ABSTRACT

Diffuse gastric antral vascular ectasia (GAVE) is a new disease entity corresponding to peculiar acquired vascular malformations. In 1984 Jabbari et al. coined the term "Watermelon Stomach" for this lesion. We report here in a new case particular by its association with systemic sclerosis in a 66 year-old female. The diagnosis of GAVE was suggested upon endoscopic and angiographic findings. Atrophic fundic gastritis associated with achlorhydria and hypergastrinemia was also noted. Antrectomy was performed with success. Pathologic examination and morphometric analysis of the antrum specimen revealed the characteristic features of GAVE: 1) increase of the mean cross sectional area of mucosal vessel lumen and increase of the mean percentage of specimen area occupied by vessels; 2) presence of intravascular fibrin thrombi and 3) fibromuscular hyperplasia of the lamina propria.

ABSTRACT

Between 1975 and 1983, 46 patients with mycosis fungoides (MF) were treated with radiotherapy. The patients were classified according to the Stanford system (table II) and to the TNMB system (table III). The superficial lesions were treated with total skin electron-therapy (TSE) (7), and the visceral and lymph node lesions received additional whole-body photon therapy (WBP) (7). The results are summarized in table I. Initial complete regression was obtained in 21 of the 46 patients; 15 had partial regression. At the moment, 11 are alive without any apparent lesion. There were 9 complete remissions in each of the stage I and stage II groups. In stages III and IVa groups the course of the disease was improved by the two types of irradiation combined. In stage IVb patients the leukaemia was not improved. The survival curves (fig. 1 to 6) offer a simplified representation of the factors responsible for a favourable course.

ABSTRACT

A prospective survey of drug-induced diseases has been performed along a one year period in a department of dermatology. Among the 550 patients hospitalized during this period for a cutaneomucous event, a drug-induced disease is pointed out in 30 of them (5.6 p. cent). The use of algorithm allows the establishment of the cause-effect relationship; the relative value of literature data and the role of three main predisposing factors (age, allergic antecedents, polymedication) are insisted upon. Three categories of drugs are primarily implicated: cardiovascular, antiinflammatory and antiinfectious drugs. The most commonly observed events are erythrodermia and allergic phenomena. If favorable evolution is usual, one fatal adverse drug reaction is firmly established in our series. Such a survey would lead to preventive attitudes.
