ABSTRACT

BACKGROUND: Gummy smile (GS) has a direct effect on individuals, especially among young adults, because of its association with smile avoidance. The younger populations are sensitive about their smiles and prefer aesthetic, beautiful smiles, a lack of which can negatively impact their quality of life. OBJECTIVES: This study aims to measure the GS prevalence among young adults aged 16 to 18 attending high schools in Ha'il City, Saudi Arabia, evaluating oral health related to quality of life (OHQoL) in those suffering GS by using the OHQoL questionnaire (OHIP-14). METHODS: A cross-sectional study was conducted on 385 female high school students located in Ha'il. Students with GS took a survey on oral health using OHIP-14. For this, SPSS was used to analyze the data. RESULTS: The study included 200 people with GS (52%). The mean age was 18±0.01. The prevalence of GS was analyzed, with a mean value of 4.68±1.2 mm, indicating most students had GS ranging between 4 and 5 mm. The most frequent value for all items in the OHIP-14 questionnaire was 1, indicating that students often had their quality of life affected. The non-parametric Kruskal-Wallis test indicated the results had a significant value (p < 0.05), showing a positive and significant association. CONCLUSION: Based on the OHIP-14 questionnaire and respecting the methodology, it was concluded that the quality of life has been affected for all female students with GS. The high prevalence for ages 16-18 showed most students agreed their lives were being affected by GS and their condition needed to be treated. It was also confirmed by the significant association of GS with items of oral health and quality of life.

ABSTRACT

BACKGROUND: The objective of the study was to retrospectively assess the clinical performance of dental prostheses, single crowns, and three-unit bridges to identify clinical biological and mechanical complications in the Ha'il province of Saudi Arabia. METHODS: The study was conducted between March 2021 to October 2021 and included 421 patients who underwent crown and tooth-supported fixed partial denture (FPD) procedures at the Dental Polyclinics Center in the Ha'il region of Saudi Arabia from 2010-2020. The planned sampling approach was applied. Patients who underwent crown and FPD placements at the dental center within the scheduled period were identified through clinical assessments. The inclusion criteria were met by patients with three-unit FPDs and a single crown containing a natural contralateral tooth or teeth. A total of six general dentists conducted clinical exams. Patient satisfaction and technical and biological issues were the evaluation criteria for crowns and FPDs. The frequency and percentage of the various characteristics employed in the current investigation were ascertained using cross-tabulation. The Chi-square test was employed to assess the associations between categorical variables, with p-values ≤ 0.05 considered significant.  Results: Marginal integrity was recorded in 332 participants (78.9%), which was satisfactory for the majority of the restoration. The acceptance morphology was present in 252 (59.9%) restorations. The highest rate of the restorations 274 (66.2%) had satisfactory color. In 86 cases (20.4%), there was visible periodontal depth of at least 5 mm. Three hundred and six (72.7%) of the fixed restorations had gingival bleeding connected to them, and 96 (22.8%) teeth had periapical lesions. A total of 311 patients (73.9%) reported they were satisfied with their fixed restorations. CONCLUSIONS: The technical and biological complications noted in the current study were not higher compared with other studies of crowns and fixed dental prostheses. The majority of patients were satisfied with the restoration.

ABSTRACT

HISTORY AND CLINICAL FINDINGS: For two months a 73-year-old woman had been suffering from intermittent colicky upper abdominal pain. She had also noted transient colourless stools and finally jaundice. Suspected cholelithiasis led to her admission for cholecystectomy. She had slight jaundice of skin and sclerae. Physical findings were unremarkable except for pain on epigastric palpation. INVESTIGATIONS: Sonography and computed tomography showed dilatation of the intrahepatic biliary passages and the proximal common bile duct, the latter containing a polycyclic mass that on endoscopic retrograde cholangiography appeared ovular and was surrounded by contrast medium. It was thought to be an intraductal bile stone. TREATMENT AND COURSE: Laparotomy, performed to remove the suspected stone, revealed a floating intraductal tumour, histologically a hepatocellular carcinoma. Further imaging discovered the primary hepatic tumour. CONCLUSION: Although a smooth-walled contrast-sparing mass in the common bile duct usually denotes a stone, other structures may rarely imitate this picture, in particular atypical tumour growth.
