ABSTRACT

Introduction: Vaccination against SARS-CoV-2 has been used to reduce the severity of COVID-19 disease and the incidence of new cases. However, a significant proportion of people have shown vaccination hesitancy. Methods: This study explored psychological factors related to vaccination hesitancy in a sample of Italian COVID-19 patients (N = 54), hospitalized during 2021, after vaccines had been made available and while the vaccination campaign was on-going. Consecutive patients, aged 18 or older, admitted to the hospital with a diagnosis of COVID-19 were assessed with a set of standardized measures. Results: In our sample, 48.1% was not vaccinated and 7.4% died within 6months after hospitalization, with a preponderance of deaths among non-vaccinated patients. Non-vaccinated participants had higher resilience scores at the CD-RISC-10 scale than vaccinated ones (33.6 ± 5.50 vs 28.6 ± 6.61; t40.2=+ 2.94, p = 0.005). No statistically significant differences were found between the two groups for any other measures. Discussion: Higher levels of resilience among non-vaccinated patients may reflect greater identity worth and self-esteem, in turn resulting in a decrease in vaccination likelihood. This finding may have important public health implications, as it indicates that specific psychological aspects, such as resilience, may result in vaccination hesitancy, with implications for hospitalization rates, and thus healthcare costs, as well as loss of lives.

ABSTRACT

Microcystic/reticular (MRV) schwannoma has been described since 2008, but remains a rarely encountered entity. MRV has a predilection for visceral locations and has variable histologic appareances. Given its rarity and anatomic variability, this entity could raise differential diagnostic issues with other tumours and malignancies.We describe the case of a 69-year-old male followed at IRCCS Ospedale Policlinico San Martino of Genoa for his previous history of non-Hodgkin lymphoma. A para-aortic mass was discovered during follow-up, which -due to its stability, also after chemotherapy- had been hypothesized to be a non-lymphomatous lesion; given the dimensions and the site, the mass was removed. Histological evaluation showed a nodule limited by a slight fibrous capsule and characterized by a proliferation of medium-sized fusiform cells, with elongated nuclei and scarce eosinophilic cytoplasm. Given the lack of malignant signs and the strong expression of protein S-100, a diagnosis of mesenchymal neoplasia with expression of neural markers compatible with reticular schwannoma was made. The neoplasm has not recurred since its removal.The case we present is, at our best knowledge, the first described in the retroperitoneum, a site where the exclusion of other mesenchymal malignancies is mandatory. The rarity and variability of presentations could create problems of differential diagnosis both with mucinous-producing carcinomas or with other soft tissue tumours, with myxoid or reticular structure. The description of this case could help raise information on this rare neoplasm and help distinguish it from other malignancies, especially in unusual sites.

ABSTRACT

Mammals can show conspicuous behavioral responses to thermal variation, including changes in movement patterns. We used an integrative approach to understand how environmental temperature can drive the movement behavior of a mammal with low capacity for physiological thermoregulation, the giant anteater (Myrmecophaga tridactyla). We tracked 52 giant anteaters in 7 areas throughout the Brazilian savannah. We estimated the distance moved, area used, use of forest areas, and mean environmental temperature for each monitoring day of each individual. We modeled these data with Mixed Structural Equations - considering the possible interactions between our variables and controlling for sex and body mass. Giant anteaters reduced displacement and increased forest use with decreasing environmental temperature, probably because of their low body heat production. It is possible that they reduce distance moved and area used by reducing the duration of activity. With decreasing temperature, forest habitats become warmer than open ones, besides buffer rain and chilly winds. Reducing displacement and using forests are important strategies to reduce body heat loss and the energetic costs of thermoregulation. However, decreasing movement can limit food access and, consequently, fitness. Therefore, we highlight the importance of forests as thermal shelters. With increasing frequency and intensity of extreme weather events, we showed the need to preserve forest patches to offer suitable conditions for tropical mammals' behavioral thermoregulation. In this context, policies favoring deforestation on Brazilian territory are especially worrisome. Finally, we emphasize the need of integrative approaches to understand the complex interactions between organisms and the environment.

ABSTRACT

Abstract: Despite their negative environmental impacts, human-modified environments such as agricultural and urban landscapes can have a relevant role on biodiversity conservation as complements of protected areas. Such anthropized landscapes may have endangered, valuable, and nuisance species, although most of them do not fit in any of these categories. Therefore, in such environments we must deal with the same decision-making process concerning the same possible interventions proposed by Caughley (1994) to wildlife management, which are related to biological conservation, sustainable use, control/coexistence, and monitoring. Such decision-making process should be based on good science and good governance. On such context, the first step should be to implement multifunctional landscapes, which keep their primary mission of human use, but incorporate a second but fundamental mission of biological conservation. In this study we present a summary of the research carried out at the Biota Program of Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) in this field since the late 1990's and propose priorities for biodiversity research and governance in multifunctional landscapes for the near future.


Resumo Apesar de seus impactos ambientais negativos, ambientes modificados pelo homem, como paisagens agrícolas e urbanas, podem ter um papel relevante na conservação da biodiversidade como complementos de áreas protegidas. Tais paisagens antropizadas podem ter espécies ameaçadas, valiosas e incômodas, embora a maioria delas não se enquadre em nenhuma dessas categorias. Portanto, em tais ambientes devemos lidar com o mesmo processo de tomada de decisão sobre as mesmas possíveis intervenções propostas por Caughley (1994) para o manejo da vida selvagem, que estão relacionadas à conservação biológica, uso sustentável, controle/coexistência e monitoramento. Esse processo de tomada de decisão deve ser baseado em boa ciência e boa governança. Neste contexto, o primeiro passo deverá ser a implementação de paisagens multifuncionais, que mantenham a sua missão primordial de uso humano, mas que incorporem uma segunda, mas fundamental missão de conservação biológica. Neste estudo apresentamos um resumo das pesquisas realizadas no Programa Biota da Fundação de Amparo à Pesquisa do Estado de São Paulo (FAPESP) neste campo desde o final da década de 1990 e propomos prioridades para pesquisa e governança da biodiversidade em paisagens multifuncionais para o futuro próximo.

ABSTRACT

Hypersensitivity pneumonitis (HP) is a diffuse interstitial lung disease (ILD) caused by the inhalation of a variety of antigens in susceptible individuals. Patients with fibrotic HP (fHP) may show histopathological and radiological manifestations similar to patients with idiopathic pulmonary fibrosis (usual interstitial pneumonia-like pattern of fibrosis) that are associated with a worse prognosis. We describe here the establishment and characterization of a fibroblastic cell line derived from the broncho-alveolar lavage (BAL) of a patient with fHP, a 53 year old man who presented at our Pneumology Unit with cough and dyspnea. The fHP diagnosis was based on international criteria and multidisciplinary discussion. Primary fibroblasts were expanded in vitro until passage 36. These fibroblasts displayed morpho/phenotypical features of myofibroblasts, showing high positivity for α-smooth muscle actin, type I collagen, and fibronectin as determined by quantitative RT-PCR and cyto-fluorographic analysis. Cytogenetic analyses further evidenced trisomy of chromosome 10, which interestingly harbors the FGF2R gene. To our knowledge, this is the first fibroblastic cell line derived from an fHP patient and might, therefore, represent a suitable tool to model the disease in vitro. We preliminarily assessed here the activity of pirfenidone, further demonstrating a consistent inhibition of cells growth by this antifibrotic drug.

ABSTRACT

Habitat loss is the main threat to biodiversity conservation worldwide. Some species may be particularly susceptible to the effects of fragmentation and the isolation of populations. The impacts of human activity on wild animal populations may be understood through relationships between individual genetic data and spatial landscape variables, particularly when considering local population dynamics influenced by fragmented habitats. Thus, the objective of this study was to analyze the population structure and genetic diversity of the giant anteater (Myrmecophaga tridactyla) using an individual sampling scheme (ISS) on a regional geographic scale. Data were collected from 41 specimens from twenty different locations in São Paulo State, Brazil, and six polymorphic microsatellite loci were genotyped. Our results indicate that barriers to gene flow exist and have segregated individuals of the farther away areas into two spatially structured clusters. The populations were also found to have high genetic diversity. The experimental sampling approach used herein enabled an analysis of the population dynamics of the giant anteater on a regional scale, as well as the identification of priority populations for genetic resource conservation for this species. The results reflect the need for adequate management plans. The efficacy of the sampling scheme may vary based on the study model used, but we argue that the use of an ISS combined with suitable molecular markers and statistical methods may serve as an important tool for initial analyses of threatened or vulnerable species, particularly in anthropized regions where populations are small or hard to characterize.

Data on the pathology of COVID-19 are scarce; available studies show diffuse alveolar damage; however, there is scarce information on the chronologic evolution of COVID-19 lung lesions. The primary aim of the study is to describe the chronology of lung pathologic changes in COVID-19 by using a post-mortem transbronchial lung cryobiopsy approach. Our secondary aim is to correlate the histologic findings with computed tomography patterns. SARS-CoV-2-positive patients, who died while intubated and mechanically ventilated, were enrolled. The procedure was performed 30 min after death, and all lung lobes sampled. Histopathologic analysis was performed on thirty-nine adequate samples from eight patients: two patients (illness duration < 14 days) showed early/exudative phase diffuse alveolar damage, while the remaining 6 patients (median illness duration-32 days) showed progressive histologic patterns (3 with mid/proliferative phase; 3 with late/fibrotic phase diffuse alveolar damage, one of which with honeycombing). Immunohistochemistry for SARS-CoV-2 nucleocapsid protein was positive predominantly in early-phase lesions. Histologic patterns and tomography categories were correlated: early/exudative phase was associated with ground-glass opacity, mid/proliferative lesions with crazy paving, while late/fibrous phase correlated with the consolidation pattern, more frequently seen in the lower/middle lobes. This study uses an innovative cryobiopsy approach for the post-mortem sampling of lung tissues from COVID-19 patients demonstrating the progression of fibrosis in time and correlation with computed tomography features. These findings may prove to be useful in the correct staging of disease, and this could have implications for treatment and patient follow-up.

Spatial ecology data are essential for conservation purposes, especially when extinction risk is influenced by anthropogenic actions. Space use can reveal how individuals use the habitat, how they organize in space, and which components are key resources for the species.We evaluated the space use and multiscale habitat selection of giant anteaters (Myrmecophaga tridactyla), a vulnerable Neotropical mammal, in a Cerrado site within a human-modified landscape in southeastern Brazil.We used GPS transmitters to track eight anteaters in the wild. With the resulting dataset, we estimated home range and core-area sizes and then used two overlap indexes. We assessed habitat selection by compositional analysis and analyzed events of spatio-temporal proximity.The average Brownian bridge kernel estimate of home range size was 3.41 km2 (0.92-7.9). Regarding home range establishment, five individuals showed resident behavior. Males (n = 4) had larger home ranges and were more active than females (n = 4). Despite the spatial overlap of home range (above 40% in four dyads), maximum temporal space sharing was 18%. Giant anteaters were found in proximity. Habitat selection favored savanna, and exotic timber plantation was always avoided. Roads and built-up areas were selected secondarily at the landscape level.The selection of anthropogenic sites denotes behavioral plasticity regarding modified habitats. However, the high selectivity for savanna, at all levels, demonstrates a high dependence on natural habitats, which provide the necessary resources for the species. The recurrent proximity of male-to-female anteaters may indicate reproductive behavior, which is essential for maintaining this isolated population.

INTRODUCTION: Malignant lymphoproliferative disorders are rarely observed in the lung and, considering their clinical and radiological heterogeneity, diagnosis is often difficult and may require invasive methods. Transbronchial cryobiopsy has been confirmed as a new tool in the diagnosis of interstitial lung diseases, given its fewer risks and costs compared to surgical approach. This study is aimed at assessing the effectiveness of cryobiopsy in the diagnosis of lymphoproliferative disorders. MATERIALS AND METHODS: Among 970 consecutive cryobiopsies, performed between January 2011 and June 2018 at Morgagni Hospital of Forlì, Italy, 13 cases of lymphoproliferative disorders were collected. RESULTS: In 12 out of 13 cases a precise pathological diagnosis could be reached with the support of immunohistochemistry (IHC) and molecular ancillary studies. In the only case in which cryobiopsy did not lead to a definitive diagnosis, the subsequent surgical biopsy also did not help to clarify the diagnosis. Severe bleeding or pneumothorax did not occur in any case. On average, five biopsies were obtained per case, with a mean total area of 1161 mm2, and only 5 out of 65 specimens were inadequate for diagnosis. Instant freezing did not produce tissue artefacts nor did it affect IHC and molecular tests. In all cases the amount of available tissue was sufficient for all ancillary studies. CONCLUSIONS: Transbronchial lung cryobiopsy is safe and effective for diagnosis in patients with suspected pulmonary involvement by lymphoproliferative disorders and it should therefore be considered a valid alternative to surgical biopsy in such cases.

Pancreatoduodenectomy is one of the most challenging surgical specimens for pathologists. Recently, two different, standardized protocols have been proposed: the axial slicing Leeds protocol (LP) and the bi-valving Adsay protocol (AP). Comparison between standardized and non-standardized protocols (NSP) was performed with emphasis on margin involvement and lymph node yield. Pancreatoduodenectomy cases were retrospectively recruited: 46 sampled with LP, 52 cases with AP and 46 cases with NSP. Clinico-pathologic data and rates of margin/surface involvement were collected and their prognostic influence on survival was assessed. Statistical differences between NSP and AP and LP were seen for nodal yield (p = 0.0001), N+ (p = 0.0001) and lymph node ratio - LNR (p < 0.0008) but not between AP and LP. Differences in R1/R0 status were statistically significant between NSP group (R1-15%) and both the LP (R1-73.9%) and AP (R1-70%) groups (p = 0.0001) but not between LP and AP groups. At univariate survival analysis, grade (p = 0.0023) and number of involved margins (p = 0.0096) in AP and "N-category" (p = 0.0057) "resection margin status" (p = 0.0094), "stage" (p = 0.0143), and "number of involved margins" (p = 0.00398) in LP were statistically significant, while no variable was significant in the NSP group. At multivariate analysis "N category," "resection margin status," "stage," "number of involved margins," and "LNR" retained significance for the LP group. These results show that both LP and AP perform better than non-standardized sampling making standardization mandatory in pancreatoduodenectomy cut up. Both AP and LP show strengths and weaknesses, and these may impact on the choice of protocol in different institutions.

ABSTRACT

Gigantorhynchus echinodiscus (Diesing, 1851) is a parasite of anteaters in South America. Although described by Diesing in 1851, there is still a lack of taxonomic and phylogenetic information regarding this species. In the present study, we redescribe G. echinodiscus collected from a giant anteater, Myrmecophaga tridactyla Linnaeus, 1758, from the Brazilian Cerrado (Savannah) in the State of São Paulo by light and scanning electron microscopy. In addition, phylogenies were inferred from partial DNA gene sequence of the nuclear large subunit ribosomal RNA gene (28S rRNA). We provide for the first time details of the proboscis with a crown having 18 large hooks and numerous small hooks, a lateral papilla at the base of the proboscis, a ringed pseudo-segmented body, large testes, cemented glands in pairs, and a non-segmented region in the posterior end of the body, which contributed to the diagnosis of the species. Molecular phylogenetic analyses recovered G. echinodiscus forming a well-supported monophyletic group with Mediorhynchus sp., which was congruent with morphological studies that allocate both genera within the family Gigantorhynchidae. In conclusion, the present work adds new morphological and molecular information, emphasizing the importance of adopting integrative taxonomic approaches in studies of Acanthocephala.

Reliable predictors of benefit from immune checkpoint inhibitors in non-small-cell lung cancer (NSCLC) are still limited. We aimed to evaluate the association between the expression of selected molecules involved in immune response and clinical outcomes in NSCLC patients receiving nivolumab. In our study, the outcomes of 46 NSCLC patients treated with nivolumab in second or subsequent lines (Nivolumab Cohort) were compared with the expression of PD-L1, PD-L2, PD-1, B7-H3, and B7-H4 assessed by immunohistochemistry (IHC). Samples from 17 patients (37.0%) in the Nivolumab Cohort were positive for B7-H4 expression. At univariate analyses, only B7-H4 expression was associated with significantly decreased progression-free survival (PFS; 1.7 vs. 2.0 months; p = 0.026) and with a disadvantage in terms of overall survival (OS) close to statistical significance (4.4 vs. 9.8 months; p = 0.064). At multivariate analyses, B7-H4 expression was significantly associated with decreased PFS (hazard ratio (HR) = 2.28; p = 0.021) and OS (HR = 2.38; p = 0.022). Subsequently, B7-H4 expression was compared with clinical outcomes of 27 NSCLC patients receiving platinum-based chemotherapy (Chemotherapy Cohort), but no significant association was observed. Our results suggest a negative predictive role of B7-H4 in a population of NSCLC treated with immune checkpoint inhibitors, which deserves further research.

Pleuroparenchymal fibroelastosis is characterized by upper lobes subpleural intra-alveolar fibrosis and elastosis with visceral pleural fibrosis, which may occur after allogenic haematopoietic stem-cell transplantation (HSCT). The longitudinal changes of lung function preceding this complication have not been described. We report the case of an adult woman undergoing allogeneic HSCT for Hodgkin's lymphoma. Pulmonary function tests evolved from normal, before transplantation, to a restrictive pattern with normal residual volume 3 months after transplantation, then to an obstructive pattern consistent with bronchiolitis obliterans 18 months after transplantation, and finally to a severe mixed pattern with preserved residual volume. Computed tomography showed the distinctive features of pleuroparenchymal fibroelastosis, confirmed by histology of specimen from apical resection after pneumothorax. This case report suggests that pleuroparenchymal fibroelastosis may occur after HSCT following bronchiolitis obliterans syndrome with a mixed (restrictive-obstructive) lung function pattern.

INTRODUCTION: Pachysentis comprises 10 species, which have been reported parasitizing mammals in Africa and the American continent. However, species of Pachysentis have not been described in brow-nosed coatis. Pachysentis lauroi n. sp. (Oligacanthorhynchidae: Acanthocephala) is described from the brown-nosed coati Nasua nasua (Linnaeus, 1766) Storr, 1780 (Procyonidae: Carnivora) in the Brazilian Pantanal wetlands of the Mato Grosso do Sul State, Brazil. METHODS: Specimens were studied using light and scanning electron microscopy. RESULT: The new species is distinguished from other species of Pachysentis by the number of hooks in each longitudinal row (12 rows of 4 hooks, total of 48 hooks), presence of barbs on all hooks, and the organization of the cement glands. Notes on the genus Pachysentis [14] and a key to its species are provided. Critical comments on some species with a dubious diagnosis and questionable or missed key taxonomic characteristics are also reviewed. We also discuss the zoogeography of the members of the genus.

ABSTRACT

Perianal fistula is a very debilitating event and a cause of morbidity in patients with Crohn's disease (CD). Its malignant transformation is very rare with an incidence of around 0.004-0.7$. Presence of disease in the colon and rectum is the major risk factor for the development of a perianal fistula. In this report we show a case of adenocarcinoma arising from a perianal fistulizing CD. This type of tumor is highly aggressive, difficult to diagnose, and has a rather poor prognosis. The different neoplastic transformations and the different types of tumors that may appear in patients with CD, especially at the colorectal level or at the level of an eventual anastomosis, are to date well documented and described in the literature, while there is a lack of information and of treated cases concerning the occurrence of cancer at the level of a fistula. Due to the rarity of cases, we tried to identify the most frequent and important risk factors: sex, duration of disease, age at diagnosis, and the history of the fistula.

ABSTRACT

Pneumomediastinum is defined as the presence of air or gas within the mediastinum and it rarely complicates bronchoscopy. We report, to our best knowledge, the first case of pneumomediastinum following a transbronchial cryobiopsy (TBLC). TBLC is considered a safe procedure as compared with both transbronchial biopsy and surgical lung biopsy. Systematic reviews, metanalysis and a Pubmed research, revealed that in literature no pneumomediastinum has been mentioned after TBLC. We report this case for to make it known to interventional pulmonologists the possibility that a pneumomediastinum can follow a TBLC. In our case the spontaneous resolution in few days did not require any intervention.

ABSTRACT

Abstract The Bare-faced Curassow Crax fasciolata (Cracidae:Cracinae) is a vulnerable species that was classified as critically endangered by the São Paulo State -Brazil, in 2008. Specialists recommended "searches for areas where there may be a few remaining populations and protection of the last remaining semideciduous forest in the State". Protected areas were investigated in the Cerrado and semideciduous forest in São Paulo State by camera trapping to evaluate the presence of the Bare-faced Curassow. Eight protected areas in Cerrado biome were investigated, but the Bare-faced Curassows was recorded only at Furnas do Bom Jesus State Park (2,069 ha), São Paulo State despite the large sampling effort in larger protected areas such as the Santa Bárbara Ecological Station (2,712 ha) and Jataí Ecological Station (9,074 ha); the largest protected area of Cerrado biome in São Paulo State. This record of C. fasciolata is important for the Conservation Action Plan for the Cracid family and shows the importance of monitoring biodiversity and conserving even relatively small remnants of native vegetation for the conservation of this species.


Resumo O mutum-de-penacho Crax fasciolata (Cracidae:Cracinae) é uma espécie ameaçada de extinção e foi considerada criticamente em perigo em 2008 no estado de São Paulo. As recomendações dos especialistas para a conservação da espécie no estado foram "a pesquisa por áreas onde poderia existir populações remanescentes e a proteção de remanescentes de floresta semidecidual". Neste trabalho, oito Unidades de Conservação foram investigadas em áreas com vegetação de Cerrado e de floresta semidecidual por meio de armadilhas fotográficas digitais para verificar a presença do mutum-de-penacho (Crax fasciolata). Entretanto, o mutum-de-penacho foi registrado apenas no Parque Estadual Furnas do Bom Jesus, que possui 2.069 ha, apesar do maior esforço amostral em áreas maiores, como a Estação Ecológica de Santa Bárbara (2.712 ha) e Estação Ecológica de Jataí (9.074),- a maior Unidade de Conservação do bioma Cerrado no estado de São Paulo. O registro do mutum-de-penacho no Parque Estadual Furnas do Bom Jesus é muito importante para o Plano de Ação e Conservação da família Cracidae e mostra a importância do monitoramento da biodiversidade e a conservação de remanescentes de vegetação nativa, mesmo pequenos, para a conservação dessa espécie.

ABSTRACT

We investigated the home range size, habitat selection, as well as the spatial and activity overlap, of four mid-sized carnivore species in the Central Pantanal, Mato Grosso do Sul, Brazil. From December 2005 to September 2008, seven crab-eating foxes Cerdocyon thous, seven brown-nosed coatis Nasua nasua, and six ocelots Leopardus pardalis were radio-collared and monitored. Camera trap data on these species were also collected for the crab-eating raccoon Procyon cancrivorus. We hypothesized that there would be large niche differentiation in preferred habitat-type or active period between generalist species with similar diet, and higher similarity in habitat-type or activity time between the generalist species (crab-eating foxes and coatis) and the more specialized ocelot. Individual home ranges were estimated using the utilization distribution index (UD- 95% fixed Kernel). With data obtained from radio-collared individuals, we evaluated habitat selection using compositional analysis. Median home range size of ocelots was 8 km2. The proportion of habitats within the home ranges of ocelots did not differ from the overall habitat proportion in the study area, but ocelots preferentially used forest within their home range. The median home range size of crab-eating foxes was 1.4 km2. Foxes showed second-order habitat selection and selected savanna over shrub-savanna vegetation. The median home range size for coati was 1.5 km2. Coati home ranges were located randomly in the study area. However, within their home range, coatis occurred more frequently in savanna than in other vegetation types. Among the four species, the overlap in activity period was the highest (87%) between ocelots and raccoons, with the least overlap occurring between the ocelot and coati (25%). We suggest that temporal segregation of carnivores was more important than spatial segregation, notably between the generalist coati, crab-eating fox and crab-eating raccoon.
