ABSTRACT

The authors have got their first experience with chemohyperthermal perfusion of the lung in 4 patients. The method and technique of the operation of ablation of metastases in the lungs with preliminary chemohyperthermal perfusion are described. All the patients survived the operative interventions.

ABSTRACT

Major vascular aneurysm is a severe pathology with poor prognosis. Due to wide variations of vascular anatomy, an endovascular intervention may require intraoperative adjustment of procedural strategy. The paper presents results of successive stent-graft implantations in different anatomical situations.

ABSTRACT

Comparative activity of cefepime, ceftazidime, ceftriaxone, ciprofloxacin, imipenem and piperacillin/tazobactam against isolates from patients of pediatric intensive care units within October-December 1998 was studied. The isolates were identified with the Walkaway-40 System. The antibiotic susceptibility was evaluated by the E-test on the Mueller-Hinton II agar. The data were interpreted in accordance with the NCCLS. The number of the isolates totaled 100. Among them not more than 10 to 12 strains belonged to the same species. 92 per cent of the isolates was susceptible to cefepime and 70 to 75 per cent of the isolates were susceptible to the third generation cephalosporins. Piperacillin/tazobactam proved to be highly susceptible, the number of the isolates resistant and moderately resistant to it being 6 and 1 per cent respectively. The results showed that cefepime was intermediate between the third generation cephalosporins and carbapenems.

ABSTRACT

The article presents an analysis of results of treatment of 39 children from 1 to 14 years of age with diagnosis of appendicular peritonitis complicated by septic shock. It was shown that complex pathogenetic treatment with controlled laparostomy, necessary compulsory or subsidiary artificial pulmonary ventilation before and after operative intervention, associated use of afferent methods allowed to decrease the number of lethal outcomes from 54.5 to 11.1%.

ABSTRACT

A total of 42 children aged 6 months to 14 years who had the portal hypertension syndrome were studied. Blood and a lymph node were taken during a planned surgery by applying venous anastomosis. Translocation to the veins of the small and large bowels and to the mesenteric lymph nodes were recorded in 35 (83.3%), 34 (81%), and 17 (41%) patients, respectively. Translocation of aerobic bacteria (81, 78.6, and 40.5%) was more common than that of anaerobic ones (11.9, 11.9, and 4.8%) to the blood of the small and large bowels and lymph nodes, respectively, gram-negative bacteria heading the list in their frequency, Streptococci ranking next to the latter. The data on the composition of fecal microflora in the same children prior to surgery are given in the paper, which suggests that they had compensatory dysbacteriosis. A contribution of various factors (portal hypertension, operative stress, dysbacteriosis, etc.) to the development of translocation and its pathogenetic value are discussed too.

ABSTRACT

The problem of prophylactics of postoperative infectious complications in the surgery of digestive tract is discussed. A comparative analysis of efficiency of two methods of antibacterial prophylactics during operations of closing the formed intestinal fistulas in children is given. High efficiency of the method of preoperative subtotal decontamination of the intestine according to individually selected schemes is shown.

The paper presents analysis of current knowledge on etiology and immunopathogenesis of multiform exudative erythema (MEE). Among a variety of pathogenetic actions of herpes simplex on immune system are those relevant to MEE onset. These variants are dealt with in detail. The view on MEE as resultant from herpes simplex infection promises appearance of new prospective modes of etiotropic therapy.

The paper describes a case of lepromatous lepra diagnosed from the skin biopsy specimen taken in a 23-year-old male patient. It provides histologic signs that are beneficial for correct diagnosis. Emphasis is placed upon the necessity for staining by the Ziehl-Neelsen method to detect microorganisms in infiltrative and granulomatous skin lesions.
