ABSTRACT

We report a primary hemifacial spasm that started four years ago with sudden twitching of the face towards the right side. It was diagnosed as a hemifacial spasm by a neurologist and prescribed with Zeptol 100 mg 0.5 tablet BID for two weeks, followed by two sittings of Botox injection in a gap of 1 year. A year later, it reappeared more severely, driving her towards an integrative treatment modality. Ayurveda treatments including Nasya, Ksheera dhooma, internal medications, and Rasona navaneetha prayoga were administered. GV20, GB14, EX-HN5, ST3, ST4, ST6, TE17, LI4, and GB34 were selected for electro-acupuncture. The scores of hemifacial spasm grading and quality of life scale were 9 and 20 (before), 6 and 16 (after treatment), and 4 and 10 (follow-up after six months), respectively. This integrative approach was safe and has shown an improvement in hemifacial spasm.

ABSTRACT

This case is about a post viral olfactory dysfunction due to SARS-CoV-2 infection, since six months treated effectively by a unique integrative approach with Ayurveda and Traditional Chinese Acupuncture (TCA). It started as a sudden onset of anosmia in the month of August 2020 with fever history two weeks prior. After three months Parosmia commenced which gradually worsened. A month later she experienced dysgeusia and nausea leading to tremendous stress and sleep disturbances. Ayurveda treatment included Shadbindu taila marsha nasya for seven days followed by Shadbindu taila pratimarsha nasya (intra nasal oil instillation) for a period of four months. Naradiya laxmivilasa rasa orally for one month. GV.20, LI.20, Ex.1, H.7, LI.11, GB.8, GB.21 and GV.25 Points were selected for TCA. The Indian Smell identification test score on day one of treatment was zero, day seven was four, at the end of one month was eight and after four months it was ten. Parosmia was assessed by VAS which was ten on day one and zero on day seven. We could achieve complete normalcy in olfactory function within four months of this treatment. This integrative approach was found to be safe and effective in treating Post covid parosmia.

ABSTRACT

Patients with seizures represent a challenging clinical population both in pediatrics and adults. Accurate diagnosis of the cause of a seizure is important in choosing an effective treatment modality, surgical planning, predicting a prognosis, and follow-up. Magnetic resonance (MR) imaging using a dedicated epilepsy protocol plays a key role in the workup of these patients. Additional MR techniques such as T2 relaxometry and MR spectroscopy show a promising role to arrive at a final diagnosis. The spectrum of epileptogenic causes is broad. Radiologists and physicians need to be updated and require a patterned approach in light of clinical history and electroencephalogram findings to arrive at a reasonable differential diagnosis. This pictorial essay aims to review a few of the common and uncommon causes of seizures and their imaging features.

ABSTRACT

Impairment of the geniculostriate pathway results in scotomas in the corresponding part of the visual field. Here, we present a case of patient IB with left eye microphthalmia and with lesions in most of the left geniculostriate pathway, including the Lateral Geniculate Nucleus (LGN). Despite the severe lesions, the patient has a very narrow scotoma in the peripheral part of the lower-right-hemifield only (beyond 15° of eccentricity) and complete visual field representation in the primary visual cortex. Population receptive field mapping (pRF) of the patient's visual field reveals orderly eccentricity maps together with contralateral activation in both hemispheres. With diffusion tractography, we revealed connections between superior colliculus (SC) and cortical structures in the hemisphere affected by the lesions, which could mediate the retinotopic reorganization at the cortical level. Our results indicate an astonishing case for the flexibility of the developing retinotopic maps where the contralateral thalamus receives fibers from both the nasal and temporal retinae.

ABSTRACT

Impairments of visual motion perception and, in particular, of flow motion have been consistently observed in premature and very low birth weight subjects during infancy. Flow motion information is analyzed at various cortical levels along the dorsal pathways, with information mainly provided by primary and early visual cortex (V1, V2 and V3). We investigated the cortical stage of the visual processing that underlies these motion impairments, measuring Grey Matter Volume and Cortical Thickness in 13 children with Periventricular Leukomalacia (PVL). The cortical thickness, but not the grey matter volume of area V1, correlates negatively with motion coherence sensitivity, indicating that the thinner the cortex, the better the performance among the patients. However, we did not find any such association with either the thickness or volume of area MT, MST and areas of the IPS, suggesting damage at the level of primary visual cortex or along the optic radiation.

ABSTRACT

Continuous psychophysics is a newly developed technique that allows rapid estimation of visual thresholds by asking subjects to track a moving object, then deriving the integration window underlying tracking behavior (Bonnen, Burge, Yates, Pillow, & Cormack, 2015). Leveraging the continuous flow of stimuli and responses, continuous psychophysics allows for estimation of psychophysical thresholds in as little as 1 min. To date this technique has been applied only to tracking visual objects, where it has been used to measure localization thresholds. Here we adapt the technique to visual motion discrimination, by displaying a drifting grating that changes direction on a binary random walk and asking participants to continuously report drift direction by alternate key press. This technique replicates and confirms well-known findings of the motion-perception system. It also proves particularly valuable in demonstrating induced motion, reinforcing evidence for the existence of antagonistic surround fields. At low contrasts, the surround summates with the center, rather than opposing it, again consistent with existing evidence on classical techniques. The user-friendliness and efficiency of the method may lend it to clinical and developmental work.
