ABSTRACT

BACKGROUND: The demand for kidney retransplantation following graft failure is rising. Repeat transplantation is often associated with poorer outcomes due to both immunological and surgical challenges. The aim of this study was to compare surgical and functional outcomes of kidney retransplantation in recipients that had previously had at least two kidney transplants with a focus on those with antibody incompatibility. METHODS: We analyzed 66 patients who underwent renal transplantation at a single center between 2003 and 2011. Consecutive patients receiving their 3rd or 4th kidney were case-matched with an equal number of 1st and 2nd transplants. RESULTS: Twenty-two 3rd and 4th kidney transplants were matched with 22 first and 22 seconds transplants. Operative times and length of stay were equivalent between the subgroups. Surgical complication rates were similar in all groups (22.7% in 1st and 2nd transplants, and 27.2% in 3rd/4th transplants). There was no significant difference in patient or graft survival over 5 years. Graft function was similar between transplant groups at 1, 3, and 5 years. CONCLUSIONS: Third and fourth kidney transplants can be performed safely with similar outcomes to 1st and 2nd transplants. Kidney retransplantation from antibody-incompatible donors may be appropriate for highly sensitized patients.

ABSTRACT

AIM: To examine whether the association between breastfeeding and childhood cognitive development is direct or whether the association is spurious owing to confounding variables. METHOD: The current study conducted a propensity score matching (PSM) analysis using data from the Early Childhood Longitudinal Study, Birth Cohort. RESULTS: The results of PSM models revealed that prior to matching, the association between breastfeeding and cognitive functioning was significant (mean difference = 3.20, t-value = 7.86, 95% confidence interval (CI) = 2.40-4.00, P ≤ 0.05). After controlling for a range of confounding factors, the mean difference was reduced by 40% (mean difference = 1.92, t-value = 3.75, 95% CI = 0.92-2.93, P ≤ 0.05); however, significant differences remained between groups. CONCLUSION: The results produced in the current study provide tentative evidence that breastfeeding may have an influence on the development of cognitive abilities in children. Additional research is necessary, however, in order to further evaluate the causal impact and exact developmental pathway of breastfeeding on childhood cognitive growth.

ABSTRACT

A line of research has revealed that the influence of genes on behavioral development is closely tied to environmental experiences. Known as gene-environment interaction, research in this area is beginning to reveal that variation in parenting behaviors may moderate genetic influences on antisocial behaviors in children. Despite growing interest in gene-environment interaction research, little evidence exists concerning the role of maternal disengagement in the conditioning of genetic influences on childhood behavioral problems. The current study is intended to address this gap in the literature by analyzing a sample of twin pairs drawn from the Early Childhood Longitudinal Study, Birth Cohort (ECLS-B). Analysis of the ECLS-B provided evidence that maternal disengagement moderates genetic influences on the development of externalizing problems.

ABSTRACT

An all solid-state Ti:sapphire laser differential absorption lidar transmitter was developed. This all-solid-state laser provides a compact, robust, and highly reliable laser transmitter for potential application in differential absorption lidar measurements of atmospheric ozone. Two compact, high-energy-pulsed, and injection-seeded Ti:sapphire lasers operating at a pulse repetition frequency of 30 Hz and wavelengths of 867 and 900 nm, with M2 of 1.3, have been experimentally demonstrated and their properties compared with model results. The output pulse energy was 115 mJ at 867 nm and 105 mJ at 900 nm, with a slope efficiency of 40% and 32%, respectively. At these energies, the beam quality was good enough so that we were able to achieve 30 mJ of ultraviolet laser output at 289 and 300 nm after frequency tripling with two lithium triborate nonlinear crystals.

ABSTRACT

Two compact, high-pulse-energy, injection-seeded, 30-Hz frequency-doubled Nd:YAG-laser-pumped Ti: sapphire lasers were developed and operated at infrared wavelengths of 867 and 900 nm. Beams with laser pulse energy >30 mJ at ultraviolet wavelengths of 289 and 300 nm were generated through a tripling of the frequencies of these Ti:sapphire lasers. This work is directed at the replacement of dye lasers for use in an airborne ozone differential absorption lidar system. The ultraviolet pulse energy at 289 and 300 nm had 27% and 31% absolute optical energy conversion efficiencies from input pulse energies at 867 and 900 nm, respectively.
