ABSTRACT

As many as 1,851 disease histories of patients belonging to the group of mental disorders of non-psychotic level (code MKB-9 300-317) were analyzed within the framework of the organizational experiment with a purpose of reorganizing the dispensary assistance. The data obtained indicate that the diagnosis and the treatment recommendations are not based enough in all the cases, with social restricting measures dominating over social and protective ones. For such a group of patients, the dispensary follow up is not justified. That is why the available dispensary services require changes in the principles and concrete patterns of their functioning.

ABSTRACT

A total of 101 cases of schizophrenia, proceeding with a prevalence of cenesthopathic disorders were studied. The following 3 types of the cenesthopathical syndrome were distinguished: cenesthoalgic, cenesthopathic and cenesthocoenesthetic, which actually present a continuum of morbid symptons. The successive transition from one state to another is a clinical reflection of the progression of a pathological process. The patients with cenesthopathic schizophrenia develop more profound negative changes in comparison to hypochondriacal schizophrenia. In 62% of the cases these changes lead to invalidization (according to epidemiological data). The results permit to consider the cenesthopathic schizophrenia as an independent variant of the disease.

ABSTRACT

The efficacy of psychopharmacological treatment modalities and comatose methods was studied in the therapy of patients with slowly progressive schizophrenia, the clinical picture of which was characterized by cenestho-hypochondriacal disturbances (155 cases). The use of high doses of neuroleptics, atropine and insulin comas in these cases were ineffective. A positive compliance to therapy was attained in the treatment by tranquilizers of the benzodiazepine series, especially by a parenteral administration. The best results were demonstrated in the treatment of pseudoneurotic hypochondria. The therapeutic reluctancy increased in the cenestho-algic syndrome and in the syndrome of "rigid" hypochondria. The most effective in these cases was a combined therapy by tranquilizers (parenteral administration) and small doses of neuroleptics.

ABSTRACT

The report contains data concerning clinical studies of a new Soviet tranquilizer of benzodiazepine derivative--phenazepam. Treatment of 100 patients gave a good therapeutical effect in 82%. The best results were attained in the treatment of generalized hystero-neurotical states, somewhat less in the group of neurotic states with a prevalence of affective disorders and the less effective in the obsessive-phobic syndrome. On the basis of comparative studies, the conclusion is made that phenazepam is not worse by its psychotropic activity than ativan (lorazepam) and exceeds diazepam (seduxen), most likely because of its myorelaxing action. The drug exerted an expressed hypnotic effect being not less than eunoctin and other eunectics.
