ABSTRACT

Clostridial infections, putrid infections with aerobic and anerobic growing germs, air forced into the tissue during the primary trauma and the formation of gas by contact of the wound with aluminium, H2O2 and gasoline may be causes for the formation of gas and oedema in the tissues. Only infections with Clostridia are gas gangrene. We must differentiate the clostridial cellulitis from the clostridial myositis. Bacterioscopy allows a rapid differentiation to be made between putrid and clostridial infection. Beside intensive care and antibiotics, putrid infections demand an early extensive incision, for gas gangrene the radical excision of the damaged tissue is required. The effect of hyperbaric oxygenation is still under discussion. It can never replace surgical treatment.

ABSTRACT

In 895 patients operated on for acute appendicitis we found Yersinia enterocolitica in 41 cases. Another two cases were detected in postoperative serological examinations. Lymphadenitis mesenterialis was present in 17 cases during operation. Once we detected a regional enteritis. Histological examination of the appendix revealed a purulent inflammation in 21 cases and a phlegmonous-gangrenous inflammation in 4 cases. The other 18 patients suffered from Oxyuriasis, coprostasis or scars of the appendix. For this reason it is impossible to exclude an acute appendicitis in patients with cultural or serological identification of Yersinia enterocolitica.

ABSTRACT

The infarction of the right ventricle as a spatial and temporal mosaic-infarct and the lesions following it were investigated. Its frequency and its connections with the infarction of the left ventricle, coronary sclerosis and hypertrophy of the right ventricle as cor mitrale and cor pulmonale were reported, treated on 11 073 post-mortem examinations of adults separated in males and females. Three groups were divided: 1. isolated lesions of the right ventricle 2. separated lesions--spatially and/or temporally of the right and left ventricle 3. lesions overlapping from the left on the right ventricle. The infarct of the right ventricle respectively cicatrices were observed in 1.2%. The coronary sclerosis was detected in all cases, and specially the hypertrophy of the right ventricle as cor pulmonale or cor mitrale were important for the pathogenesis of right-cardiac infarcts. The pulmonary embolism was more frequent in isolated infarcts of the right ventricle.
