ABSTRACT
In a preliminary study of 32 women with stress urinary incontinence, a modification was added to the standard suprapubic urethropexy to decrease the incidence of postoperative urinary retention. Immediately before the elevating sutures are tied, a urethroscope is inserted into the midurethra and the bladder neck is visualized while the sutures are being tied with just enough tension to close the bladder neck. This resulted in a 100 per cent continence rate at twenty-four months and an average catheter time of 6.2 days, with 69 per cent of the patients voiding easily on the fourth postoperative day. This has resulted in a significant improvement over the length of catheter time when the urethroscope is not used, and additionally provides good experience and immediate feedback in a teaching situation for accuracy of suture placement and adequacy of bladder neck closure.

Ten patients with biopsy-proved interstitial cystitis were treated with a program of self-administered dimethyl sulfoxide (DMSO) using the technique of intermittent self-catheterization. Nine of the 10 patients responded favorably to the treatment and are on a maintenance program. The technique was well tolerated and without complications.

A bladder cancer presented as a calcified pelvic mass on roentgenograms. The frequency and significance of calcified bladder tumors is discussed.

Congenital mesoblastic nephroma is the most common neoplasm in the neonatal age group. It is generally benign in nature and treatment consists solely of complete excision. Congenital mesoblastic nephroma has been mistaken for Wilms tumor and subsequently overtreated. Awareness of congenital mesoblastic nephroma will aid in giving proper therapy.

This series of 24 female patients with stress urinary incontinence managed surgically by a pubis-pubovaginal fascia fixation demonstrates success comparable or superior to other techniques. Several advantages in carefully selected patients are noted.

A recent case of primary osteosarcoma of the kidney prompted a review of the literature for this rare malignancy. Six cases have been recorded, and this case represents the seventh. The clinical and pathologic features of this lethal tumor are discussed. The presentation of flank pain, gross hematuria, a palpable mass, and a localized parenchymal calcification on x-ray film should alert the clinician to the possibility of renal osteosarcoma.
