ABSTRACT

We report about a secondary malignant fibrous histiocytoma of bone which is most frequently found in distal femur. It appears as a malignant form of a giant-cell tumor after the exposure to radiation. The typical symptoms given in literature publications: pain, swelling and the pathological fracture were present. After amputation there was no metastasis for 15 months.

ABSTRACT

There are different procedures for surgical treatment of a recidivating ventral dislocation of the shoulder. In our hospital the rotatory osteotomy according to Weber proved good; in accordance with the findings it can be combined with other procedures. The surgical procedures and our results are explained. In our opinion, this surgical treatment will effect, besides known results (rotation of head defect to lateral), an essential stabilisation of the joint, by means of strain exercised on M. infraspinatus and the upper part of M. teres minor.

ABSTRACT

There are numerous methods for surgical treatment of osteochondrosis dissecans of the knee. These methods and their indications are pointed out. In our clinic screw-fixation of dissecates respectively of osteocartilaginous grafts proved good. Surgical procedure, after-treatment and results are reported.

ABSTRACT

The aetiology of Morbus Ahlbäck (spontaneous osteonecrosis of the femoral condyles of the knee joint) is still unknown. Sudden onset of pain characterises the beginning of complaints. Only scintimetry results in early diagnosis. Larger osteonecroses are to be treated surgically, for example by osteotomy. In extensive osteonecroses we prefer alloarthroplastic replacement (mostly unicondylar).

ABSTRACT

The decrease in the incidence of tuberculosis may today readily result in a lack of attention being paid to the extrapulmonary forms of this disease in the differential diagnosis of bone changes. Today massive joint destruction can be prevented with the aid of anti-tuberculous chemotherapy, the early establishment of the diagnosis (for example arthrotomy) and surgical intervention (synovectomy or excision of focus, osteosynthesis). On the basis of our patients treated between 1947 and 1982 we describe the diagnostic and therapeutic procedures followed over the last decades. The results are discussed. Our present therapeutic concept is explained.

ABSTRACT

In an attempt to clarify the question if the level walkway and the treadmill present comparable conditions for analytic examinations of the human gait we investigated in either situation with a computer-controlled motion analysis system some kinematic and electromyographic aspects of the gait pattern of 20 healthy subjects. The results show that on the treadmill certain kinematic events in the time pattern of the gait cycle are shifted towards the beginning of the full stride. Moreover, in the treadmill walk the amplitude of the angular motion of the thigh is diminished in the forward and enhanced in the backward direction, while the angular amplitude of the shank is diminished in either direction. As to the electromyographic results, we found in the treadmill walk a prolonged activity of the gluteus maximus, and adductor magnus, the semitendinosus, and the gastrocnemius muscle. These results show that there may exist significant kinematic and electromyographic differences between the treadmill and the normal level walk. This fact has to be considered whenever a treadmill is considered for gait analytical investigations.

ABSTRACT

19 patients were treated via a tumour prosthesis in the hip joint region. The indication resulted from osteolyses, loosening of prosthesis and subtrochanteric fractures in aged patients involving risk factors. The results show that application of a tumour prosthesis is a suitable method for bridging over major defects in the proximal femur to achieve early mobilisation of the patients associated with full load of the extremity on which the operation had been performed.

ABSTRACT

The article reports on seven patients with a histologically established chondromatosis of the joints. The aetiology, diagnosis, pathologico-anatomic findings and treatment of this rare joint disease are discussed. The case history of a patient serves to illustrate how a benign chondromatosis can undergo malignant degeneration.

ABSTRACT

The article describes a few guiding points for indicating the need for a knee-joint endoprosthesis, basing on experience with 253 cases, followed up for 2--48 months. Following the application of prostheses according to Walldius-Debeyre and Guepar, several complications were seen which we tried to reduce by introducing four prostheses of different sizes, since the choice of the implantates could be adapted to the prevailing anatomical conditions. A hinged-joint prosthesis of the St. George type is indicated in cases of most severe axial deviation, severe flexion contraction, extensive loss of bone substance, and instability of the ligaments. Sheeban's prosthesis is indicated in severe damage involving both compartments, limited frontal axial deviation, and flexion contraction of not more than 25 degrees with intact lateral ligaments. In younger patients with rheumatoid arthritis we implant the Geomedic prosthesis. If only one knee-joint compartment is involved, and after the possibility of readjustment osteotomy has been excluded, we consider a sliding prosthesis to be the prosthesis of choice.

ABSTRACT

In mechanical devices in general the friction plays an important role in force transmission. To find out about the magnitude of frictional forces between internal fixation plate and bone in vivo, simultaneous strain recording in plated bone and instrumented plate were performed. After 3 weeks of implantation the torque applied to a single plate-screw was set to definite levels. Thus it could be determined at what level of screw compression and at what tangential load of the related surface bone-plate, gliding occurred. The coefficient of friction was found to be 0.2 +/- 0.09 (x +/- Sx). It can be concluded that in vivo friction produced at the plate-bone contact surface is able to guarantee stability of fixation.

ABSTRACT

A double-blind placebo trial was taken with Lioresal, and antispasticity drug acting on the spinal cord level. The drug produced improvement of voiding function in 13 paraplegics with upper motor neuron lesions and detrusor external sphincter dyssynergia. As a main result, the opening time was reduced.

Eelctromagnetic and a.c.-stimulation according to the method of Kraus was investigated using experimental hypertrophic delayed unions of the radius in beagles. We could not show any beneficial or adverse effect of the method of Kraus.

ABSTRACT

The effect of the magnetic field according to the Kraus-Lechner model was investigated on experimental pseudarthroses in beagle dogs. Double-blind evaluation of the 12 experimental pseudarthroses demonstrated that the magnetic field, applied for 3 months, had no effect on healing.
