ABSTRACT

A 51-year-old woman with breast cancer metastatic to the pleura was treated with intrapleural instillation of adriamycin after thoracentesis, and systemic administration of FEMP, with complete response for more than 11 months. She underwent radical mastectomy and bilateral oophorectomy for advanced breast cancer, and 18 months later presented radiological evidence of pleural fluid contralateral to the affected breast. Initially, thoracentesis for symptomatic relief was done with a positive cytology in the pleural effusion. Intrapleural instillation of adriamycin was followed by partial resolution, and administration of FEMP achieved complete disappearance of both pleural metastasis and pleural effusion.

ABSTRACT

A rare case of massive gastrointestinal bleeding from ileal varices was reported. On December 28, 1982, a 49-year-old male was admitted to Kurashiki Central Hospital because of massive gastrointestinal bleeding. Eleven years ago, the patient underwent an emergency operation for rupture of esophageal varices. The venous phase of selective superior mesenteric and celiac angiography showed mesenteric varices, but no definite bleeding point was noted. Endoscopy revealed esophageal varices, but no area of bleeding was encountered. Because of frequently repeated hemorrhages, laparotomy was performed. A large vein was found on the surface of the ileal wall, through an adhesion to the lateral pelvic wall. Partial resection of ileum was performed. Histological examination of the resected small bowel demonstrated ruptured submucosal varices. The postoperative course was uneventful and he was discharged on the 40th postoperative day. It is stressed that this disease should be considered as a differential diagnosis for a patient with lower gastrointestinal bleeding and portal hypertension.

ABSTRACT

Intra-arterial infusion chemotherapy was performed in 50 patients with cancer of the liver including 12 primary hepatoma and 38 metastatic cancers, at Kurashiki Central Hospital from December 1977 through December 1981. B.D. Fomocath 5 French catheter was inserted through the lateral femoral circumflex artery and advanced retrogradely to the common hepatic or proper hepatic artery using a loop catheter technique and the catheter was connected to the tube of infusion pump. The dose of 5-Fluorouracil and Mitomycin C was 250 mg per day and 4-10 mg once a week, respectively. The patients had average survival of 8.2 months in hepatoma and 4.8 months in metastatic tumor. The results were unfortunately not encouraging but this regimen occasionally provides excellent results and has no contraindication.

ABSTRACT

A 66-yr-old woman had several attacks resembling relapsing acute pancreatitis with elevation of serum amylase for about three months. Obstruction of the main pancreatic duct was detected with endoscopic retrograde pancreatoductography, though no abnormality was revealed on celiac and mesenteric arteriograms. At that time she had no jaundice. Laparotomy revealed a walnut-sized tumor in the head of the pancreas compressing the main pancreatic duct. Pancreaticoduodenectomy was performed. The pathological diagnosis of the tumor was well-differentiated papillary cysto-adenocarcinoma of the pancreas. She had no attack of hyperamylasemia since the operation, and is well 2 years after the surgery. Close attention to persistent elevation of pancreatic amylase levels in serum and urine would enhance the chance of detection of small pancreatic cancers before the development of jaundice.

ABSTRACT

603 schoolchildren of the greater Athens area were screened for psychiatric disorders as expressed by a total score of 13 or more in the Rutter parents' questionnaire, and 10 or more in the Rutter teachers' questionnaire. Psychiatric disorder was negatively correlated with school marks. IQ (vocabulary, draw-a-man, visual-motor integration), motor coordination, and achievement in reading and arithmetic, and positively with size of family. The incidence of disturbance as assessed by the teachers' questionnaire was similar to that in other big cities in other countries, while the incidence of disturbance as assessed by the parents' questionnaire was greater in comparison to that of other countries. The incidence of hyperkinesis in our sample was considerably greater in comparison to other studies in other countries. Boys were found to be generally more disturbed than girls with the exception of neurosis.
