ABSTRACT
Main principles which should be followed when choosing the strategy of treatment in patients with deformities of lower extremities are describes. The authors' experience includes 213 patients with deformities of lower extremities, 59 of them were operated upon. Basing on this experience the authors have developed a classification allowing systematization of the methods for correction of deformities of legs. The following factors were taken into account: the character and degree of the deformity, the degree of impairment of the joint functions, compensatory changes, the degree of alteration of soft tissues, patient's age and the esthetic effect estimated by the patient.

Clinical and experimental researches were conducted to study the possibilities for enhancing the efficiency of treatment of open composite comminuted and crushed fractures of long bones with the help of repercussive action of electrets. The results of roentgenomorphometric, clinical and biomechanical researches have shown the efficiency of electrets that could shorten the healing time of the infected wounds, stop the inflammatory reaction and promote the repairing process of bones. The application of electrets using the repercussive methods in complex treatment of complicated open fractures in cases with multiorgan and concomitant injuries creates favourable conditions for the healing process. This method is simple, atraumatic and economical.

With the goal of improvement of intramedullary osteosynthesis efficiency in treatment of composite and associated fractures of long bones by way of improvement of fixatives according to their electrical properties and optimization of reparative electrogenesis conditions in 72 experiments with animals the effect of electro-conductive properties and electrostatic field of intra-bone implanted rods on the osteo-reparation following transverse osteotomy has been studied, the method has been elaborated and on the basis of dynamic analysis of 147 clinical observations has been given comparative evaluation of the results of polytetrafluorethylene intramedullary osteosynthesis. The advantages ethylene-based electrets application in case of intramedullary osteosynthesis. The advantages of electret application for activation of osteo-reparation, improvement of the results of treatment and reduction of terms of rehabilitation of victims with critical traumata have been demonstrated.

The authors substantiate the expediency of improving osteosynthesis with apparatuses of perosseous external fixation on the basis of electromechanical principles making use of statistically reliable results of 58 chronic experiments on animals, 27 stand studies on the tibia of cadavers and 207 dynamic clinical observations employing quantitative results of clinical, roentgenomorphometric, electrophysiologic, biomechanical and histomorphological studies. Optimal parameters of the rigidity of fixation for early restoration of the properties of elasticity and strength of the injured bone are determined. Dampered external osteosynthesis in the treatment of open comminuted fractures of the bones of the crus reduced the mean periods of in-patient treatment and consolidation of the fractures by 27-34. 5% and the frequency of complications and unsatisfactory outcomes by 75%.
