ABSTRACT

Background: Fungal ball sinusitis is a sinonasal fungus ball that usually affects immunocompetent adults with female predominance. The most affected sinus is the maxillary sinus. Aspergillus species is the most typically found fungus. Computed tomography (CT) scan is the gold standard tool in order to diagnose fungal ball sinusitis. The ultimate method for a fungal ball is functional endoscopic sinus surgery (FESS), which has a high success rate and a low morbidity rate. Objective: This study aims to demonstrate the various clinical presentations of fungal ball sinusitis including isolated maxillary sinus, sphenoid sinus, simultaneous occurrence of maxillary and sphenoid fungal ball, and post endonasal endoscopic pituitary surgery fungal ball with various age groups. Also, this study aims to emphasize the importance of early diagnosis and treatment in such cases. Patients and Methods. A retrospective study that was carried in the otorhinolaryngology department of two hospitals: King Fahad Specialist Hospital and Qatif Central Hospital, Eastern Region, Saudi Arabia. The study was conducted on a total of 16 patients who were diagnosed with paranasal sinuses fungal ball in an 11-year period from January 2008 and November 2019. Results: Out of 16 patients with paranasal sinuses fungal ball, 11 cases were female and 5 males, with age ranging between 16 and 46 years. Results showed eight isolated sphenoid (50%), six isolated maxillary fungal ball (38%), one simultaneous occurrence of the sphenoid and maxillary fungal ball (6%), and one post endonasal endoscopic pituitary surgery for pituitary adenoma (6%). CT scan was performed for all 16 cases which is the standard tool for the diagnosis of the fungal ball. Conclusion: Fungal ball may present with variety of symptoms but most commonly with postnasal discharge (PND), headache, and facial pain. CT sinuses is the diagnostic radiological modality to confirm the diagnosis. The FESS functional endoscopic sinus surgery is the gold safe approach for patients with fungal ball to manage their symptoms, confirm the diagnosis, and removal of disease with no morbidities.

ABSTRACT

The coronavirus disease 2019 (COVID-19) pandemic has resulted in nationwide stay-at-home orders in an effort to slow the spread severely impacting the healthcare sector. Telepsychiatry provides a platform bridging the gap through advanced technologies connecting mental health providers and patients who need their services, overcoming previous barriers of great distances, lack of transportation, and even time constraints. The most obvious benefit is increased accessibility to mental healthcare, especially in underserved and remote areas where there is no easy access for in-person care. It is important to note that benefits are not limited to patients, but also allow clinicians greater flexibility in scheduling and reduced practice overhead costs, both of which aid with physician burnout and burden. Telepsychiatry during COVID-19 provides its own unique advantages over in-person visits. The risk of exposure to healthcare workers and patients receiving care is reduced, allowing immunocompromised patients to receive much-needed psychiatric care. Without the need to meet in person, self-isolating psychiatrists can still provide care, decreasing strain on their co-workers. Although telepsychiatry is relatively new, it has already exhibited considerable success in its effectiveness at treating psychiatric conditions and widespread corollary benefits. Telepsychiatric consults may be carried out synchronously and asynchronously, each having benefits and setbacks. Different mobile application interventions have been explored, which are available for the purpose of both monitoring/assessing patients and/or providing treatment. The scope of conditions these applications address is broad, from anxiety disorders to schizophrenia to depression. As promising and beneficial telepsychiatry may seem, it is necessary to recognize that building the program can be challenging. It involves adapting to new methods in medicine. We highlighted barriers to general telepsychiatry, the most prominent being technological literacy of both physician and patient, and possible negative effects of eliminating the in-person patient-doctor interaction.

Introduction: Over a period of time researchers have become more interested in finding out the potential of various foods to maintain the general health and to treat diseases. Almonds are a very good source of many nutrients which may help to sharpen the memory and to reduce cardiovascular risk factors. Objective: The present study was conducted to evaluate the nootropic effects of almonds. Effect of oral intake of almond was also monitored on food intake and plasma cholesterol levels. Methods: Rats were given almond paste orally with the help of feeding tube for 28 days. Memory function in rats was assessed by Elevated Plus Maze (EPM) and Radial Arm Maze (RAM). Brain tryptophan, 5-HT and 5-HIAA were estimated at the end of the treatment by HPLC-EC method. Results: A significant improvement in learning and memory of almond treated rats compared to controls was observed. Almond treated rats also exhibited a significant decrease in food intake and plasma cholesterol levels while the change in growth rate (in terms of percentage) remained comparable between the two groups. Analysis of brain tryptophan (TRP) monoamines exhibited enhanced TRP levels and serotonergic turnover in rat brain following oral intake of almonds. Conclusion: The findings show that almonds possess significant hypophagic and nootropic effects. Results are discussed in context of enhanced 5-HT metabolism following almond administration (AU)


Introducción: De un tiempo a esta parte, los investigadores se han interesado cada vez más en encontrar el potencial de diversos alimentos para mantener la salud en general y tratar la enfermedad. Las almendras son una muy buen fuente de muchos nutrientes que pueden ayudar a agudizar la memoria y reducir los factores de riesgo cardiovascular. Objetivo: Este estudio se realizó para evaluar los efectos nootróficos de la almendra. También se monitoreó el efecto de la ingestión oral de almendra sobre el consumo de alimentos y las concentraciones plasmáticas de colesterol. Métodos: Se suministró a las ratas pasta de almendra por vía oral con la ayuda de una sonda de alimentación durante 28 días. Se evaluó la función de memoria de las ratas mediante Elevated Plus Maze (EPM) y Radial Arm Maze (RAM). Se estimaron el triptófano cerebral, la 5-HT y el 5-HIAA al final del tratamiento mediante un método de HPLC-EC. Resultados: Se observó una mejoría significativa en el aprendizaje y la memoria en las ratas tratadas con almendra en comparación con los controles. Las ratas tratadas con almendra también mostraron una reducción significativa del consumo de alimentos y de las concentraciones plasmáticas de colesterol, mientras que el cambio en la tasa de crecimiento (en términos de porcentaje) siguió siendo comparable entre ambos grupos. El análisis de las monomaninas y de triptófano cerebral mostró unas concentraciones superiores de triptófano y del recambio serotoninérgico del cerebro de la rata tras el consumo de almendras. Conclusión: los hallazgos sugieren que las almendras poseen unos efectos hipofágicos y nootróficos significativos. Los resultados se debaten en el contexto del metabolismo mejorado de la 5-HT tras la administración de almendras (AU)

ABSTRACT

Plant species dominance and arbuscular mycorrhizas (AMs) were evaluated in four different habitats of Cholistan desert, Pakistan. The sites were selected on the basis of variations in topography, floristic composition and biotic interference. The phytosociological data revealed obvious differences in the floristic composition, plant species diversity, AM colonization and arbuscular mycorrhizal fungi (AMF) spore densities in the rhizospheres. The vegetation pattern and composition was governed by topography and edaphic characteristics of a particular site. Grasses were dominant and main components of vegetation at all the study sites and were invariably mycorrhizal. Some plant species were mycorrhizal at one site but non-mycorrhizal at the other. The AMF survey reported here offers an important starting point from which to analyse AMF community structure in different phytosociological habitats and land uses of Cholistan desert.
