ABSTRACT

Angiogenesis is a major factor in the development of benign, inflammatory, and malignant processes of the skin. Endothelial cells are the effector cells of angiogenesis, and understanding their response to growth factors and inhibitors is critical to understanding the pathogenesis and treatment of skin disease. Hemangiomas, benign tumors of endothelial cells, represent the most common tumor of childhood. In our previous studies, we have found that tumor vasculature in human solid tumors expresses similarities in signaling to that of hemangiomas, making the knowledge of signaling in hemangiomas widely applicable. These similarities include expression of reactive oxygen, NFkB and akt in tumor vasculature. Furthermore, we have studied malignant vascular tumors, including hemangioendothelioma and angiosarcoma and have shown distinct signaling abnormalities in these tumors. The incidence of these tumors is expected to rise due to environmental insults, such as radiation and lumpectomy for breast cancer, dietary and industrial carcinogens (hepatic angiosarcoma), and chronic ultraviolet exposure and potential Agent Orange exposure. I hypothesize that hemangiomas, angiosarcomas, and vascular malformations represent the extremes of signaling abnormalities seen in pathogenic angiogenesis.

ABSTRACT

The behaviour of the blood flow passing through artificial heart values in aortic position is numerically simulated by discretizing the Navier-Stokes equations for viscous incompressible flow through the finite element method. Three different artificial valves, Starr-Edwards, St Jude and disc valves, are modelled by using the finite element method as well as the Navier-Stokes equations for viscous incompressible flow. The blood flow behaviour is characterized by discretizing both planar and axisymmetric domains of the value devices. The streamlines as well as the velocity distributions are studied and compared. Likewise, the pressure patterns are analysed and compared. Some numerical examples of the three valves are presented and discussed herein.

ABSTRACT

We studied the use of the holmium laser for sclerostomy through a small lamellar corneal incision and the effects of transconjunctival mitomycin-C on the outcome of filtration surgery without conjunctival incision. The holmium laser, equipped with a straight-firing probe, was used to create sclerostomies in seven New Zealand white rabbits through a corneal lamellar incision. One eye in each rabbit was treated with transconjunctival mitomycin-C (0.4 micrograms/mL for 5 minutes), and the fellow eye underwent sclerostomy without pretreatment with mitomycin-C as a control. The reduction in intraocular pressure was greater and persisted significantly longer in the eyes pretreated with mitomycin-C than in the controls. All control eyes had flat blebs by day 7 to 12, while the treated eyes maintained a bleb throughout the study. Microscopic examination showed that sclerostomies created by the straight-firing probe induced significantly less thermal damage than those created by the stationary side-firing probe. These results demonstrate that successful transcorneal sclerostomy without conjunctival incision can be created using the straight-firing holmium laser probe, with enhancement of filtration by pretreatment with transconjunctival mitomycin-C.

ABSTRACT

En el presente estudio multicéntrico abierto, se evaluó la eficacia del antagonista H2 FAMOTIDINA MSD en promover la cicatrización de la úlcera péptica gástrica, así como su tolerancia y seguridad. Veintiocho pacientes con úlcera péptica gástrica, demostrada por endoscopía e histología, recibieron famotidina 40 mg (una tableta) noche, hasta por ocho semanas. El tratamiento se suspendió a la cuarta semana en caso de evidenciarse cicatrización. Todos los pacientes fueron evaluables para eficacia y seguridad. Se consiguió cicatrización de la úlcera en veintidos de ellos (78.6%), requiriéndose sólo cuatro semanas de tratamiento en quince (53.6%). El 25% de los pacientes se tornó asintomático a las 72 horas y el 60% al finalizar la primera semana. La droga fue bien tolerada y no se registraron reacciones adversas clínicas, ni de laboratorio. Los resultados del presente estudio utilizando famotidina 40 mg. noche, muestran que esta es una terapia eficaz, bien tolerada y segura en la mayoría de pacientes con úlcera péptica gástrica

ABSTRACT

Arteriovenous (AV) malformation is a rare vascular lesion of the jaws with a benign clinical presentation. As a result, AV malformation may be excluded from a presurgical differential diagnosis, with catastrophic results. Review of the pertinent literature showed no documentation of AV malformation in the maxillary sinus, and therefore we present the case of a 43-year-old white man with a history of blunt trauma to the paranasal region, whose differential diagnosis included a vascular lesion.

ABSTRACT

Se efectúa el estudio de 12 pacientes con diagnóstico de enfermedad quística congénita del sistema biliar, en diez de estos la comprobación fue quirúrgica. Según la clasificación de Todani, 8 (67%) correspondieron al tipo I, 2(17%) al tipo IVA y al tipo V los dos restantes (17%). Nueve pacientes (75%) fueron del sexo femenino. Seis de los pacientes (50%), estuvieron comprendidos entre los 30 y 39 años de edad. Las manifestaciones clínicas más frecuentes fueron: Dolor abdominal (83%), náusea (75%), coluria (66%), ictericia (58%), hepatomegalia (50%), y fiebre (50%). En 8 pacientes (67%), se registró elevación de la fosfatasa alcalina y en 7 (58%), de las aminotransferasas. El procedimiento diagnóstico de mayor efectividad fue la ecografia (86%). Diez pacientes fueron sometidos a cirugia, ellos correspondieron a la totalidad de los pacientes con quistes tipo I y IVA. En tres pacientes se comprobó litiasis intraquística y en uno se documentó cirrosis hepática

ABSTRACT

Se hace el estudio comparativo de la patología digestiva consignada en los diagnósticos de alta del Hospital "Cayetano Heredia" a nivel del mar, con 20,927 egresados entre 1973 a 1975 ; Hospital "Daniel A. Carrión" de Huancayo a 3,300m. sobre el nivel del mar, con 23,069 egresados entre 1973 a 1976; y los Hospitales del Seguro Social y Esperanza de Cerro de Pasco a 4,200 m. sobre el nivel del mar, considerados en conjunto con un total de 13,065 egresos entre 1972 y 1975. Se señala y discute que en relación al grupo del nivel del mar, entre los hospitalizados en la altura: a) Es más frecuente la úlcera péptica. Predomina la úlcera duodenal a los 3,300m.s/n.m.; en cambio la úlcera gástrica lo es en la serie estudiada a 4,200 m.s/n.m. b) Es mayor la presencia del dolicolon, vólvulo del sigmoides e hidatosis hepática. c)En el grupo de los 3,000 m.s/n.m. es mayor el cáncer gástrico y el diagnóstico de pseudo obstrucción intestinal. d) En los investigados 4,200 m.s/n.m. son mucho más frecuentes la hemorragia digestiva alta y los hemorroides. e) Es menor la incidencia de diverticulitis, cáncer del colon y cáncer del páncreas, llamando igualmente la atención la menor frecuencia con que se consigna el diagnóstico de cirrosis hepática.
