ABSTRACT

The article analyzes the results of studying the effects of antihypertensive treatment with the use of agonists dopamine receptors on clinical and hemodynamic and neurohumoral indicators in hypertensive patients with concomitant obesity. Special role is given to the patogenetice factor as dopamine and his relationship with the renin - angiotensin.- aldosterone system, endothelial function, which are aimed at the regulation of blood pressure and obesity formation and progression.

ABSTRACT

In the article literature data about application of alpha-lipoic acid in medical practice are summarized. The article presents characteristic of chemical structure and functional activity of alpha-lipoic acid. LA is marketed primarily as a "weight loss" and "energy" supplement and has been branded as antioxidants. Later LA has been the subject of numerous research studies and clinical trials. LA was shown to be hepatoprotective, to improve liver circulation and treat chronic liver diseases, including jaundice hepatitis, cirrhosis, and hepatic coma, to treat diabetes, and diabetic neuropathy to alter carbohydrate metabolism, atherosclerosis, coronary atherosclerosis and many others diseases. One of the most studied clinical uses of LA is the treatment of diabetes and diabetic neuropathy LA has also been used experimentally and/or clinically to prevent organ dysfunction, reduce endothelial dysfunction and improve albuminuria, treat or prevent cardiovascular disease. LA is "small" drug with very big potential.

ABSTRACT

The results of the study hemodynamic changes in postprandial period in patients with coronary heart disease (CHD) and chronic pancreatitis (CP). It is established that patients with CHD and HP reduced pumping function of the heart after a meal in patients with isolated CP. In patients with isolated coronary artery disease marked increase in the pumping function of the heart. Reduced pumping function in patients with CP due to reduced end-diastolic index. All three groups are not significant differences in terms of diastolic function. The main differences between groups were found in the subgroup of patients with CP at the time of the survey was in acute phase. It is in this subgroup, a significant decrease not only the pump, but also inotropic cardiac function.

ABSTRACT

In many epidemiological studies, the relationship between metabolic syndrome and the development of congestive heart failure has been established. Due to the latter, an increased interest has been noticed in scientific studies about the changes in the function of the heart in patients with metabolic syndrome and possible disturbance mechanisms of the pumping function of the heart in this group of patients. In our research, 120 patients with metabolic syndrome and 20 health individuals were studied. The cardiac functions were assessed with the ultrasound imaging machine "Aloka SSD 5000" (Japan) convex sensor of frequency 3.5 MHz with generally used methods. In the course of the experiment, in patients with MS, concentric hypertrophy of the heart and disturbance of the diastolic function of the heart was noticed. Also it was set that there was a significant correlation between the indexes, which in a superlative degree demonstrate abdominal obesity (WHR) and the indexes of LV hypertrophy during diastole (r = 0.72; P < 0.001), LVM (r = 0.81; P < 0.001) and LA (r = 0.63; P < 0.001).

ABSTRACT

The authors presents in the article information on influence of endothelial dysfunction and level of blood leukocytes with clinical course of stable angina pectoris in patients with ischemic heart disease (IHD). 104 male patients aged from 36 to 69 (on average 54.9+/-2.1 years) with IHD and stage I-III stable angina pectoris have been observed. The level of leukocytes, ECG daily monitoring, endotelial function with the help of Celermaera-Sorensena test and plasma concentration of endothelin I have been studied in patients. It was established that there is no significant different (P>0.05) in the level of leukocytes between health persons and patients with IHD independently of functional class of angina pectoris. Patients with IHD had higher level of endothelin I in their blood serum than those in a control group. Patients with IHD have a complicated endothelial-non-dependent vasodilation and an increased thickness of the complex intima-media, there is a negative correlation bond between these characteristics. Patients III functional class of angina pectoris has also reliably decreased endothelial-dependent vasodilation by 37%. Frequency of ventricular extrasystoles in patients with IHD and endothelial dysfunction increases by 12 times.

ABSTRACT

In the article we discuss problem of the coexisting ulcers of stomach and duodenum in patients with atherosclerosis on the coronary syndrome, depending on the localization of the ulcer. We have been revealed that worsening of the peptic ulcer causes increasing of the ischemia episodes and intensity of the cardiac rhythm disorders. Localization of the ulcers in the stomach has significant influence on algesic ischemia and postprandial ischemia, duodenal localization increasing night ischemia. Localization of the ulcers in the stomach, especially in the upper third cause disorders of the cardiac rhythm.

ABSTRACT

The article presents results of the analysis of indicators immunogenotypic tests of 104 patients with stable stenocardia I-III functional classes. It was established that patients with stable stenocardia have lower level of CD3+ T- lymphocytes, decreased rate of T- lymphocytes helpers/ suppressors (CD4+/CD8+) and increased concentration of circulating immune complexes. These changes did not depend from Functional class of stenocardia and old myocardial infarction. It was established a close interrelation between immunological indicators on the one hand and cholesterol and leukocytosis from another. This correlation has allowed to offer the new classification approach for patients with stenocardia, which considers level of cholesterol and leukocytes.

ABSTRACT

The objective of the work was the study of particular features of fatty acid (FA) composition of the low density lipoproteids (LDL) in patients with metabolic syndrome (MS) (39,7+/-3,4 years). In patients with MS, compared to the healthy adults of the same age was revealed a significant increase in saturated FAs and significant decrease in unsaturated and polyunsaturated FAs in LDL particles. Significant correlation was revealed between indexes of abdominal adiposity, intima media thickness of arteria carotis communis, and level of some FAs in the LDLs. This changes of FA composition could be a cause of enhancing of LDL atherogenic process in patients with methabolic syndrome.

ABSTRACT

The authors presents in the article based on observations of 89 patients questions related to the changes of intracardial hemodynamics in patients with bronchial asthma and arterial hypertention (AH) observed using echocardiography. Disorders of indices of intracardial hemodynamics in patients with bronchial asthma and arterial hypertention have more evident negative tendency than that in patients with isolated bronchial asthma (BA) or arterial hypertention. It was established that such patients (without taking into consideration the degree of severity of BA) have considerably an increased volume indices of left and right ventricule, increased pressure in the lung artery, right ventricule hypertrophy, left ventricule concentric hypertrophy as well as disorders of diastolic heart function of relaxation type. Comparing indices of intracardial hemodynamics depending on the degree of severity of BA it was found that progression of lung hypertension, disorders of diastolic and systolic heart functions correlates with the degree of severity of BA. It leads to earlier formation of biventricular heart failure.
