ABSTRACT

Background: COVID-19 created an unexpected situation for dental patients. However, the impact of the lockdown on orthodontic patients is still unknown. Hence, this study was conducted to analyse the number and type of emergencies, distress and fear of getting infected among brackets and aligners orthodontic patients during the COVID-19 lockdown. Material and Methods: A total of 324 questionnaires about emergency appointments during the lockdown and Kessler Psychological Distress Scale-K10 were distributed between March and December 2022 among patients who were on active orthodontic treatment during lockdown at the Master of Orthodontics at the University Alfonso X el Sabio during February and December 2022. Chi-square was used to compare groups. Results: Emergency appointment was needed by 37.78% of patients during lockdown: 73.5% among those with aligners and 45.9% with brackets (p<.01). Loose bracket was the most prevalent emergency for bracket patients and lack of aligners among invisible orthodontic patients. Of those, 36.95% in need of orthodontic emergency appointments didn´t attend the clinic for fear of being infected; 21% of patients had high or very high anxiety levels. Conclusions: Orthodontic patients had moderate levels of anxiety and fear of being infected during lockdown. Of these, 31.78% needed an emergency appointment. ABSTRACT

Camera calibration is necessary for many machine vision applications. The calibration methods are based on linear or non-linear optimization techniques that aim to find the best estimate of the camera parameters. One of the most commonly used methods in computer vision for the calibration of intrinsic camera parameters and lens distortion (interior orientation) is Zhang's method. Additionally, the uncertainty of the camera parameters is normally estimated by assuming that their variability can be explained by the images of the different poses of a checkerboard. However, the degree of reliability for both the best parameter values and their associated uncertainties has not yet been verified. Inaccurate estimates of intrinsic and extrinsic parameters during camera calibration may introduce additional biases in post-processing. This is why we propose a novel Bayesian inference-based approach that has allowed us to evaluate the degree of certainty of Zhang's camera calibration procedure. For this purpose, the a prioriprobability was assumed to be the one estimated by Zhang, and the intrinsic parameters were recalibrated by Bayesian inversion. The uncertainty of the intrinsic parameters was found to differ from the ones estimated with Zhang's method. However, the major source of inaccuracy is caused by the procedure for calculating the extrinsic parameters. The procedure used in the novel Bayesian inference-based approach significantly improves the reliability of the predictions of the image points, as it optimizes the extrinsic parameters.

ABSTRACT

Based on the results of the 2015 oral health survey in Spain, the prevalence of untreated dental caries in temporary dentition, in the six-year cohort was 27,8%, and in the twelve-year cohort 14,6%. Due to the magnitude of the problem, the high health cost and the possibilities of prevention, monitoring the oral health status of the child population is extremely important, as it will allow the implementation of prevention and early care strategies. OBJECTIVES: To study the caries prevalence and malocclusion in the mixed dentition in children aged 6-12 years in Madrid, Spain. . MATERIALS AND METHODS: This is a descriptive, cross-sectional study carried out in the city of Madrid during the 2018-2019 school year. Fifth year students of the European University of Madrid through health surveys, calibrated and supervised by the two main researchers, collected the data. The schoolchildren assessed were students from different schools in the city of Madrid, between 6 and 12 years old. According to WHO standards, data were collected in 2 months. RESULTS: The total number of students examined was 303 students from 4 different schools in the community of Madrid. The prevalence of caries in our sample in the primary dentition was 45.21% with dmf> 0 and in the permanent dentition it was 23.7 % with a DMF> of 0. CONCLUSIONS: The oral health survey carried out in the sample shows a high prevalence of caries, as well as in the dmf and DMF indicators. Most of the children needed fluoride applications, so it would be interesting to evaluate which of them can be prevented with different public health projects.

ABSTRACT

BACKGROUND: Although the clinical practice guidelines recommend continuous adjustment of asthma treatment and reducing the maintenance drugs when achieving control (step-down), there are few studies of standard clinical practice aimed at collecting information on the factors that determine step-down failure. OBJECTIVE: To determine the factors that determine step-down failure in standard clinical practice of patients with moderate-severe asthma controlled by a combination of inhaled glucocorticoids and long-acting beta agonists. METHODS: A multicentre retrospective study included 374 patients with moderate-severe asthma controlled with inhaled glucocorticoids and long-acting beta agonists for whom the physician indicated a step-down in 2016. RESULTS: The step-down failed in 41.7% of the patients. The following factors were related to failure: greater patient age (P=.006), presence of at least 2 comorbidities (P=.016), greater severity level (severe persistent vs. moderate persistent) (P<.001), greater age at diagnosis (>40 years) (P=.045), the higher the therapeutic step before (P=.003) and after the change (P<.001), the shorter the time of improvement/control prior to the change (P=.019), lower FEV1 (P=.001) and a poorer Asthma Control Test score or Asthma Control Questionnaire score before the step-down (P<.001). The logistic regression analysis showed a higher probability of step-down failure in the more elderly patients (OR, 0.983; 95% CI 0.969-0.997) and those with severe asthma compared to those with moderate asthma (OR, 0.537; 95% CI 0.292-0.985), as well as an increased probability of success if the patients had the disease controlled for more than 6 months (OR, 2.253; 95% CI 1.235-4.112). CONCLUSION: In standard clinical practice conditions, step-down fails in a high percentage of patients, and the suggestion is to indicate step-down when the patient has had more than 6 months of disease control.

Se describe el caso de una trabajadora con asma ocupacional en el que se detecta un doble agente etiológico condicionado por la exposición mixta en distintas áreas de trabajo. Los isocianatos y resinas epoxi se han descrito como responsables de enfermedad profesional y se ha confirmado su relevancia en las pruebas de función pulmonar tanto de manera inmediata como tardía y en la prueba de provocación bronquial específica. Se destaca la necesidad de evaluar exhaustivamente la exposición a sustancias en el ambiente laboral y comprobar la mejoría clínica tras el cese permanente de la exposición a los agentes implicados


We describe a case of a worker with occupational asthma caused by two separate etiologic agents, resulting from mixed exposures in different work areas. Isocyanates and epoxy resins are established causes of occupational disease, and their role in this case was confirmed by immediate and delayed pulmonary function testing, together with a specific bronchial challenge. A thorough evaluation of exposure to harmful substances in the workplace is essential, together with continued clinical monitoring following cessation of exposure in order to verify clinical improvement

ABSTRACT

INTRODUCTION: Our objective was to evaluate the efficacy of conservative treatment with imiquimod in Paget vulvar disease. MATERIALS AND METHODS: We describe a case series that includes 10 patients with histopathologic diagnosis of extramammary Paget disease of the vulva, who were treated with 5% imiquimod cream. Of these patients, 3 were treated for recurrent disease and 7 were treated for initial primary disease. The patients applied the cream every other day until the lesions were no longer clinically detected.They were previously instructed on how and where to apply the cream by making them use a mirror while following the physician's directions. RESULTS: Complete clinical and histologic remission of the disease was achieved in 9 patients. The remaining patient had partial histologic response and is still under treatment. The treatment was well tolerated despite moderate irritation. No recurrences were observed during a mean follow-up of 18 months. CONCLUSIONS: On the basis of the results, the authors consider that 5% imiquimod cream could be considered a safe and effective therapeutic option for the treatment of primary vulvar Paget disease. Further studies are needed to determine the real efficacy and safety of 5% imiquimod cream for the treatment of this infrequent disease.
