ABSTRACT

Objetivo: Evaluar las preferencias y satisfacción de los pacientes con asma grave, relacionadas con el lugar de administración de fármacos biológicos subcutáneos: hospital, centro de salud y domicilio, durante la pandemia COVID-19. Métodos: Estudio observacional, descriptivo y transversal que analizó, mediante una encuesta telefónica realizada del 23 al 27 de noviembre de 2020, las preferencias y el grado de satisfacción con la administración de fármacos biológicos subcutáneos en pacientes asmáticos atendidos en la consulta de Alergología de un Hospital General. Resultados: Respondieron la encuesta 33 pacientes, edad media 51,5 años, 57,6% mujeres. Un 57,6% de los pacientes se administraron los fármacos (omalizumab, mepolizumab y benralizumab) en su domicilio, 21,2% en el Hospital de Día y en el Centro de Salud, respectivamente. Los motivos para la administración fuera del hospital fueron la comodidad y evitar el contagio por virus SARS-CoV-2 (30,7%).Tras la pandemia, los pacientes preferían continuar con la dispensación y autoadministración domiciliaria del biológico y tener consultas médicas presenciales. El grado de satisfacción fue 9,7 (escala 0 a 10). Conclusiones: Los pacientes prefieren autoadministrarse en su domicilio los fármacos biológicos para el AG con el apoyo de la dispensación domiciliaria de éstos, mostrando un alto grado de satisfacción por la comodidad que les aporta. Finalizada la pandemia, demandan que las visitas médicas sean presenciales pero desean continuar autoadministrándose el fármaco tras su dispensación domiciliaria por el Servicio de Farmacia. (AU)


Objective: To assess preferences and satisfaction of patients with severe asthma about the place of administration of subcutaneous biological drugs: hospital, health center and home, during the COVID-19 pandemic. Methods: Observational, descriptive and cross-sectional study that analyzed, from November 23 to 27, 2020, through a telephone survey, the preferences and degree of satisfaction with the administration of subcutaneous biological drugs in asthmatic patients treated in the Allergology consultation of a General Hospital. Results: A total of 33 patients responded to the survey, the mean age was 51.5 years, 57.6% were women. The patients that received subcutaneous biological drugs (omalizumab, mepolizumab and benralizumab) at home were 57,6%, at the Day Hospital and at the Health Center 21,2 %, in both cases. The reasons for the administration outside the hospital were comfort and to avoid the spread of the SAR-CoV-2 virus (30.7%). After the pandemic, patients prefer home deliveries, self-administration and face-to-face medical consultations. The degree of satisfaction with the treatment was very high. Conclusions: Patients prefer to self-administer biological drugs for GA at home with the support of their home dispensing, showing a high degree of satisfaction with the comfort it provides. Once the pandemic is over, they demand that medical visits be face-to-face but they want to continue self-administering the drug after it is dispensed at home by the Pharmacy Service. (AU)

ABSTRACT

El síndrome de Tolosa-Hunt es una entidad poco frecuente cuya etiopatogenia y mecanismos fi- siopatológicos son controversiales, se caracteri- za por cefalea asociada a parálisis de uno o más nervios craneales, diplopía, estrabismo y ptosis palpebral, ocasionados por el compromiso del seno cavernoso o la fisura orbitaria superior. Su diagnóstico es un reto y se aborda dentro de los diagnósticos diferenciales de las oftalmoplejías dolorosas. Con el objetivo de describir y actua- lizar el conocimiento sobre esta enfermedad se presenta el caso de una paciente de 14 años que acudió a consulta por cefalea intensa, dolor ocu- lar y afección de nervios craneales. Los hallazgos clínicos y la resonancia magnética confirmaron el diagnóstico del síndrome de Tolosa Hunt...(AU)

ABSTRACT

PURPOSE: To examine retinal and choroidal thicknesses in individuals with obstructive sleep apnea (OSA) and determine the impacts of continuous positive airway pressure (CPAP) treatment. METHODS: Prospective follow-up study conducted at a university hospital. 40 patients with OSA, 28 treated with CPAP, and 12 untreated, were enrolled immediately after diagnosis and graded according to the apnea hypopnea index (AHI) determined in an overnight polysomnography. Inclusion criteria were a new diagnosis of OSA and CPAP indicated. Participants underwent a full ophthalmologic examination including optical coherence tomography (OCT) at the peripapillary, macular, and choroidal levels and the same examination 3 months later. Outcome measures were peripapillary retinal nerve fiber layer (RNFL), total retinal (TRT), retinal ganglion cell layer (RGCL), inner plexiform layer (IPL), photoreceptor layer (PL), and choroidal thicknesses. RESULTS: At 3 months, RGCL thickness was reduced at the inner nasal macula segment in the no-CPAP group (P = 0.016). In + CPAP, increases were produced in RNFL thickness (5/6 segments) and TRT (7/ 9 segments), while choroidal thinning was observed temporally (P = 0.003). At baseline, positive correlation was detected between choroidal thickness and AHI (r = 0.352, P = 0.005) and between IPL thickness (7/9 segments) and AHI (r = 0.414, P < 0.001). CONCLUSIONS: Initial retinal and choroidal thickening was followed by RGCL thinning over 3 months. In patients receiving CPAP, we observed no thinning of any retinal layer and normalization of choroidal thickness.

ABSTRACT

The proton beam produced in the Nuclear Physics line of the tandem accelerator of the Centro Nacional de Aceleradores was used to generate a neutron field. In particular, 1.912 MeV protons were used to produce well-known epithermal neutrons through the p+7Li → n+7Be reaction. The aim of the work was to characterize this field while testing the performance of a Bonner sphere spectrometer in the epithermal range. Measurements were performed in four locations at different angle (0°, 30°, 60° and 90°) from beam incidence direction in order to study the angular dependence of the field. Both a parametric and numerical unfolding methods were tested to process the counts of the central detectors and obtain the energy distribution of the neutron fluence. In addition, a Monte Carlo simulation was carried out to complete the study and provide a guess spectrum for numerical unfolding. It was found that the fluence rate and mean energy decrease as the angle from beam direction increases. Total fluence was 2.75, 1.36, 0.366 and 0.216 cm-2 per charge collected in the target at 0°, 30°, 60° and 90°, respectively. Mean energy of the field ranges from 46 to 17 keV at 0° and 60°, respectively. In all cases, given that the irradiation room is so large, the contribution of thermal neutrons to the field is small. Regarding the unfolding, the total fluences estimated by all methods were in agreement within the uncertainties.

ABSTRACT

El propósito de este estudio fue realizar una caracterización psicológica de jugadores profesionales de fútbol y baloncesto de la primera división en Costa Rica. Se contó con la participación de 242 deportistas (140 futbolistas y 99 basquetbolistas). Se aplicó el Cuestionario de Características Psicológicas relacionadas con el Rendimiento (CPRD) y la escala de Clasificación del Rendimiento (PCQ). Los resultados encontrados muestran valores promedio que sugieren la necesidad de trabajar en el desarrollo de destrezas psicológicas. De manera particular en el manejo de mecanismos de afrontamiento del estrés en la competencia deportiva. Especialmente estresante se evidenció la evaluación que hacen del rendimiento. Muy relacionado con el tema de las destrezas psicológicas, se presenta la necesidad de trabajar más en el desarrollo de habilidades mentales que potencien el desarrollo del talento del deportista. Conclusión: se requiere una mayor inversión de tiempo en el trabajo psicológico deportivo, como factor determinante para que el deportista rinda al máximo (AU)


The aim of this study was to establish a psychological characterization in professionals Costa Rica first division soccer and basketball players. METHODOLOGY: two hundred and forty players were recluted for this study (140 soccer players and 99 basketball players. The 'Cuestionario de Características Psicológicas relacionadas con el Rendimiento' (CPRD) and the Performance Classification Questionnaire (PCQ) were applied. RESULTS: the results showed averages that suggest the need of work on the development of psychological skills, particularly in the management of stress coping mechanism during competition. These results revealed a particularly stressful situation for the athlete when other people evaluate them, especially when it comes from an important person for the athlete. Closely related to the issue of psychological skills comes the need to work on improve mental skills to enhance the athletes talent development. Conclusions: further investment of time is required in sport psychological work, such as determining the athlete squeeze the maximum factor (AU)


A finalidade deste estudo foi realizar uma caracterização psicológica de jogadores profissionais de futebol e basquete da primeira divisão na Costa Rica. Houve a participação de 242 esportistas (140 futebolistas e 99 basquetebolistas). Foi aplicado o Questionário de Características Psicológicas relacionadas com o Rendimento (CPRD) e a escala de Classificação de Rendimento (PCQ). Os resultados encontrados mostram valores em média que sugerem a necessidade de trabalhar no desenvolvimento de destrezas psicológicas. Especialmente no manejo de mecanismos de enfrentamento do estresse na competência esportiva. Especialmente estressante ficou evidenciada as avaliações que fazem do rendimento. Muito relacionado com o tema das destrezas psicológicas, vemos a necessidade de trabalhar mais no desenvolvimento de habilidades mentais que potenciem o desenvolvimento do talento do esportista. Conclusão: é necessário investir mais tempo no trabalho psicológico esportivo, como fator determinante para que o atleta possa dar um máximo rendimento (AU)
