ABSTRACT

Presented is the case of a nurse who had 4 occupational exposures to potentially infectious material between December 2020 and June 2022. In 2 of the cases, the source patient was unknown, so pharmacological HIV post-exposure prophylaxis was implemented (in 1 of these cases, the nurse developed weakness and increased dyspeptic symptoms, necessitating a change in the antiretroviral medications used). During the interview collection, the nurse reported that multiple exposures to potentially infectious material are common in her work environment, but most of these are not reported. This is supported by the results of several studies devoted to the problem of non-reporting of occupational exposures by health care workers. However, there is significant discrepancy in the results of these studies, which may be due to different methods. The authors of this article believe that after 10 years since the entry into force of the regulation of the Minister of Health standardizing procedures for dealing with injuries caused by sharp instruments used in the provision of health care services, a serious problem remains of non-reporting of cases by employees (resulting in a lack of post-exposure prophylaxis). The authors call for the introduction of a nationwide reporting system. There is also a need to increase the importance of prophylaxis of stabbings and to improve the quality of training of medical personnel in post-exposure prophylaxis procedures. ABSTRACT

A large number of monkeypox (MPOX) cases have been reported in Europe and North America in 2022, and a new outbreak of this disease was declared. We describe a case of a patient with probable monkeypox during the height of this epidemic in Poland. The patient's symptoms resolved within two weeks, but over the next two months, he developed community-acquired pneumonia requiring hospitalization and, subsequently, non-specific pleuritis. The simultaneous occurrence of such severe infections in a previously healthy young man is not typical and suggests a potential underlying cause. We believe the potential association of these diseases with probable monkeypox virus infection is very likely. Cases of monkeypox pneumonia, both viral and secondary bacterial, have already been reported in the literature. Cases of viral pleuritis in the course of MPOX in animals have also been described; however, to our knowledge, no similar cases have been described in humans to date. Our case indicates that it is important to monitor patients after MPOX in order to respond promptly to potentially life-threatening but, as of yet, not fully understood complications.

ABSTRACT

We conducted a non-systematic review of epidemiological studies on a potential link between exposure to outdoor artificial light at night (O-ALAN) and disease occurrence in humans published since 2009. In recent years, a number of presses have been published on this issue, but the conclusions have been mixed. We therefore decided to critically analyze the available epidemiological evidence of such a correlation. After a careful search, 51 studies were identified and included in the review. They addressed the potential link between O-ALAN exposure and the incidence of breast cancer, other cancers, sleep and circadian rhythm disorders, obesity and cardiovascular diseases, mental disorders, infectious diseases, and complications during pregnancy and childbirth. The vast majority of papers revealed the existence of such a link. However, the amount of epidemiological evidence supporting the correlation across groups of disorders varied widely. In addition, we found that all papers contained at least one of the following omissions: lack of the temporal and spatial resolution in light at night measurements, measuring only light intensity without considering its wavelength, and not accounting for many important confounding factors in their statistical analyses. Therefore, we believe that the link between O-ALAN exposure and the occurrence of the disorders in question suggested by the authors of the reviewed papers may be in some cases at least to some extent, a coincidence. Further epidemiological studies, free of significant omissions highlighted in this paper, are needed.
