ABSTRACT

Introducción: Históricamente, la práctica de la medicina ha llevado implícito el principio de la beneficencia, que expresaba el bien del paciente, el bienestar y la satisfacción de sus intereses. Hoy resaltamos el principio de autonomía. Ha cambiado la relación del médico con el paciente, rompiéndose la verticalidad impositiva del médico para la horizontalidad democrática en la toma de decisiones sobre la salud de cada individuo Objetivos: Se describe una búsqueda exhaustiva de publicaciones de autonomía del paciente asociadas a ética, decisión y legislación de intervenciones sobre salud, publicados en el período 1985-2002.Material y método: Búsqueda de artículos de publicaciones de evaluación económica de intervenciones sobre salud en MEDLINE y EMBASE entre 1985-2002. Estudio de fiabilidad interobservadores e intraobservador de la estrategia de selección y clasificación. Análisis descriptivo de las publicaciones (tablas de frecuencia).Resultados: Las publicaciones seleccionadas de autonomía del paciente asociadas a decisión, ética, legislación y coste-beneficio de intervenciones sobre salud englobaban: 2.626 estudios sobre decisión aplicada y teoría de la decisión (32,22 por ciento), 2.379 estudios de ética (29,19 por ciento), 2.270 artículos de legislación (27,85 por ciento), y 873 artículos de costebeneficio (10,71 por ciento). Un 1,76 por ciento fueron excluidas. El acuerdo interobservadores en la selección fue de 0,81; en la clasificación fue 0,66.Conclusión: La autonomía da autoridad al paciente para tomar sus propias decisiones y es el que hoy tiende a prevalecer en la relación médico-paciente, en detrimento de la beneficencia hipocrática. Así, el desprecio a la autonomía representa, una violación a los derechos del paciente y una transgresión ética y legal. (AU)

ABSTRACT

OBJECTIVE: To demonstrate that the categorization of different therapeutic options is essential for the decision-making applied to clinical management. METHOD: This paper compares the usefulness of percutaneous echo-guided cholecystostomy as the first therapeutic option in acute long-term cholecystitis versus the exclusive surgical or clinical management. RESULT: We have analyzed the usefulness of different therapeutic options in a case of long term acute cholecystitis and we have obtained the following results: isolated intravenous antibiotic therapy (0.76), surgery (0.73) and echo-guided cholecystostomy (0.93). This last option has provided good clinical-radiological results. CONCLUSION: Decision trees are a good methodological option as support of surgical and non surgical therapeutic procedures.

ABSTRACT

Objetivo: demostrar que la categorización de las diferentes opciones terapéuticas, es esencial en la aplicación clínica de la teoría de la decisión. Método: este artículo balancea la utilidad de la colecistostomía percutánea eco-dirigida como primera opción terapéutica en la colecistitis aguda de larga evolución frente al tratamiento quirúrgico o médico exclusivos. Resultados: del cálculo de las utilidades de las diferentes opciones terapéuticas ante un caso de colecistitis aguda evolucionada hemos obtenido los siguientes resultados: tratamiento médico exclusivo (0,76), intervención quirúrgica (0,73) y radiología intervencionista (0,93). Ha sido esta última opción, la que ha aportado una buena evolución clínico-radiológica. Conclusiones: los árboles de decisión representan una herramienta metodológica útil como soporte en la selección de procedimientos terapéuticos quirúrgicos o no quirúrgico (AU)

ABSTRACT

The aim of this work is to carry out a follow-up of oncological inpatients who received parenteral nutrition (PN). We analysed the connection between the clinical situations and the indication and type of PN administered. A retrospective review of oncological patients who received PN during 1996 was carried out. Age, sex, primary tumor, PN indication and compliance degree, nutritional evolution, different categories of nutritional intervention, characteristics of PN as well as related complications and connected with PN were analysed. 33 patients, 23 female and 10 male, between 20 and 82 years old, were studied. 39% received PN with intensive antineoplastic therapy; 55% received PN as supportive treatment in various clinical situations related to the tumor or with antineoplastic therapy; 6% received palliative PN. 25 patients received PN without lipids and 8 total PN. The mean duration of PN was 10 days. 17 patients received PN by peripheric venous route and 16 through central venous catheter. The PN was well tolerated. In conclusion, most patients (94%) complied with some of the criteria established for the PN indication. The biggest percentage of patients in treatment with PN were divided into two main groups: patients with intensive antineoplastic therapy and patients with gastrointestinal disfunction caused by tumor or antineoplastic therapy. The role of PN in oncological patients it is not defined yet. The incidence of complications due to PN was low and without clinical relevance.

The purpose of this annotation is to review the serological data to diagnosis, severity of the disease, treatment and prognosis of the autoimmune haemolytic anemia (AIHA) associated with carcinoma. A patient with AIHA and larynx carcinoma is presented.
