ABSTRACT

Purpose: To characterize corneal biomechanical properties using the CORVIS-ST device in myopic individuals. Methods: This prospective cross-sectional study included patients with myopia. Our study included 154 eyes of 154 myopic patients aged between 18 and 40 years, with stable refraction for at least 2 years. A full ophthalmological examination and corneal tomography were performed using a Pentacam HR device. Corneal biomechanical parameters were assessed using the CORVIS-ST device in mild, moderate, severe, and extreme myopia groups. Results: Statistically significant differences were observed in the DA ratio (p = 0.033), SP-A (p=0.009), CBI (p=0.041), SSI (p=0.000), and Peak distance (p = 0.032). In correlation with different Corvis ST biomechanical variables, SE was found to be correlated with DA ratio(r=-0.191, p=0.018), SP-A(r=0.199, p=0.013) and SSI(r=-0.336, p=0.000), while in multiple regression analysis, SE was found to be independently correlated with SSI and peak distance(p=0.036,0.038 respectively) while the grade of myopia was found to be independently correlated with SP-A(p=0.034). Conclusion: SSI, Peak distance, and SP-A were independently related to SE and myopia grade, confirming the hypothesis that eyes with higher myopia are more deformable and less stress resistant.

ABSTRACT

OBJECTIVES: To evaluate the use of peripapillary microperimetry in the diagnosis and follow-up of medically and surgically treated cases of papilledema due to idiopathic intracranial hypertension (IIH). METHODS: This study was a prospective non-comparative observational case series of patients with IIH. All patients underwent full ophthalmological examination, radiological imaging, and lumbar puncture, as well as microperimetric measurements around the optic nerve head. Patients were classified into a medical group, treated by weight reduction and acetazolamide, and a surgical group, treated by optic nerve sheath fenestration. Peripapillary microperimetric examinations were done at presentation and after one-month of treatment. Findings were compared to values obtained from an age-matched healthy control group. RESULTS: The study included 78 eyes of 39 patients diagnosed with IIH and 32 eyes of 16 healthy control subjects. The average of the sum of the 12 peripapillary microperimetric thresholds for both eyes of IIH cases before treatment was 53.4±59.9 dB, while for controls it was 171±14.0 dB (p<0.001). After 1 month of treatment, it improved to 86.7±56.1 dB (p<0.001) in IIH cases. Analyzed separately, the medical group improved from 68.9±43.4 dB to 105.2±76.5 dB (p<0.001), while the surgical group improved from 13.5±21.9 dB to 54.9±35.5 dB (p<0.001). Several potential roles for peripapillary microperimetry use in IIH management were identified including confirming papilledema diagnosis, follow up of treatment, and selection of cases for surgery. CONCLUSION: Microperimetry can accurately and quantitatively monitor changes in peripapillary retinal sensitivity in patients undergoing treatment for papilledema due to IIH.

ABSTRACT

Purpose: To describe the clinical experience with intermediate uveitis at six Egyptian tertiary eye centers.Methods: A multicenter retrospective chart review of all patients with intermediate uveitis seen at six ocular inflammation referral clinics in Egypt between January 2010 and January 2017.Results: The study included a total of 781 patients with intermediate uveitis. The study cohort comprised of 282 male and 499 female patients. In over half of our cohort (58.77%), no specific cause could be confirmed. The remaining patients had sarcoidosis (16.26%), tuberculosis (14.85%), multiple sclerosis (9.09%), and TINU (1.02%). By the end of our study, 62% of the affected eyes had a BCVA better than 20/40Conclusion: More than 40% of our patients with intermediate uveitis had sarcoidosis, tuberculosis, multiple sclerosis, or TINU as the underlying etiology. Owing to their potential morbidity, these diseases need to be considered in Egyptian patients presenting with intermediate uveitis.
