ABSTRACT
A 37-year-old man, treated case of left temporal glioblastoma presented with headache, seizures, and progressive right-sided weakness with MRI evidence of recurrence. Exploratory Ga-PSMA PET/CT demonstrated PSMA expression in the recurrent lesion; it was decided to treat this patient with Lu-PSMA-617. After 3 cycles of Lu-PSMA-617, Ga-PSMA PET/CT showed significant reduction in PSMA uptake and regression in size of lesion on MRI with improvement in patient's symptoms and performance status. Lu-/Ga-PSMA theranostics has potential in patients with recurrent glioblastoma multiforme when other therapeutic options are not feasible.

We describe the extrastriatal accumulation of dopamine transporter imaging agent Tc-TRODAT in the peri-infarct area in a 75-year-old man referred for dopamine transporter SPECT imaging with a suspicion of idiopathic Parkinson disease.

Primary cardiac tumors are rare with angiosarcoma being the most common among malignant cardiac tumor. We present a case of 30-year-old female patient in whom F-18-fluorodeoxyglucose positron emission tomography/computed tomography demonstrated a necrotic mass in right atrium with multiple fluorodeoxyglucose avid lesions in both upper and lower alveolus, liver, multiple bones, and bilateral lungs. Patient underwent biopsy from gum swelling which revealed metastatic angiosarcoma.

Paraneoplastic neurological syndromes (PNSs) are commonly encountered with underlying malignant pathology. Though anti--neuronal antibodies play a major role in the diagnosis of the underlying malignant pathology but at many times it becomes inconclusive. As early detection of the primary cause and its treatment gives the best result in such situations, there arises an early and accurate diagnostic need. We present a 65--year--old patient presenting with rapidly progressive quadriparesis with both distal and proximal involvement. With all routine work--up tests within normal limits, 18F-fluorodeoxyglucose positron emission tomography/computed tomography (PET/CT) was done which revealed multiple findings that suggested a diagnosis of lymphoma. In our case, PET/CT proved to be an important modality for finding the underlying malignant pathology in a suspected case of PNS.
