ABSTRACT

Ureteral hernia is uncommon and usually misdiagnosed. From an anatomic point of view, we can distinguish between two uretero-inguinal hernias: intraperitoneal and extraperitoneal. Ureter inguinal hernias are nearly always indirect. This kind of hernia can include the ureter alone or, frequently, other abdominal sliding organs within the hernia sac (bladder, bowel tracts, etc.). Kidneys and urinary tracts present normal anatomic conformation, although renal ptosis may be found. As of July 2004, 139 cases of ureteral hernia had been described in the literature. Here we report a case of inguino-scrotal herniation of double district ureter and review the current literature to analyze the main clinical characteristics of this pathology and to establish pitfalls.

ABSTRACT

OBJECTIVE: To investigate whether the use of the endoscopic gastrointestinal anastomosis (GIA) stapler for hemostasis of the dorsal vein complex during radical retropubic prostatectomy saves operation time and blood loss together with therapeutic efficacy. MATERIALS AND METHODS: From April 1990 to December 1998, a total of 296 patients underwent radical retropubic prostatectomy for prostate cancer in clinical stages T1, T2 and T3. In 157 patients we evaluated the efficacy of the endoscopic GIA stapler in order to minimize blood loss from the dorsal vein complex and to reduce mean operative time. RESULTS: The mean total operative time decreased an average of 35 min and the mean blood loss fell from 850 to 400 cm(3) using the stapling technique. Although complications are seen with traditional methods, we noted a net increase in anastomotic strictures with this new technique. CONCLUSIONS: Even if this technical trick is effective in reducing blood loss, a higher incidence of anastomotic strictures has to be taken into account when using staplers close to the anastomotic site.

ABSTRACT

The authors report two cases of retropharyngeal abscess in breast-fed babies and highlight the salient points of the disease: 1) The disease is rare, severe and sometime atypical. 2) The clinical features are: the patient age (breast-fed babies), difficult in swallowing, drooling, dyspnea and malaise. 3) The diagnosis is confirmed by a forward movement of the pharynx and of the soft palate except when the abscess is located on the lower half of the retropharyngeal space. 4) The most important diagnostic procedure is the lateral radiography of the neck that shows a widening of the retropharyngeal prevertebral space. 5) If an adequate therapy is not undertaken, following complications are possible: opening of the abscess with passage of the pus into the bronchopulmonary space, obstructive dyspnea with oxygen lack, propagation of the abscess into the mediastinum and possible development of fistulas into lung, and pleura, and exceptionally into pericardium. 6) The therapy includes the use of drugs active toward the possible etiological agents: the most common are the S.B.E.A., anaerobes, staphylococcus and gram-negative. At stage of suppuration, surgical operation is required.

ABSTRACT

Tissue antibodies (Ab) were investigated in 85 patients affected with pulmonary malignancy of different histologic type and, as control, in 40 non-smoker healthy subjects and in 40 smoker subjects with chronic bronchitis. A significantly higher number of patients (64, 7%) had tissue Ab in comparison with both control groups (17% in non smoker group and 25% in smoker and chronic bronchitis group, respectively). Antinuclear antibodies were found in 29.4% of patients, antismooth muscle in 31.7%, anti-mitochondrial Ab in 4.7%, antityroid microsomes Ab in 4.7%, anti-gastric parietal cells Ab in 4.7%, anti-reticulin Ab in 11.7%. The patients at third stage of disease showed all antibodies as well as anti-nuclear more frequently that patients at first and second stage. Antibodies to cytoplasmic antigens were found in patients with anaplastic and squamous cancer, but not in patients with adenocarcinoma. Our observations suggest that immunologic abnormalities similar to those present in autoimmune disease, may be associated to pulmonary malignancy.
