ABSTRACT

We present a prospective study of 88 patients with thyroid nodules seen in our institution in 1985-86 and the results of a 5 year follow up. The algorithm for resolution among different therapeutic options was established in regard to the clinical characteristics, imaging and particularly the histopathologic studies. Their age ranged from 18 to 79 years; 94% of the patients were females. Most of the nodules were solid (69%) and measured 1-4 cm in diameter. The image of 62% of the thyroid scans was of a non-functioning nodule and 13% were hyperfunctioning. In 80% the ultrasonographic pattern was solid or mixed. Surgery was undertaken in 19 patients (21%). In 58%, a diagnosis of malignancy was established. The biopsy (aspiration and tru-cut) suggested the presence of the malignant tumors when taken together in 90% of the cases. Hormonal treatment was given to 62 patients; in 40-45% of them there was a significant reduction in the size of the nodule. Aspiration and sclerosis of cystic nodules were performed in 19 patients with significant shrinkage in 82%. Radioactive iodine was used in 11 patients. Our algorithm reduces costs and precludes unnecessary morbidity in patients with thyroid nodules.

ABSTRACT

Se informa un caso de cetoacidosis alcoholica en el que las determinaciones de glucosa, betahidroxibutirato y cortisol en suero resultaron elevadas. La medicion de hormona del crecimiento fue normal y la de insulina baja relativa al nivel de glucosa correspondiente. Aunque no se observo aumento del lactato, la relacion lactato/piruvato estuvo elevada. El tratamiento fue con bolo unico de 10 u. de insulina regular y liquidos intravenosos glucosados. La curva de tolerancia a la glucosa realizada despues del evento agudo fue normal.Por lo anterior y dentro de ciertos limites, se puede descartar que el sujeto fuera diabetico. Se revisan ademas los conceptos actuales de la fisiopatologia en la cetoacidosis alcoholica

ABSTRACT

Se analizo en forma retrospectiva la instalacion y/o progesion de la oftalmopatia en relacion al tratamiento y al estado de funcion tiroidea en 91 enfermos con enfermedad de Graves. La oftalmopatia se encontro al ingreso en 26 (28%) y en 7 (7,6%) se presento posterior al tratamiento. En 27 sujetos se usaron drogas antitiroideas (DAAT), 131-I en 38, DAAT mas 131-I en 16 y cirugia en 10. En 19 enfermos hubo progresion de la oftalmopatia (12 tuvieron progresion y 7 desarrollo), pero solo en 3 dicha progresion fue grave. Ningun enfermo tratado quirurgicamente mostro progresion del exoftalmos encontrando un menor porcentaje cuando se indujo y mantuvo el eutiroidismo. Se concluye que para evitar en lo posible el deterioro de la oftalmopatia en pacientes hipertiroideos con Enfermedad de Graves y exoftalmos "activo" debera evitarse la disfuncion tiroidea como consecuencia del tratamiento y utilizar tratamientos ablativos solo cuando haya seguridad de la "inactividad" clinica del proceso ocular
