ABSTRACT

Fundamento en la medicina china, los accidentes cerebrovasculares se relacionan con el síndrome de golpe de viento (Zhong Feng), lo cual se corresponde en la medicina occidental con los accidentes cerebrovasculares hemorrágicos e isquémicos. Objetivo caracterizar a los pacientes con accidentes cerebrovasculares en fase aguda según diagnóstico de la medicina china. Métodos estudio descriptivo, de serie de casos (n=40), que incluyó a pacientes atendidos en el periodo marzo-diciembre de 2018 en el Hospital Dr. Gustavo Aldereguía Lima, de Cienfuegos. Se analizaron las variables: edad, sexo, tipo de enfermedad cerebrovascular en fase aguda, gravedad del ataque del golpe de viento, síndrome según órganos y vísceras, y factor patógeno presente. Resultados predominaron los accidentes cerebrovasculares trombóticos, de los cuales 75,0 % se comportaron como golpe de viento graves tipo flácido; y el 81,3 % estuvo asociado al síndrome de insuficiencia de sangre de hígado. Los embólicos: ataques leves en el 33,3 %; y 18,8 % síndrome de insuficiencia de sangre de hígado. Las hemorragias cerebrales intraparenquimatosas: 62,5 % ataque grave tipo tenso; y 71,4 % fuego de hígado. Hemorragia subaracnoidea: 15 % como ataque grave tipo flácido; y 17,6 % síndrome de insuficiencia de Yin de hígado. El factor patógeno más frecuente resultó el viento interno, presente en 90,0 % de los casos. Conclusiones el análisis de los pacientes con accidente cerebrovascular en fase aguda según diagnóstico tradicional chino, mostró que existe relación clínica con el síndrome golpe de viento y de los Zhang fu de la medicina china.


Foundation In Chinese medicine, strokes are related to wind stroke syndrome (Zhong Feng), which corresponds in Western medicine to hemorrhagic and ischemic strokes. Objective to characterize patients with strokes in the acute stage according to Chinese medicine diagnosis. Methods descriptive case series study (n=40), which included patients treated from March to December 2018 at the Dr. Gustavo Aldereguía Lima Hospital, Cienfuegos. The analyzed variables were: age, sex, type of cerebrovascular disease in the acute stage, severity of the wind stroke attack, syndrome according to organs and viscera, and pathogenic factor present. Results thrombotic strokes predominated, of which 75.0% behaved as severe flaccid type wind stroke; and 81.3% were associated with liver blood failure syndrome. Embolics: mild attacks in 33.3%; and 18.8% liver blood failure syndrome. Intraparenchymal cerebral hemorrhages: 62.5% severe tense attack; and 71.4% liver fire. Subarachnoid hemorrhage: 15% as severe flaccid type attack; and 17.6% liver Yin deficiency syndrome. The most frequent pathogenic factor was internal wind, present in 90.0% of cases. Conclusions the analysis of patients with stroke in the acute phase according to traditional Chinese diagnosis showed that there is a clinical relationship with wind stroke syndrome and the Zhang fu of Chinese medicine.

ABSTRACT

The Magellanic sub-Antarctic Forest is home to the world's southernmost avian community and is the only Southern Hemisphere analogue to Northern Hemisphere temperate forests at this latitude. This region is considered among the few remaining pristine areas of the world, and shifts in environmental conditions are predominantly driven by climate variability. Thus, understanding climate-driven demographic processes is critical for addressing conservation issues in this system under future climate change scenarios. Here, we describe annual survival patterns and their association with climate variables using a 20-year mark-recapture data set of five forest bird species in the Cape Horn Biosphere Reserve. We develop a multispecies hierarchical survival model to jointly explore age-dependent survival probabilities at the community and species levels in a group of five forest passerines. At the community level, we assess the association of migratory behavior and body size with survival, and at the species level, we investigate the influence of local and regional climatic variables on temporal variations of survival. We found a positive effect of precipitation and a negative effect of El Niño Southern Oscillation on juvenile survival in the white-crested Elaenia and a consistent but uncertain negative effect of temperature on survival in juveniles and 80% of adults. We found only a weak association of climate variables with survival across species in the community and no temporal trends in survival for any of the species in either age class, highlighting apparent stability in these high austral latitude forests. Finally, our findings provide an important resource of survival probabilities, a necessary input for assessing potential impacts of global climate change in this unique region of the world.

ABSTRACT

Fundamento: los pacientes con diabetes mellitus cursan con un aumento importante en el estrés oxidativo, donde hay una disminución en la actividad de los sistemas antioxidantes y un incremento de los productos de oxidación. El ozono actúa induciendo estrés oxidativo moderado y estimulando una defensa antioxidante enzimática. Objetivo: caracterizar el control metabólico de los pacientes diabéticos al inicio y al culminar la terapia con ozono rectal. Métodos: se realizó un estudio cuasi experimental con pre y post terapia, donde el universo estuvo constituido por 121 pacientes diabéticos del consultorio médico de familia, número 15 del Policlínico Comunitario Docente José Luis Chaviano de Cienfuegos aplicándoles ozonoterapia por vía rectal previa evaluación médica, por 15 sesiones dos veces al año. Fueron recogidas como variables demográficas: edad y sexo; clínicas: parámetros hemoquímicos, tiempo de evolución de la enfermedad y estado nutricional; como complicaciones neurovasculares: pie diabético, polineuropatía diabética y retinopatía diabética; tratamientos: hipoglucemiantes; factores de riesgo modificables: hipertensión arterial, diabetes mellitus, tabaquismo, alcoholismo, realización de ejercicio físico; cálculo del índice de masa corporal. Los resultados hallados fueron expresados fundamentalmente mediante tablas y gráficos como principales recursos del lenguaje estadístico. Resultados: los pacientes con diabetes mellitus tipo 2 predominaron con un 95,0 %. Existió una disminución de 2, 769 mmol/L de las cifras de glucemia después del tratamiento con ozono rectal lo que muestra un control metabólico. Conclusiones: la ozonoterapia rectal puede ser utilizada como terapia complementaria al tratamiento convencional de pacientes con diabetes mellitus, pues mejora los parámetros de glucemia.


Background: patients with diabetes mellitus have a significant increase in oxidative stress, where there is a decrease in the activity of antioxidant systems and an increase in oxidation products. Ozone acts by inducing moderate oxidative stress and stimulating an enzymatic antioxidant defense. Objective: to characterize the metabolic control of diabetic patients at the beginning and at the end of rectal ozone therapy. Methods: a quasi-experimental study was carried out with pre and post therapy, where the universe consisted of 121 diabetic patients from the family doctor's office, number 15 of the José Luis Chaviano Community Teaching Polyclinic in Cienfuegos, applying rectal ozone therapy prior medical evaluation for 15 sessions twice a year. Demographic variables were collected: age and sex; clinics: hemochemical parameters, time of evolution of the disease and nutritional status; as neurovascular complications: diabetic foot, diabetic polyneuropathy and diabetic retinopathy; treatments: hypoglycemic; modifiable risk factors: arterial hypertension, diabetes mellitus, smoking, alcoholism, physical exercise; calculation of body mass index. The results found were expressed mainly through tables and graphs as the main resources of the statistical language. Results: patients with type 2 diabetes mellitus predominated with 95.0 %. There was a 2,769 mmol/L decrease in blood glucose levels after treatment with rectal ozone, which shows metabolic control. Conclusions: rectal ozone therapy can be used as a complementary therapy to the conventional treatment of patients with diabetes mellitus, since it improves glycemia parameters.

ABSTRACT

We describe a new taxon of terrestrial bird of the genus Aphrastura (rayaditos) inhabiting the Diego Ramírez Archipelago, the southernmost point of the American continent. This archipelago is geographically isolated and lacks terrestrial mammalian predators as well as woody plants, providing a contrasted habitat to the forests inhabited by the other two Aphrastura spp. Individuals of Diego Ramírez differ morphologically from Aphrastura spinicauda, the taxonomic group they were originally attributed to, by their larger beaks, longer tarsi, shorter tails, and larger body mass. These birds move at shorter distances from ground level, and instead of nesting in cavities in trees, they breed in cavities in the ground, reflecting different life-histories. Both taxa are genetically differentiated based on mitochondrial and autosomal markers, with no evidence of current gene flow. Although further research is required to define how far divergence has proceeded along the speciation continuum, we propose A. subantarctica as a new taxonomic unit, given its unique morphological, genetic, and behavioral attributes in a non-forested habitat. The discovery of this endemic passerine highlights the need to monitor and conserve this still-pristine archipelago devoid of exotic species, which is now protected by the recently created Diego Ramírez Islands-Drake Passage Marine Park.

ABSTRACT

A natural laboratory is a place supporting the conditions for hypothesis testing under non-anthropogenic settings. Located at the southern end of the Magellanic sub-Antarctic ecoregion in southwestern South America, the Cape Horn Biosphere Reserve (CHBR) has one of the most extreme rainfall gradients in the world. Subject to oceanic climate conditions, it is also characterized by moderate thermal fluctuations throughout the year. This makes it a unique natural laboratory for studying the effects of extreme rainfall variations on forest bird communities. Here, we monitor the bird species richness in the different forest types present in the CHBR. We found that species richness decreased with increasing precipitation, in which an increase of 100 mm in average annual precipitation showed about 1% decrease in species richness. Similar patterns were found among different forest types within the CHBR. These results provide a baseline to investigate the interactions between physical and biotic factors in a subpolar region that climatically contrasts with boreal forests, which is subject to continental climatic conditions. This research highlights the importance of ecological and ornithological long-term studies in the CHBR, which can contribute both to a higher resolution of the heterogeneity of climate changes in different regions of the world, and to orient conservation policies in the Magellanic sub-Antarctic ecoregion in the face of growing development pressures.

Latitudinal gradients are well-suited systems that may be helpful explaining distribution of haemosporidian parasites and host susceptibility. We studied the prevalence, diversity and drivers of haemosporidian parasites (Leucocytozoon, Plasmodium and Haemoproteus) along a latitudinal gradient (30°-56° S), that encompass the total distribution (~3,000 km) of the Thorn-tailed Rayadito (Aphrastura spinicauda) in the South American temperate forests from Chile. We analyzed 516 individuals from 18 localities between 2010 and 2017 and observed an overall prevalence of 28.3% for haemosporidian parasites. Leucocytozoon was the most prevalent parasite (25.8%). We recorded 19 distinct lineages (13 for Leucocytozoon, five for Plasmodium, and one for Haemoproteus). Differences in haemosporidian prevalence and diversity by genus and type of habitat were observed in the latitudinal gradient. Further, we support the existence of a latitudinal associate distribution of Leucocytozoids in South America, where prevalence and diversity increase toward higher latitudes. Distribution of Leucocytozoon was associated with sub-antarctic habitat (higher latitude) and explained by cold temperature and high precipitation. On the other hand, we lacked to find a latitudinal associate pattern for Plasmodium and Haemoproteus, however low prevalence and high diversity were recorded in areas considered as a hotspot of biodiversity in Central Chile. Our findings confirmed the importance of habitat and climatic variables explaining prevalence, diversity and distribution of haemosporidian parasites in a huge latitudinal gradient, belonging the distribution of the Thorn-tailed Rayadito in the world's southernmost forests ecosystems.

ABSTRACT

Muchos son los reportes acerca de los beneficios de la ozonoterapia en múltiples enfermedades, sin embargo, no existe reporte de caso sobre su uso en el hipotiroidismo. Por tal motivo el presente artículo tiene como objetivo evaluar los beneficios de la ozonoterapia en ese padecimiento, tan frecuente en nuestro medio, que causa múltiples trastornos y síntomas. Conocidos son los efectos biológicos de la ozonoterapia, tanto a nivel preclínico como a nivel clínico. Entre ese tipo de efectos se pueden mencionar la estimulación de los sistemas antioxidantes, el efecto inmunomodulador y el efecto metabólico del ozono. Por otra parte, se sabe que hipotiroidismo es la disminución de la función de la glándula tiroidea y en dependencia del origen del mismo se divide en primario y secundario. El caso que se presenta es un paciente con hipotiroidismo posquirúrgico con tratamiento de ozonoterapia por insuflación rectal. Se comprobaron los efectos de la ozonoterapia como herramienta complementaria en el mejoramiento de la calidad de vida en esta entidad nosológica.


Many are the reports about the benefits of ozone therapy against multiple diseases, but there are no case reports about its usage against hypothyroidism. Therefore, the objective of this article is to assess the benefits of ozone therapy against this condition that causes multiple disorders and symptoms. The biological effects of ozone therapy are known at the preclinical and clinical levels. Among these types of effects, we can mention the stimulation of antioxidant systems, the immunomodulatory effect, and the metabolic effect of ozone. Hypothyroidism is the decrease in the function of the thyroid gland, and, depending on its origin, it is divided into primary and secondary. A case is presented of postoperative hypothyroidism treated with ozone therapy by rectal insufflation. The effects of ozone therapy were verified, being a complementary tool for improving the quality of life in this nosological entity.

Se construyó con base en la búsqueda metódica de literatura respecto al tema, y el consenso entre los especialistas en Medicina Tradicional y Natural y en Medicina Física y Rehabilitación del Centro Especializado Ambulatorio del Hospital Dr. Gustavo Aldereguía Lima de Cienfuegos. Se hace énfasis en el manejo individualizado de estos pacientes con diagnóstico previo de enfermedad reumática y la fase en que se consultan, la evaluación consecuente del mismo, la evolución y el manejo oportuno; se aborda el tratamiento farmacológico, la acupuntura y sus técnicas afines, ozonoterapia, homeopatía así como los agentes físicos artificiales, las medidas de prevención y los ejercicios terapéuticos.

Este trabajo tiene el propósito de exponer los resultados del trabajo del Centro Especializado Ambulatorio Héroes de Playa Girón adjunto al Hospital General Universitario "Dr. Gustavo Aldereguía Lima" tras tres años de su puesta en marcha. Estudio descriptivo para el cual se tuvieron en cuenta los principales indicadores del Hospital General desde el 2007 hasta el 2012. A partir de septiembre de 2009, apertura del Centro Especializado Ambulatorio, se incluyeron sus indicadores conjuntamente con los anteriores. Los datos se obtuvieron de los informes estadísticos anuales de ambos recintos hospitalarios. La apertura del Centro Especializado permitió un incremento en la cirugía mayor ambulatoria en el Hospital General; en el Centro Especializado, en el 2010, 2011 y 2012, se realizaron el 36,2; 43,9 y el 41,1 por ciento respectivamente del total de la cirugía mayor electiva ambulatoria realizada en el Hospital General. Las intervenciones realizadas con mayor frecuencia fueron: herniorrafías, hemorroidectomía, colecistectomía por cirugía mínimamente invasiva, extirpación de tumores de partes blandas, intervenciones ginecológicas del suelo pélvico, cirugías ortopédicas y entre otras. El Centro Especializado asumió el 20 por ciento del total de pacientes vistos en la consulta del Hospital General. En los tres años de trabajo se hospitalizaron 3 119 pacientes, la mayoría en el servicio de rehabilitación y la enfermedad más frecuente para rehabilitar fue la cerebrovascular. El Centro Especializado Ambulatorio, desde su apertura, contribuye a disminuir la carga asistencial del Hospital General, ofrece un servicio de alta calidad y juega un importante papel en el desarrollo de la cirugía ambulatoria en Cienfuegos(AU)


This paper was aimed at putting forward the work results of Heroes de Playa Giron specialized ambulatory center subordinated to ¿Dr Gustavo Aldereguia Lima¿ general university hospital after three years of operation. This descriptive study took into account the main indicators of the general hospital from 2007 through 2012. As from September 2009, the date of opening of the specialized ambulatory center, these indicators were jointly included with the above-mentioned. Data were collected from the yearly statistical reports of both hospital facilities. The opening of the Center allowed increasing the number of ambulatory major surgeries in the general hospital, since 36.2 percent, 43.9 percent and 41.1 percent of the total number of ambulatory elective major surgeries recorded in the hospital were performed in 2010, 2011 and 2012, respectively, in this specialized center. The most frequent were herniorraphies, hemorrhoidectomy, colecistectomy through minimally invasive technique, excision of soft tissue tumors, gynecological treatments of the pelvic floor, orthopedic surgeries and others. The Specialized Center gave service to 20 percent of the total number of patients seen at the outpatient service in the general hospital. In three years, 3 119 patients were admitted to the hospital, most of them in the rehabilitation service and the most treated disease was the cerebrovascular one. Since its opening, the Specialized Ambulatory Center has contributed to reduce the medical assistance burden of the general hospital, provides high quality service and plays a key role in the development of ambulatory surgery in Cienfuegos province(AU)

La historia de la Medicina Tradicional y Natural (MTN) en nuestra Facultad se inicia en la década del 80, cuando los alumnos y profesores pertenecientes al grupo "Juan Tomás Roig" investigaban sobre plantas medicinales y recorrieron nuestros campos y ciudades en busca del conocimiento que sobre ellas tenía nuestra población, pero no es hasta inicios de 1991 cuando, como parte de las tareas de preparación que realizó todo el pueblo para enfrentar el período especial y la opción cero, que se comienza la introducción acelerada de estas técnicas, encaminadas a contribuir al arsenal terapéutico del personal de salud, sin la utilización de los medicamentos químicos convencionales, fundamentalmente con las especialidades de Medicina Tradicional China y Plantas Medicinales.

ABSTRACT

Se realizó un estudio clínico controlado tipo evaluación de tecnología, para evaluar la eficacia del tratamiento homeopático en pacientes afectados de forunculosis recurrente diagnosticados en el Instituto Superior de Medicina Militar "Dr. Luis Díaz Soto". Se seleccionaron 100 pacientes. Cada paciente constituyó su propio control, se tomó como referencia las manifestaciones clínicas y la respuesta al tratamiento. Los medicamentos homeopáticos fueron seleccionados según los criterios de similitud local, además del bioterápico en correspondencia con el microorganismo encontrado. Se logró una curación del 97,0 % de los tratados con la combinación medicamento homeopático y bioterápico, y se redujo la aparición de recidivas. Los medicamentos más utilizados fueron Hepar Sulphur, Silicea y el bioterápico Staphylococcinum.
