ABSTRACT

Membrane trafficking is a physiological process encompassing different pathways involved in transporting cellular products across cell membranes to specific cell locations via encapsulated vesicles. This process is required for cells to mature and function properly, allowing them to adapt to their surroundings. The retromer complex is a complex composed of nexin proteins and peptides that play a vital role in the endosomal pathway of membrane trafficking. In humans, any interference in normal membrane trafficking or retromer complex can cause profound changes such as those seen in neurodegenerative disorders such as Alzheimer's and Parkinson's. Several studies have explored the potential causative mechanisms in developing both disease processes; however, the role of retromer trafficking in their pathogenesis is becoming increasingly significant with promising therapeutic applications. This manuscript describes the processes involved in membrane transport and the roles of the retromer in the onset and progression of Alzheimer's and Parkinson's. Moreover, we will also explore how these aberrant mechanisms may serve as possible avenues for treatment development in both diseases and the prospect of its future application.

ABSTRACT

BACKGROUND: In the past decade, many machine learning (ML) models have been used in the management of normal pressure hydrocephalus (NPH). This study aims at systematically reviewing those ML models. METHODS: The PubMed, Embase, and Web of Science databases were searched for studies reporting applications of ML in NPH. Quality assessment was performed using Prediction model Risk Of Bias ASsessment Tool (PROBAST) and Transparent Reporting of a multivariable predication model for Individual Prognosis Or Diagnosis (TRIPOD) adherence reporting guidelines, and statistical analysis was performed with the level of significance of <0.05. RESULTS: A total of 22 studies with 53 models were included in the review, of which the convolutional neural network was the most used model. Inputs used to train various models included clinical features, computed tomography scan, magnetic resonance imaging, intracranial pulse waveform characteristics, and perfusion infusion. The overall mean accuracy of the models was 77% (highest for the convolutional neural network, 98%, while lowest for decision tree, 55%; P = 0.176). There was a statistically significant difference in the accuracy and area under the curve of diagnostic and interventional models (accuracy: 83.4% vs. 69.4%, area under the curve: 0.882 vs. 0.729; P < 0.001). Overall, 59.09% (n = 13) and 81.82% (n = 18) of the studies had high-risk bias and high-applicability, respectively, on PROBAST assessment; however, only 55.15% of the studies adhered to the TRIPOD statement. CONCLUSIONS: Though highly accurate, there are many challenges to current ML models necessitating the need to standardize the ML models to enable comparison across the studies and enhance the NPH decision-making and care.

ABSTRACT

Malaria is a severe public health issue in Nigeria, with higher morbidity and mortality rates than any other country. An estimated 100 million malaria cases and over 300,000 deaths are recorded yearly in Nigeria. Vaccination is an effective strategy in combating and eliminating infectious diseases such as malaria, thus, the deployment of a prospective malaria vaccine in Nigeria offers hope to the country's health sector. However, vaccination programmes face challenges, particularly in communities that are difficult to reach geographically or culturally, and these obstacles can only be overcome through continued international, national, and individual commitment. There is a need for expanded and continuous public health information, education, and communication particularly on contemporary health issues such as malaria and vaccination hesitancy. This will enable easier implementation and compliance to strategies for the sustainable control and eventual elimination of malaria. This article highlights some of the lessons learned from previous vaccination programs in Nigeria and how the insight gotten can be pivotal in ensuring the success of a prospective malaria vaccination programme in Nigeria.

ABSTRACT

The COVID-19 pandemic caused by the SARS-CoV-2 virus made governments across the world announce country-wide lockdowns suspending all activities and banning travels. This affected the lives of millions of people across all the continents in the world, including South America. The low-income classes were most affected by the lockdowns as many lost their only source of income. In addition, the uncertainty of the disease and its deadly nature along with lack of access to correct information led to a deterioration of the mental health of the low-income and indigenous groups in South America. The low-income population has little chance of gaining access to proper mental healthcare as many healthcare systems collapsed due to the nature of the pandemic. A good number of them faced short-term mental effects such as anxiety and panic attacks as well as long-term effects of depression. Proper steps must be taken to maintain and help the most vulnerable sections of the population. The pandemic along with the widespread poverty-stricken regions in the country have affected the population of the country. This article assesses the impact of COVID-19 on the poor and low-income population of the South American continent and the impact it has had on their mental health. It also reviews the recommended steps that can be taken to improve the situation and help the affected people.

ABSTRACT

The Uttarakhand State, known for its Himalayan Mountains, is a territory in Northern India that is extremely vulnerable to earthquakes, landslides, and floods. Currently, due to the COVID-19 outbreak, India is facing the dual challenge of containing a pandemic and responding to natural disasters. This situation can have a negative impact on the health and the economic development of the region, leading to a long-lasting humanitarian crisis that can disrupt even more, the already overburdened health service. In addition, it can pose serious threats to the wellbeing of the population as it complicates physical distancing and other COVID-19 prevention measures. It is of utmost importance to analyse the impact of floods, landslides, and COVID-19 pandemic on the health system of the Uttarakhand State, and how these crises interact with each other.

ABSTRACT

COVID-19 being a public health emergency of international concern has emerged in most African countries including Niger. Niger, a landlocked country, is tasked with controlling the pandemic. However, of the big challenges the country faced is the fragility of healthcare system which posed limitations to the fight against the virus. The virus overwhelmed the fragile healthcare system which led to inaccessibility of quality healthcare to the citizens coupled with issues of flooding and economic recession that happened during the pandemic. The healthcare sectored has further been crippled by exposure and infection of the already insufficient healthcare workers. In addition to this, there was the burden of NTDs and other communicable and non-communicable diseases that subverted the country in the depths of difficulties. As per the predictions of World Bank, the poverty curve is likely to escalate due to the outrageous impacts of COVID-19. Adding on to this, the occurrence of natural disasters such as flooding has further stretched the country. It's no coincidence that the country would confront plethora of challenges amidst the second wave. Therefore, timely decision and necessary interventions are needed to strengthen the country's fight against the pandemic. However, this is only feasible when Nigerien government, international allies and other wealthy nations work closely to ensure that the challenges faced by the healthcare system are tackled.
