ABSTRACT

In the surgical clinic of Nizhny Novgorod medical academy and in specialized cardiosurgical hospital, from January 1979 to January 1999 the reconstructive operations in all arterial segments were performed in 1078 patients, 1238 dura mater's allogenic transplants of various length and shape were used. In 1072 patients the side plasty from 2 to 19 cm was performed, in 6 patients--arteries grafting with sagittal sinus. Positive immediate results in 971 (93.4%) patients were achieved. The absence of surgical effect or unsatisfactory outcome occurred in 69 (6.6%). 38 patients died in early postoperative period, total lethality was 3.5%. Systemic and local complications were seen in 131 (12.6%) patients. Long-term results in the period from 12 months to 19 years were studied in 683 patients (65.7% of all discharged patients). The results after operations with use of explants and dura mater were better than after endarterectomy and autovenous plasty.

ABSTRACT

The authors made an analysis of ultrasonic flowmetry in 36 operations of carotid endarterectomy in 22 patients with stenosis of brachiocephalic arteries. After removal of atherosclerotic plaques the volumetric blood flow in carotid arteries increased from 65-260 ml/min up to 210-400 ml/min. No substantial difference was noted between the increase of blood flow after classical endarterectomy and after the formation of a new carotid bifurcation. Flowmetry allows the estimation of adequacy of vascular reconstruction and the choice of correct management of the patients at the postoperative period.

ABSTRACT

The authors have analyzed results of using dura mater as a plastic material in 1078 patients with occluding diseases of the aorta and major arteries. Immediate positive results were noted in 93.4% of the patients. Long-term results from 12 months to 19 years were studied in 683 patients.

ABSTRACT

Operations were performed on 262 patients with occlusive lesions of the abdominal aorta and major arteries. In 41 patients a lateral venous autograft was applied after endarterectomy, in 42 an autovenous shunt was created after open endarterectomy, 26 patients underwent eversion endarterectomy. Prosthetic alloplasty of the aorta and major arteries was conducted on 63 patients and an allograft shunt was formed in 41. Lumbar sympathectomy was carried out on all patients as a rule after reconstructive-restorative operations. Nineteen patients died after the operation. Complications developed in the postoperative period in 41 patients. Marked improvement or the disappearance of the symptoms of the disease was noted in the immediate postoperative period in 231 patients (95%).
