ABSTRACT

Lung transplantation is one of the most difficult fields of contemporary transplantology. The operation was performed on a 53-year-old female who had been suffering from terminal stage COPD. The period of cold ischemia for the right and left lung was 1 hour 30 minutes and 2 hours 25 minutes respectively. The total operation time was 4 hours and 40 minutes. There were no surgical complications. There were no signs of transplant rejection 6 weeks after the operation. The considerable decrease in dispnea and increase in quality of life were registered during the follow-up. FEV1 was increased by 80% and 6 minute walk test distance was increased by 200 meters.

ABSTRACT

Genetic relationships with tuberculous process and immunity were investigated within immunogenetic study of 178 patients with active pulmonary tuberculosis. Tuberculosis occurrence seemed to be associated with phenotypical antigen Cw2. Unfavorable outcome of the disease was observed in the presence of HLA A9, A11, B14, B40, B41 antigens. A pronounced specific humoral and cellular responses more frequently arise in carriers of antigens Cw2 and B13, while high proliferative activity of T-cells in carriers of A10, B7 and Cw2. Production of IgA, IgG and IgM is determined by different genetic factors. The results obtained are essential for prognosis of tuberculosis course and choice of individual therapy.

ABSTRACT

The association of duodenal ulcers with antigens AIO and combinations of antigens B18B40, B35B40, A10B5, A10B40 and others and the association of gastric ulcer with antigens A3B7, A3B40, B14B15 were revealed by an investigation of HLA antigens in 46 patients with ulcer disease of the duodenum and 25 patients with ulcer disease of the stomach. It was concluded that the ulcer disease had a hereditary predisposition characterized by genetic heterogeneity with different etiopathogenetic mechanisms of the development of ulcers of different localizations. The HLA antigens may be used as immunogenic markers of ulcer disease if observing the population method in the investigations.

ABSTRACT

HLA antigens (A, B, DR) of the tissues of 171 patients with different types of diabetes mellitus were investigated. Controls were 1867 healthy Leningrad residents (control I), not investigated with the GTT, and 38 pregnant women with the unchanged GTT during pregnancy (control II). Some features of the frequency of occurrence of individual antigens and their interlocular (HLA A, B) combinations in type I and type II diabetes mellitus and diabetes of pregnant women were established. The risk of diabetes mellitus, type I, development was shown to be on the increase in the presence of HLA DR4 in the phenotype and considerably on the decrease in the presence of HLA B17. The results point out to the genetic heterogeneity of different types of diabetes mellitus. The authors think it possible to use HLA typing for the diagnosis of type I diabetes mellitus.

ABSTRACT

The dynamics of the placental lactogenic hormone level, insulin requirements, and glycaemia during pregnancy was studied in diabetes I and II patients and diabetic pregnancies. Fructose amine and C-peptide levels in maternal and neonatal blood were measured in different diabetes types. The risk of diabetes I development in relation to the incidence of the HLA-system antigens was assessed. The need for a functional insulin therapy prior to and during pregnancy was substantiated.
