ABSTRACT

The activity of vegetative reception, immunological reactivity, and glucocorticoid provision was studied in 196 patients with bronchial asthma who were admitted for operative treatment. The authors devised the principle of the stage-by-stage impairment of the activity of vegetative reception, immunological reactivity, and glucocorticoid provision during the formation of bronchial asthma and distinguished six types of neuroimmune interrelations. On the basis of these six types they suggest pathogenetically substantiated indications for various methods of surgical treatment of patients with bronchial asthma.

ABSTRACT

The authors discuss the late-term results of operative treatment of 115 patients with severe forms of bronchial asthma in 6 months to 5 years after surgery with due regard for the adaptation reactions of the immune, sympathoadrenal, and hypophyseal-adrenal systems. The high efficacy of surgical treatment of bronchial asthma was confirmed, particularly of one-stage combined operations on various links of the autonomic nervous system which were introduced into practice for the first time. It is shown that operative treatment allows the adaptation status of the patients to be raised, which ensures a positive clinical effect. The role of immunocorrection in increasing the efficacy of surgical treatment is shown. The causes of the poor results of treatment were studied.

ABSTRACT

Immunological reactivity, sympathetic-adrenal+ and pituitary-adrenal systems were studied in 758 patients with bronchial asthma. The results served the basis for construction of 4 adaptogram types which reflect the status of adaptation. Therapeutic and surgical indications are suggested for bronchial asthma sufferers allowing for the relationship between standard conservative treatment, modification of immunity, various surgical interventions and adaptation status.

ABSTRACT

The article analyses 1,000 cases with unspecific spontaneous pneumothorax, 512 (51.2%) patients were treated by operation. Thoracoscopy is of principal importance in establishing the diagnosis; it makes it possible to identify the cause of spontaneous pneumothorax and determine the indications for operative treatment. The choice of economical resection of the bullous part of the pulmonary tissue is most important from the standpoint of principle, it allows radical treatment of the patient and prevention of the advancement of lung dystrophy with good occupational rehabilitation, especially among the young contingent of patients, which is important socially also.

ABSTRACT

Indications for various types of operative intervention in 411 patients (among 986 patients who were examined in a specialized pulmonological department) were determined according to the degree of functional and morphological changes of the respiratory tract by means of complex radiological examination together with other modern methods (radioisotope, immunological, bronchological, etc.) for examination of the bronchopulmonary system in bronchial asthma.
