with activity monitors that were attached around the right hind leg. Synchrony and distance to nearest neighbour were recorded, as was the cows' location on the first 3 d from the day they returned to the main herd. Motion index, mean number of steps and number of lying bouts were significantly higher after calving compared to the week before calving and the difference was higher amongst heifers compared to cows (P < 0.001). Both cows and heifers lay down more in the strawyard compared to cubicle housing (P < 0.01) and cows were more aggressive than heifers in both housing systems (P < 0.001 and P < 0.05, respectively). As hypothesized, heifers were more affected by regrouping and cows with more experience settled quicker to their new environment.

ABSTRACT

OBJECTIVE: In this study, we built a mobile continuous Blood Oxygen Saturation (SpO2) monitor, and for the first time, explored key design principles towards daily applications. METHODS: We firstly built a customized wearable computer that can sense two-channel photoplethysmogram (PPG) signals, and transmit the signals wirelessly to smartphone. Afterwards, we explored many SpO2 model building principles, focusing on linear/nonlinear models, different PPG parameter calculation methods, and different finger types. Moreover, we further compared PPG sensor placement principles by comparing different hand configurations and different finger configurations. Finally, a dataset collected from eleven human subjects was used to evaluate the mobile health monitor and explore all of the above design principles. RESULTS: The experimental results show that the root mean square error of the SpO2 estimation is only 1.8, indicating the effectiveness of the system. CONCLUSION: These results indicate the effectiveness of the customized mobile SpO2 monitor and the selected design principles. SIGNIFICANCE: This research is expected to facilitate the continuous SpO2 monitoring of patients with clinical indications.

ABSTRACT

Foraging behavior of livestock in species-rich, less intensively managed grassland communities will require different methodologies from those appropriate in floristically simple environments. In this pilot study on sheep in species-rich grassland in northern Estonia, foraging behavior and the plant species of the immediate area grazed by the sheep were registered by continually-recording Go-Pro cameras. From three days of observation of five sheep (706 animal-minutes), foraging behavior was documented. Five hundred and thirty-six still images were sampled, and a plant species list was compiled for each. Each plant species was assigned a score indicating its location, in the ecophysiological sense, on the main environmental gradient. The scores of the plant species present were averaged for each image. Thus, the fine structure of foraging behavior could be studied in parallel with the vegetation of the precise area being grazed. As expected, there was considerable individual variation, and we characterized foraging behavior by quantifying the patterns of interspersion of grazing and non-grazing behaviors. This combination of behavior recording and vegetation classification could enable a numerical analysis of the responses of grazing livestock to vegetation conditions.

Threshold temperatures for cold stress in sheep are not well understood, the available literature is somewhat dated and reports relate to winter temperatures that are relatively benign. Sheep's preferences for outdoor versus indoor environments, when given free access to both, were investigated in the winter period at temperatures as low as -23 °C. Two sheep farms, one with access to a permanent uninsulated barn and one with a polytunnel shelter, both with free access to an outdoor area, were used. Observations were made with a camera positioned to register numbers of sheep outdoors and indoors, with one image taken hourly over twenty-four hours. The sheep clearly preferred to be outdoors; on all occasions the majority of the sheep were outdoors. There was, however, a significant decrease, albeit small, in the numbers of sheep choosing to be outdoors at lower temperatures (p < 0.001), higher relative humidity (p < 0.001) and greater wind chill (p < 0.001). Therefore, even at cooler temperatures than reported previously, sheep are motivated to be outdoors rather than indoors. It is not implicitly good for their welfare, and may not be true for lambs and shorn sheep, but accessing an outdoor area appears to be what they choose to do when given the choice.

ABSTRACT

If a perfusionist weans a patient off the heart lung machine (HLM) and the anesthesiologist has not re-started the ventilator, the patient will become hypoxic. The objective of this project was to create a redundant safety system of verbal and electronic communication to prevent failure to ventilate errors after cardiopulmonary bypass. This objective could be realized by building an electronic communication bridge directly between the HLM and ventilator. A software application was created to retrieve and interpret data from the pump and ventilator and trigger a programmed smart alarm. The software is able to interpret data from the pump and ventilator. When both are off simultaneously (defined as a pump flow of 0 L/min with a respiratory rate of 0 breaths/min), the application will raies an alarm. Communication between a pump and ventilator is possible, enabling the deployment of a safety system that could exist in the operating room (OR) as a standalone alarm. A device dataset can be used to optimize clinical performance of the alarm. The application could also be integrated into smart checklists and computer-assisted OR process models that are currently in development.

In a system in which cows are grouped and given differential access to feeding bins with different rations, and where these groups change over time, it is important to find out how a change in the ration (and hence feeding bin) affects the cow's feeding behaviour. Monitoring the locomotion of cows can be used to predict oestrus and improve health (lameness diagnosis), but activity monitors can also be used to estimate both activity and numbers of feeding visits by cows. Ice tag activity monitors were attached to the right hind legs of ten cows. Walking, standing, lying data and health records were used to record changes before and after a change in each cow's feeding bin. Results comparing activity before and after feeding bin change revealed significant increases in motion index, number of steps taken per minute and number of lying bouts per minute (all P < 0.001). Comparing the behaviours of cows subsequently followed during the dry period showed significant differences in motion indices and number of steps taken per minute (P < 0.001) in the dry period. The results indicate that cows are affected by feeding bin change and group change, which can lead to an increase in behaviour associated with the stress response, especially in heifers.

ABSTRACT

In the surgical setting, team members constantly deal with a high-demand operative environment that requires simultaneously processing a large amount of information. In certain situations, high demands imposed by surgical tasks and other sources may exceed team member's cognitive capacity, leading to cognitive overload which may place patient safety at risk. In the present study, we describe a novel approach to integrate an objective measure of team member's cognitive load with procedural, behavioral and contextual data from real-life cardiac surgeries. We used heart rate variability analysis, capturing data simultaneously from multiple team members (surgeon, anesthesiologist and perfusionist) in a real-time and unobtrusive manner. Using audio-video recordings, behavioral coding and a hierarchical surgical process model, we integrated multiple data sources to create an interactive surgical dashboard, enabling the analysis of the cognitive load imposed by specific steps, substeps and/or tasks. The described approach enables us to detect cognitive load fluctuations over time, under specific conditions (e.g. emergencies, teaching) and in situations that are prone to errors. This in-depth understanding of the relationship between cognitive load, task demands and error occurrence is essential for the development of cognitive support systems to recognize and mitigate errors during complex surgical care in the operating room.

ABSTRACT

Procedural flow disruptions secondary to interruptions play a key role in error occurrence during complex medical procedures, mainly because they increase mental workload among team members, negatively impacting team performance and patient safety. Since certain types of interruptions are unavoidable, and consequently the need for multitasking is inherent to complex procedural care, this field can benefit from an intelligent system capable of identifying in which moment flow interference is appropriate without generating disruptions. In the present study we describe a novel approach for the identification of tasks imposing low cognitive load and tasks that demand high cognitive effort during real-life cardiac surgeries. We used heart rate variability analysis as an objective measure of cognitive load, capturing data in a real-time and unobtrusive manner from multiple team members (surgeon, anesthesiologist and perfusionist) simultaneously. Using audio-video recordings, behavioral coding and a hierarchical surgical process model, we integrated multiple data sources to create an interactive surgical dashboard, enabling the identification of specific steps, substeps and tasks that impose low cognitive load. An interruption management system can use these low demand situations to guide the surgical team in terms of the appropriateness of flow interruptions. The described approach also enables us to detect cognitive load fluctuations over time, under specific conditions (e.g. emergencies) or in situations that are prone to errors. An in-depth understanding of the relationship between cognitive overload states, task demands, and error occurrence will drive the development of cognitive supporting systems that recognize and mitigate errors efficiently and proactively during high complex procedures.
