ABSTRACT

Electron tubes continue to provide the highest speeds possible for recording dynamics of hot high-energy density plasmas. Standard streak camera drive electronics and CCD readout are not compatible with the radiation environment associated with high DT fusion yield inertial confinement fusion experiments >1013 14 MeV DT neutrons or >109 n cm-2 ns-1. We describe a hardened x-ray streak camera developed for the National Ignition Facility and present preliminary results from the first experiment on which it has participated, recording the time-resolved bremsstrahlung spectrum from the core of an inertial confinement fusion implosion at more than 40× the operational neutron yield limit of the previous National Ignition Facility x-ray streak cameras.

ABSTRACT

Intravascular lymphoma is an uncommon subtype of B-cell lymphoma with neoplastic cells limited to the lumen of small blood vessels. We report a case of a 52-year-old man who presented with constitutional symptoms and rapidly progressive dementia. He was found to have diffuse leptomeningeal and faint parenchymal enhancement on magnetic resonance imaging and was subsequently diagnosed with intravascular lymphoma following a brain biopsy. He responded remarkably well to systemic and intrathecal chemotherapy. The diagnosis and treatment of intravascular lymphoma have been guided by a few case reports and are largely based on expert opinion.

ABSTRACT

BACKGROUND: A cadaveric and clinical study was performed to assess whether a minimally invasive technique is a potentially safer alternative compared to a calcaneal osteotomy through an open lateral approach. METHODS: Cadaveric: A minimally invasive calcaneal osteotomy (MICO) was performed on nine cadaveric specimens. These were examined to identify any nerve injury and to quantify the displacement achieved with the osteotomy. Clinical: A retrospective review of a sequential case series of medializing MICO's performed by the senior author between October 2011 and November 2014 was undertaken. RESULTS: Cadaveric: Neurological structures remained uninjured in all specimens. The distance of the sural nerve and the main trunk of the tibial nerve to the superior limb of the oblique osteotomy was 6.2±4.8mm (range 0-14mm) and 7.3±3.6mm (range 4-12mm), respectively. The mean coronal shift achieved was 16.7±3.4mm (range 12-21mm). Clinical: The senior author carried out 35 medializing MICO procedures. Radiological and clinical union occurred in all 35 cases (100%). There were no neurovascular or wound complications. CONCLUSION: Minimally invasive calcaneal osteotomy offers a safe alternative to an open procedure with promising clinical results.

ABSTRACT

Concerns have been expressed that dust from the minerals processing facilities at Karumba Queensland Australia have resulted in elevated lead (Pb) concentrations in rain water tanks. The ores derived from the Century mine some 304 km from the port. High precision Pb isotopic measurements on environmental samples have been undertaken to evaluate the source of Pb in rainwaters and acid digests from roof wipes and gutter wipes. There does not appear to be any relationship between sample location and the processing facility but samples from the area subject to the prevailing winds show the highest contribution of Century Pb. All gutter wipes (82 to 1270 µg Pb/wipe) have contributions of Century ore ranging from 87% to 96%. The contribution of Century ore to five roof wipes (22 to 88 µg Pb/wipe) ranges from 89% to 97% and in the other two samples there is a mix of Century and Broken Hill Pb. Three of the seven rainwater have contributions of Century ore Pb ranging from 33% to 75%. Two of the other four rainwater samples have the highest water Pb concentrations of 88 and 100 µg/L and their isotopic data show Broken Hill Pb contributions ranging from 77% to 80%. The source of the Broken Hill Pb is probably from the galvanized roofing material and/or brass fittings in the rainwater tanks. The discrimination between various sources is only detectable using high precision (204)Pb-based isotopic ratios and not the now common inductively coupled plasma mass spectrometry (ICP-MS ) data presentations of the higher abundance isotopes (208)Pb, (207)Pb and (206)Pb. Isotopic results for the waters demonstrate that apportioning blame where there is an obvious point source may not always be the correct conclusion. Nevertheless the isotopic data for the gutter wipes indicates that there was widespread contamination from the processing facilities throughout the town.

ABSTRACT

OBJECTIVES: To investigate and compare the prevalence of pharyngeal reflux (PR) events in normal controls and patients with clinically diagnosed reflux laryngitis. METHOD: A systematic review of the literature was performed to identify all prospective studies on the results of 24-hour double-probe (pharyngeal and esophageal) pH monitoring in normal controls and in patients with symptoms and/or signs of reflux laryngitis. RESULTS: Eleven relevant studies on 192 normal controls and 13 studies on 512 patients with reflux laryngitis were identified. One or more PR events were detected in 51 normal controls (22.9%; 95% CI, 13.9% to 33.3%) and in 154 of 422 patients (38.3%; 95% CI, 25.4% to 52.1%). There is no significant difference in the prevalence of PR events between normal controls and patients with reflux laryngitis (P = 0.079). In addition, the prevalence of PR events in patients with reflux laryngitis is much lower than reported in previous reviews on this subject. CONCLUSION: This systematic review calculated that (1) only a minority of patients with clinically diagnosed reflux laryngitis will show PR events, and (2) there is no significant difference between the prevalence of PR events in patients with reflux laryngitis and healthy controls. At the moment, there is no reliable means to confirm reflux of gastric juice in patients with suspected reflux laryngitis. This diagnostic vacuum is fundamental and may pose important questions at the current concept of reflux of gastric juice as a common cause of laryngopharyngeal inflammation.

ABSTRACT

The ability of a genotype to change its phenotype was once considered rather a nuisance -- making it difficult to define a genotype. This led to the idea that there was a problem called 'instability'. But quite early it was recognized that stability was under genetic control, and was a character like other attributes of an individual. From this realization came the idea that there were two sides to the character of 'instability', and that the ability to change could be important. This ability was thus given the title of 'plasticity'. Once recognized, it became clear from surveys of different species and populations that plasticity can (i) be a complex character, and (ii) be selected to fit species to the particular demands of different environments. For plants, which cannot meet variations in environment like animals by behavioural responses, phenotypic plasticity can be very important. Plants should therefore be valuable tools for unravelling the mechanisms of plasticity whilst also demonstrating its contribution to fitness experimentally. We ought also to be able to demonstrate that appropriate genetic variability is available through which complex responses can be built up by selection. Genes must exist not only to determine character means, but also to determine character response, which adds interesting complexity to our ideas about evolution.
