ABSTRACT

OBJECTIVE: This study investigates body concept with respect to psychiatric diagnosis, age and gender of inpatients before and after treatment. METHODS/SAMPLE: We examined 153 inpatients with 131 complete datasets. The examination was done at the beginning and at the end of the treatment using the Frankfurt Body Concept Scale (FKKS, Deusinger 1998). The results were calculated by t-test for dependent samples, posthoc Scheffé-tests and univariate variance analysis. RESULTS: The diagnoses indicated a more positive body concept at the end of the treatment (patients with somatoform disorder had effect sizes between -0.2 and -0.4, patients with depressions between -0.4 and -0.7, and patients with eating disorders between -0.3 and -0.7). Gender and age had only little influence on the positive changes. DISCUSSION: The improvement of body concept through inpatient treatment represents a challenge especially for somatoform disorders. The duration of this improvement after inpatient treatment needs further investigation.

ABSTRACT

OBJECTIVE: This study investigates body image with respect to the age, gender, and psychiatric diagnosis (ICD-10) of psychosomatic outpatients based on apriori formulated hypotheses. METHODS/SAMPLE: 865 of the 1114 patients examined over the course of 10 months in a psychosomatic clinic agreed to participate in the study and filled in the Frankfurt Body Concept Scales (FKKS, Deusinger 1998). RESULTS: Patients with one or more psychiatric diagnoses showed significantly more impaired body images than did healthy controls examined with the FKKS. Especially patients with eating disorders stood out. Men generally had more positive body images than women. Higher age was related to some subscales, but not to the sumscore of the FKKS. DISCUSSION: We discuss the complex relationship between gender- and age-related differences, mental disorders and body image, as well as their therapeutic implications.

ABSTRACT

INTRODUCTION: The objective of this study was to investigate the influence of accidents, the physical and psychological consequences, the patient's predisposition as well as work-related cognitions on return to work (RTW) post accident. Despite the costs of time-off from work after accidental injuries, very few investigations have been carried out so far. METHOD: In a consecutive sample, 163 patients were investigated directly and 12 months after an accident. RESULTS: 32% of the patients had a poor occupational integration 12 months after an accident. As predictors for return to work were found type of prior work: laborer (OR = 4.34; 1.79-10.50 CI 95%), type of accident: recreational (OR = 0.27; 0.11-0.69 CI 95%) and subjective perception of the accident severity (OR = 0.98; 0.96-0.99 CI 95%). CONCLUSION: Laborers after a traffic or work accident, who estimate the accident as severe, are at greater risk of developing long-term disability. Future efforts should be conducted especially for this target group.

ABSTRACT

The German Lung Cancer Cooperative Group (GLCCG) is assessing the impact of chemoradiation in addition to chemotherapy in the neoadjuvant treatment of stage III NSCLC. After three cycles of cisplatin/etoposide patients receive either hyperfractionated radiotherapy (RT) with concurrent carboplatin/vindesine and then surgery (arm A) versus surgery and then conventional RT (arm B). Quality of life (QL) was assessed throughout therapy using the EORTC QLQ-C30 and EORTC QLQ-LC 13. Of 126 eligible patients, 54 completed treatment. For patients in both treatment arms physical functioning decreased, whereas dyspnoea, fatigue and pain increased from beginning to the end of treatment. For self-assessed QL no statistically significant effect was found in or between the two treatment arms. The combined modality approach with preoperative radio/chemotherapy proves to be feasible in treating locally advanced NSCLC patients without decreasing their subjective QL.
