ABSTRACT

Four patients with the scimitar [correction of yataghan] syndrome underwent examinations and surgical management. The syndrome includes abnormal drain of the right pulmonary vein into the vena cava inferior, abnormality of the right lung, right displacement of the heart and right pulmonary hypoplasia. Three patients were diagnosed as having the scimitar [correction of yataghan] syndrome during examinations and the defect was found in 1 patients during surgery. Cardiac catheterization with selective angiopulmonography is one of the most informative diagnostic techniques. The methods of choice in surgical management involve direct reimplantation of the abnormal pulmonary vein into the left or right atrium, followed by transfer of its ostium into the left atrium.

ABSTRACT

The authors analyze their experiences with surgical treatment of 33 patients aged from 3 years 10 months to 44 years having a defect of the venous cardiac sinus. In all the cases a modified method of septoplasty was used. There were no lethal outcomes after the correction by the method proposed. Two patients had complications: transient atrio-ventricular blockade with the recovery of the sinus rhythm on the 11th day after operation and right ventricle insufficiency at the nearest postoperative period. At later terms from 1 to 11 years 22 patients were examined. After catheterization of heart cavities which was fulfilled on 12 patients the pressure gradient between the vena cava superior and the right auricle was found in 3 patients and was not more than 5 mm Hg. Recanalization of the defect was detected in 1 patient. The method recommended is considered by the authors to allow effective correction of the defect of the venous sinus of the heart.

ABSTRACT

An analysis of roentgenograms of 50 patients with a respiratory distress-syndrome has shown that roentgenosemiotics of this complication discloses alterations of permeability of pulmonary and bronchial capillaries, drainage of liquid from the interstice and alveoli, hemodynamics in the microcirculation link of the lesser circulation circle. A classification of the respiratory distress-syndrome is proposed which includes 3 stages and a period of involution of the process.

ABSTRACT

An analysis of 299 operations on the thoracic part of the esophagus has been made. The authors consider the method of choice to be a one-step operation of the Lewis type in cancer and scarry strictures of the esophagus. Clinical signs of diverticulitis even of a small degree are thought to be indication to diverticulectomy.

ABSTRACT

An experience with the surgical treatment of 30 patients has been analyzed. A conclusion is made of the efficiency of correction of the supracardial form of a partial anomalous drainage of the pulmonary veins with the help of septoplasty modified by the authors.

ABSTRACT

The authors analyse the experience with the surgical treatment of about 3000 patients with different forms of cardiac pathology and general lethality in 4,1% of cases. Favorable results were obtained following operations in children of the first three years of life with the open arterial duct, in 4 to 6-year-old children with coarctation of the aorta. The authors show expediency of correction of septal defects before severe disturbances of hemodynamics could develop. The closed method of correction is indicated in non-complicated forms of mitral stenosis. Patients with complicated forms of mitral stenosis, mitral insufficiency should be operated upon under conditions of artificial blood circulation.

ABSTRACT

A total of 175 adult patients with congenital heart diseases were investigated, of whom 140 were operated upon and 62 examined late after surgery. As noted, characteristic of adult patients were subjective complains, progressive pulmonary hypertension in defects with the blood shunting from the left to right and, because of this, the emergence of atypical symptoms of the defects. Surgical correction yields quite satisfactory results, but the presence of pulmonary hypertension tends to greatly increase the operative risk.
