ABSTRACT

OBJECTIVE: To assess the link between enuresis nocturna and the severity of behavioural and/or emotional problems in Dutch children and the course of these problems. SETTING: West-Mine Region in the Netherlands. SUBJECTS AND METHODS: Prospective cohort study involving 66 of the 80 bedwetting children from all 1652 children born in 1983 in this region. After 1 y, contact was still possible with 64 of the enuretics. We used the Dutch version of the Child Behaviour Checklist (CBCL) and a questionnaire about bedwetting. RESULTS: The mean T-score for Total Problems (CBCL score) in 1992 (M1; mean age 8.6) was 52.1, and 1 y later was 49.2 (M2). There was no significant difference in the CBCL scores for M1, M2 and a matching group from the Dutch CBCL norm population, either in the group who remained wet or in the group who became dry. There were no differences between the sexes. There was no link between the severity of behavioural and emotional problems and the frequency of bedwetting. However, more children with bedwetting than expected were in the clinical range. CONCLUSION: There was no difference in behavioural and/or emotional problems between the first and the second measurement and the matching group from the CBCL norm group. There were no differences in behavioural and/or emotional problems between primary and secondary bedwetters, nor were there any consequences related to the frequency of bedwetting.

ABSTRACT

Hypogonadotropic hypogonadism is a disorder of puberty characterized by absence of spontaneous sexual maturation. 8 male adolescents with this disorder, who were treated with pulsatile GnRH administration, were examined psychologically by means of standardized interviews. Problems were found in the development of independence (specifically relating to own body image and social functioning) and in identity development (particularly on personal characteristics).

ABSTRACT

Subject of study was the analysis of behavioral characteristics of 51 10-year-old ex-NICU patients at home and at school. Parental assessment of child behavior proved not to differ from the normative data. Teachers however reported more problematic behaviors, particularly with regard to the introverted behavior in boys. On the basis of these findings one can conclude that the prognosis for this patient group is better than expected. A plea is made for future research to focus on factors that will facilitate a more favorable behavior development.

ABSTRACT

Eight male adolescents with hypogonadotropic hypogonadism, who were being treated with pulsatile GnRH administration, were examined psychologically. We focused on the development of independence and on the manner in which they experienced their illness and treatment. The patient group was compared with a group of male adolescents of normal height and a group of male adolescents of short stature. We found that there were problems in the patient group in the development of independence, specifically relating to their own body image and social functioning. The retarded sexual development of these patients is probably of greater importance than the height growth-related problems, if any. The problems we found appeared to be caused primarily by the presence of the physical disorder and not by the stress of treatment.

ABSTRACT

A 'behavioral pediatric' approach applied to the treatment of children with chronic retentive encopresis is reported. The treatment program, based on behavioral psychological principles, in which the parents participate, is found to be highly effective in eliminating the problem of encopresis. However, it will be shown through casuistry that general psychological functioning in children can affect the course of the therapeutic process. Both 'somatic' attitudes and psychological obstacles need to be taken into account. Unbiased diagnostics and more differentiated therapeutic indications are necessary conditions for a favourable outcome of treatment.

ABSTRACT

Malignant lymphoepithelial lesion (MLEL) is a rare neoplasm of the salivary gland which has an exceptionally high incidence in the Eskimo. Eighty-four cases have been previously reported. Seven additional cases in six Eskimos and one Filipino patient are reported in this paper. The etiology, treatment and prognosis of these lesions is then discussed.

ABSTRACT

Sixteen children with indwelling tracheostomy tubes or Montgomery T-tubes were cared for at home under the supervision of the Respiratory Home Care Program. The infants and children ranged in age from five months to 15 years and required tracheostomy for various forms of pathology. There was one unexpected death at home, but it did not appear to be caused by tracheostomy tube problems. We believe that home tracheostomy care in the pediatric patient is safe and is a great benefit to the wellbeing of the patient.
