ABSTRACT

An examination was made of 22 hypertensive patients with clinical and echoCG symptoms of left ventricular hypertrophy. All the patients underwent coronary angiography, transvenous multiphase left ventriculography and acute captopril (11 patients), nifedipine (11 patients) tests. A single oral 25 mg dose of captopril increased energetic efficacy of cardiocycle, restored diastolic function of the left ventricle, aroused myocardial contractility.

ABSTRACT

The study included 39 hypertensive subjects with clinical echocardiographic evidence of left ventricular hypertrophy. All the patients were subjected to coronary angiography and transvenous multiple-phase left ventriculography. Early subclinical signs of left ventricular dysfunction in hypertensive patients have been defined. They concern energetic efficacy of the cardiac cycle, left ventricular diastolic function, myocardial asynchronism. It is noted that the above dysfunction was recorded in cases where standard two-image analysis provides normal values.

ABSTRACT

To make a comparative assessment of transesophageal left atrial pacing (TLAP) and bicycle ergometry (BE) in the diagnosis of coronary heart disease (CHD) in patients with arterial hypertension (AH) of different origin, the authors examined 56 patients. The patients underwent TLAP and selective coronary angiography, of them 39 patients had BE testing. No significant differences were found in their specificity (73 and 78%) and sensitivity (92 and 81%) between TLAP and BE, respectively. The maximum heart rate in CHD patients with AH was significantly higher (130 +/- 6 per min) during TLAP than that in BE (112 +/- 5 per min, p > 0.05), ischemic changes occurring at the same value of double product despite the mode of myocardial ischemia induction in these functional tests (240 +/- 10 and 236 +/- 12 arbitrary units, respectively; p < 0.05). The mean systolic blood pressure was higher in TLAP and in BE (210 +/- 10 and 185 +/- 8 mm Hg). This follows that TLAP and BE finding are comparable and no profound changes occur in TLAP, hence it can be recommended for wide application in the diagnosis of CHD in AH patients.

ABSTRACT

A study of the correlation between electrophysiologic effects of intracoronary injections of radiopaque agents and anatomical features of blood supply of cardiac conduction was carried out in 60 patients with intact coronary arteries. Coronary angiography was performed in all the patients. Disorders in cardiac conduction and repolarization in the myocardium were observed in intracoronary injection of radiopaque agents, which was accompanied by the development of S-A bradycardia. A bradyarrhythmic reaction type depends on prevalence of the left or right coronary artery in the atrial blood supply and cardiac conduction. The bradyarrhythmic effect was more pronounced in injection of a radiopaque agent in the right artery than in the left one.

ABSTRACT

The main functional characteristics of the left ventricle (LV) were defined using a method of multiphase radiopaque left ventriculography. Three groups of indices of LV function (energetic LV indices, indices of chamber diastolic function, and indices of local motion of the LV walls in the course of a complete cardiocycle) were standardized in healthy persons. An original algorithm of analysis of local LV myocardial contractility was used. A combined approach to interpretation of results of this method was worked out. A standard set of indices, reflecting LV function, was defined. Standardized values of these indices, necessary for an effective use of this method, were obtained.
