ABSTRACT

Authors studied 109 patients with eating disorders, including 49 with anorexia nervosa (AN) and 60 with bulimia nervosa (BN), using psychopathological and experimental/psychological methods, psychometric scales and follow-up. Four variants (2 AN and 2 BN) of clinical presentations and dynamics of affective disorders were singled out. It has been shown that many features of their symptoms and responses of patients to the complex therapy (diet-, psycho- and pharmacotherapy) depend on the belonging of AN or BN to a group of borderline mental disorders or to endogenous diseases. Taking into account the revealed features of affective disorders, the authors have formulated recommendations for treatment tactics and prevention measures for these groups of patients.

ABSTRACT

The aim of the study was to reveal risk factors for development of alcoholic psychosis in patients with eating disorders. Ten inpatients with anorexia nervosa and bulimia nervosa abusing alcohol for 1-4 years have been examined. Delirium tremens was diagnosed in 5 patients in the first few days of staying in the hospital, other 5 patients were included in the control group. Factors precipitating delirium tremens were low body mass index, water-electrolytic imbalance, severe dehydration caused by vomiting and hypoglycemia. A careful choice of doses of psychotropic drugs taking into account concomitant somatic-endocrine disturbances, the degree of electrolytic imbalance and thorough alcoholic anamnesis is important at the earlier stage of treatment.

ABSTRACT

The data on 1500 patients with syndrome of anorexia and bulimia nervosa were summarised in terms of clinical-psychopathological, catamnestic and experimental-psychological investigation. This syndrome was considered as one disease which presented the variation of borderline mental pathology or the manifestation of schizophrenic process. Obsessive-phobic disorders were observed in majority of patients (95%). Such disorders were characterized by prevalence of obsessive phenomena with food content. It was noted that diagnosis and prognostic significance had either obsessive-phobic disturbances or obsessions with other content which occurred on the initial stage of disease, before development of syndrome of anorexia and bulimia and in period of pronounced cachexia. The therapeutic resistance of obsessive disturbances in patients with anorexia and bulimia were emphasized as well as the necessity of prolonged ambulatory treatment of such patients.
