ABSTRACT
Precision termino-lateral pancreatico-jejunal anastomosis with or without mobilization of the pancreatic duct, with the use of inert suture materials in pancreatoduodenal resection was conducted in 21 patients, 3 of them died. Incompetence of the anastomoses was not encountered. The number of patients treated by radical operation increased almost 2.5 fold, and was 27.4%. The authors consider inclusion of the stump of the gland into digestion together with other conservative and technical measures to be most promising trend in the prevention of incompetence of the anastomoses and postoperative pancreatitis, in improvement of the results of pancreatoduodenal resection, and in expanding the scope of its use.

Clinical observations of 478 patients operated for pancreatitis resulting from different obturating lesions of the Vater papilla (stones, cicatricial stenosis, polyps etc.) are analyzed. The questions of diagnosis, preoperative preparation, medical methods, technique of the operation and postoperative management are discussed. Recommendations are given for the improvement of results of the treatment and the decrease of postoperative lethality.

Systematic performance of fibroduodenoscopy in patients with diseases of bile tracts and the pancreas made it possible in 90 cases to reveal polyps of papilla duodeni major, which were the cause of chronic recurrent pancreatitis. An experience with 50 operations evidences that transduodenal polypectomy is a radical method of treatment obviating recurrences of pancreatitis.
