ABSTRACT

INTRODUCTION: Venous thromboembolism (VTE) is a serious, frequent, and preventable medical complication in hospitalized patients. Although the efficacy of prophylaxis (pharmacological and/or mechanical) has been demonstrated, compliance with prophylaxis is poor at international and national levels. AIM: To determine the indication and use of pharmacological thromboprophylaxis in hospitalized patients in Uruguay. METHODS: An observational, descriptive, cross-sectional, multicentre study involving 31 nationwide healthcare facilities was conducted. Baseline characteristics associated with hospital admission, the percentage of the population with an indication for thromboprophylaxis, and the percentage of patients receiving pharmacological thromboprophylaxis were assessed. The VTE risk was determined using the Padua score for medical patients; the Caprini score for surgical patients; the Royal College of Obstetricians and Gynaecologists (RCOG) guidelines for pregnant-postpartum patients. RESULTS: 1925 patients were included, representing 26% of hospitalized patients in Uruguay. 71.9% of all patients were at risk of VTE. Of all patients at risk of VTE, 58.6% received pharmacological thromboprophylaxis. The reasons for not receiving thromboprophylaxis were prescribing omissions in 16.1% of cases, contraindication in 15.9% and 9.4% of patients were already anticoagulated for other reasons. Overall, just 68% of patients were "protected" against VTE. Recommendations of major thromboprophylaxis guidelines were followed in 70.1% of patients at risk. CONCLUSIONS: Despite the progress made in adherence to thromboprophylaxis indications, nonadherence remains a problem, affecting one in six patients at risk of VTE in Uruguay.

ABSTRACT

OBJECTIVE: To identify pressure areas using an impression technique and to evaluate whether adjustment of dentures prior to installation can decrease ulcer frequency, number of adjustments, and improve satisfaction with treatment. MATERIALS AND METHODS: Fifty complete denture users were selected and randomly divided into two groups. Pressure areas were identified in 25 subjects using the impression technique with fluid silicone, and the denture base in these areas was adjusted before installation. A total of 25 subjects received new complete dentures without adjustments before installation. The number of ulcers observed was registered at 1, 7, 15, and 30 days after installation. Patient satisfaction was collected before and 1 month after the installation. The number of necessary post-installation adjustments was considered. Comparisons between the groups with respect to the number of ulcers, adjustments, and patient satisfaction were performed using the Mann-Whitney test. RESULTS: Identification of pressure areas reduced the number of ulcers after installation of the new prostheses. These results were observed in the 1- (p = 0.004) and 7-day (p = 0.002) adjustments for the maxilla and in all the adjustments for the mandible. The technique improved patient satisfaction (p = 0.031) and reduced the number of post-installation adjustments (p ≤ 0.001). CONCLUSION: A reduction in the incidence of ulcers, number of adjustments and improvement in patient satisfaction could be observed with the use of the technique.

ABSTRACT

Cardiovascular diseases (CVD), mainly ischemic heart disease and stroke, is the main cause of death worldwide and each year more people die from CVD than from any other cause. These data call for a paradigm shift, where health promotion and cardiovascular prevention will acquire a central role in health policies. From this perspective, dedicating time during the consultation to promoting the acquisition of heart-healthy habits would be indicated in all individuals, regardless of cardiovascular risk classification, the role of the internist being fundamental. This position document from the International Forum of Internal Medicine (FIMI) presents the main indications regarding changes in lifestyle and acquisition of healthy habits to prevent CVD. The different sections will address topics including: nutrition, physical activity, sedentary lifestyle, obesity, smoking, alcohol consumption, sleep, stress, environmental problems related to CVD and specific conditions in women. A section is included about starting CVD promotion and prevention measures at an early age, childhood and adolescence, also mentioning epigenetic aspects related to CVD. Social determinants in CVD are also taken into account, since some of these aspects, such as low socioeconomic level, modify cardiovascular risk and should be taken into account.


Las enfermedades cardiovasculares (ECV), principalmente la cardiopatía isquémica y el accidente cerebrovascular (ACV), constituyen la principal causa de muerte a nivel mundial y cada año mueren más personas por ECV que por cualquier otra causa. Estos datos requieren la necesidad de un cambio de paradigma, en donde la promoción de la salud y la prevención cardiovascular adquieran un papel central en las políticas sanitarias. Desde esta perspectiva, dedicar tiempo durante la consulta en promocionar la adquisición de hábitos cardiosaludables estaría indicado en todos los individuos, independientemente de la clasificación de riesgo cardiovascular, siendo fundamental el rol del médico internista en su función de médico de cabecera. En este documento de posicionamiento del Foro Internacional de Medicina Interna (FIMI) se presentan algunas pautas para recomendar e indicar modificaciones en el estilo de vida y adquisición de hábitos saludables para prevenir la ECV, que tienen el objetivo de ser una herramienta practica para el médico internista. Las diferentes secciones abordaran temas que incluyen: nutrición, actividad física, sedentarismo, obesidad, hábito tabáquico, consumo de alcohol, sueño, estrés, problemas ambientales relacionados a la ECV y condiciones específicas en la mujer. Se incluyó un apartado acerca de comenzar las medidas de promoción y prevención de ECV en edades tempranas, infancia y adolescencia, mencionando además aspectos epigenéticos relacionados a la ECV. Se tienen en cuenta además los determinantes sociales en ECV, ya que algunos de estos aspectos, como el bajo nivel socioeconómico, modifican el riesgo cardiovascular y debieran ser tenidos en cuenta.

Introducción: La obesidad es una enfermedad epidémica multifactorial en constante aumento en los últimos años. Asocia el desarrollo de múltiples patologías con aumento de la morbimortalidad, entre ellas el síndrome de apneas e hipopneas obstructivas del sueño (SAHOS). Objetivo: Estudiar la prevalencia de SAHOS en una población de obesos pertenecientes a un Programa de Obesidad y Cirugía Bariátrica de un hospital público de Montevideo. Metodología: Estudio observacional descriptivo de cohorte transversal. Se incluyeron pacientes en valoración preoperatoria desde abril 2015 a marzo 2020. Se les realizó una polisomnografía. Se evaluó la prevalencia de SAHOS y la relación con otros factores de riesgo cardiovascular. El análisis estadístico se realizó con SPSS versión 25.0. Resultados: Se estudiaron 358 pacientes con predominio del sexo femenino (86,3%), edad media de 42,8 ± 10,7 años y un índice de masa corporal (IMC) promedio de 50,1 ± 11,4 kg/m2. Se evidenció una prevalencia de SAHOS de 69%: 43,3% leve; 23,9% moderada y 32,8% severo. El Índice apnea hipopnea (IAH) se correlacionó positivamente con IMC (p ≤ 0,001). Se demostró la asociación de SAHOS con hipertensión arterial (HTA), diabetes 2 (DM2), sexo masculino e hipertrigliceridemia. Conclusiones: El SAHOS es altamente prevalente en la obesidad. Este estudio sugiere la realización de un screening en todos los obesos, así como su estudio con polisomnografía o poligrafía respiratoria a aquellos que vayan a someterse a una cirugía bariátrica.


Introduction: Obesity is a multifactorial epidemic disease that has been constantly increasing in recent years. It associates the development of multiple pathologies with increased morbidity and mortality, including obstructive sleep apnea and hypopnea syndrome (OSAHS). Objective: To study the prevalence of OSAHS in a population of obese patients belonging to an Obesity and Bariatric Surgery Program of a public hospital in Montevideo. Methodology: Observational descriptive cross-sectional study. Patients in preoperative evaluation were included from April 2015 to March 2020. A polysomnography was performed. The prevalence of OSAHS and the relationship with other cardiovascular risk factors were evaluated. Statistical analysis was performed with SPSS version 25.0. Results: 358 patients were studied, predominantly female (86.3%), mean age of 42.8 ± 10.7 years and mean BMI of 50.1 ± 11.4 kg/m2. A prevalence of OSAHS of 69% was evidenced: 43.3% mild; 23.9% moderate and 32.8% severe. The Apnea Hypopnea Index (AHI) was positively correlated with the Body Mass Index (BMI) (p=<0.001). The association of OSAHS with arterial hypertension (AHT), diabetes 2 (DM2), male gender and hypertriglyceridemia was demonstrated. Conclusions: OSAHS is highly prevalent in obesity. This study suggests screening all obese people, as well as polysomnography or respiratory polygraphy for those who are going to undergo bariatric surgery.

Associa o desenvolvimento de múltiplas patologias ao aumento da morbimortalidade, incluindo a síndrome da apneia e hipopneia obstrutiva do sono (SAHOS). Objetivo: Estudar a prevalência de SAHOS em uma população de pacientes obesos pertencentes a um Programa de Obesidade e Cirurgia Bariátrica de um hospital público de Montevidéu. Metodologia: Estudo observacional descritivo transversal. Os pacientes em avaliação pré-operatória foram incluídos de abril de 2015 a março de 2020. Foi realizada polissonografia. A prevalência de SAHOS e a relação com outros fatores de risco cardiovascular foram avaliadas. A análise estatística foi realizada com SPSS versão 25.0. Resultados: Foram estudados 358 pacientes, predominantemente do sexo feminino (86,3%), idade média de 42,8 ± 10,7 anos e IMC médio de 50,1 ± 11,4 kg/m2. Evidenciou-se prevalência de SAHOS de 69%: 43,3% leve; 23,9% moderado e 32,8% grave. O Índice de Apnéia e Hipopnéia (IAH) correlacionou-se positivamente com o Índice de Massa Corporal (IMC) (p=<0,001). Foi demonstrada a associação da SAHOS com hipertensão arterial (HA), diabetes 2 (DM2), sexo masculino e hipertrigliceridemia. Conclusões: A SAHOS é altamente prevalente na obesidade. Este estudo sugere a triagem de todos os obesos, bem como a polissonografia ou poligrafia respiratória para aqueles que serão submetidos à cirurgia bariátrica.

Resumen: Introducción: La obesidad mórbida es un factor de riesgo para litiasis renal. La cirugía bariátrica, logra buenos resultados metabólicos, pudiendo generar un aumento del riesgo de litiasis renal. Objetivo. Estudiar los factores de riesgo litogénicos en pacientes obesos en el pre operatorio de cirugía bariátrica. Metodología: Estudio descriptivo, transversal. Se incluyeron pacientes del Programa de Obesidad y Cirugía Bariátrica, de febrero de 2019 a marzo de 2020. Resultados: Se analizaron 68 pacientes, 83,3% mujeres, mediana de edad 46 (37-52) años. La mediana del IMC fue de 46 (43-53) kg/m² con un rango de 35 a 70 kg/m². De los participantes 29 (43%) eran súper-obesos (IMC>50kg/m2), 31 (48%) presentaban síndrome metabólico, 19 (28,7%) eran diabéticos, 39 (59%) eran hipertensos. La mediana del clearence de creatinina medido fue de 136,5 (100,5-162,5) ml/min, 41 (60%) pacientes fue mayor a 120 ml/min. En 16 (23%) pacientes se constató el antecedente de manifestación clínica - ecográfica de litiasis. Todos los pacientes estaban asintomáticos al momento del estudio. Encontramos al menos 1 factor litogénico en 97% pacientes y 2 en el 71%. El 60,6% tenían hiperparatiroidismo, el 63% con hipovitaminosis D ( 100 mmol/24hs, 60,3% hiperuricosuria, 48,5% tenían hipocitraturia, 42,6% hiperoxaluria, 25% hipercalciuria y 79,4% con hiperamoniuria. No se evidencio diferencias en las variables litogénicas, entre pacientes con antecedentes de litiasis y sin antecedentes, en pacientes obesos y superobesos, ni al comparar pacientes diabéticos y con síndrome metabólico vs pacientes sin estas alteraciones. Discusión y conclusiones: En nuestro estudio la alta prevalencia de factores de riesgo litogénicos, apoya el vínculo entre obesidad y la patología litiásica renal. Es aconsejable la evaluación clínica específica y la realización de un estudio litogénico previo a la cirugía bariátrica, incidiendo su resultado en la elección de la técnica quirúrgica.


Abstract: Introduction: Morbid obesity is a risk factor for kidney stones. Bariatric surgery achieves good metabolic results, and can generate an increased risk of kidney stones. Target. To study the lithogenic risk factors in obese patients in the preoperative period of bariatric surgery. Methodology: Descriptive, cross-sectional study. Patients from the Obesity and Bariatric Surgery Program were included, from February 2019 to March 2020. Results: 68 patients were analyzed, 83.3% women, median age 46 (37-52) years. The median BMI was 46 (43-53) kg/m² with a range of 35 to 70 kg/m². Of the participants, 29 (43%) were super-obese (BMI>50kg/m2), 31 (48%) had metabolic syndrome, 19 (28.7%) were diabetic, and 39 (59%) were hypertensive. The median creatinine clearance measured was 136.5 (100.5-162.5) ml/min, 41 (60%) patients were greater than 120 ml/min. In 16 (23%) patients, a history of clinical-ultrasound manifestation of lithiasis was confirmed. All patients were asymptomatic at the time of the study. We found at least 1 lithogenic factor in 97% patients and 2 in 71%. 60.6% had hyperparathyroidism, 63% with hypovitaminosis D (100 mmol/24h, 60.3% had hyperuricosuria, 48.5% had hypocitraturia, 42.6% hyperoxaluria, 25% hypercalciuria and 79.4% with hyperammoniuria. No differences were found in the lithogenic variables, between patients with a history of lithiasis and without, in obese and super obese patients, or when comparing diabetic patients and patients with metabolic syndrome vs patients without these alterations. Discussion and Conclusions: In our study, the high prevalence of lithogenic risk factors supports the link between obesity and kidney stone disease. It is advisable to carry out a specific clinical evaluation and a lithogenic study prior to bariatric surgery, with its result affecting the choice of surgical technique.

Estudar os fatores de risco litogênicos em pacientes obesos no pré-operatório de cirurgia bariátrica. Metodologia: Estudo descritivo, transversal. Foram incluídos pacientes do Programa de Obesidade e Cirurgia Bariátrica, no período de fevereiro de 2019 a março de 2020. Resultados: Foram analisados ​​68 pacientes, 83,3% mulheres, idade mediana de 46 (37-52) anos. A mediana do IMC foi de 46 (43-53) kg/m² com variação de 35 a 70 kg/m². Dos participantes, 29 (43%) eram superobesos (IMC>50kg/m2), 31 (48%) tinham síndrome metabólica, 19 (28,7%) eram diabéticos e 39 (59%) eram hipertensos. Não foram encontradas diferenças nas variáveis litogênicas, entre pacientes com e sem história de litíase, em pacientes obesos e superobesos, ou ao comparar pacientes diabéticos e pacientes com síndrome metabólica versus pacientes sem essas alterações. Discussão e Conclusões: Em nosso estudo, a alta prevalência de fatores de risco litogênicos suporta a ligação entre obesidade e patologia de cálculos renais. Aconselha-se a realização de avaliação clínica específica e estudo litogênico prévio à cirurgia bariátrica, cujo resultado interfere na escolha da técnica cirúrgica.

ABSTRACT

Resumen: La hipopotasemia se define como la concentración de potasio sérico menor a 3,5 mEq/l. La hipopotasemia menor a 3 mEq/l puede cursar con sintomatología de la esfera cardiovascular y neurológica. Se presenta un caso de miopatía secundaria a hipopotasemia por consumo excesivo de bebidas cola azucaradas. Si bien esta etiología es poco frecuente, está descrita en la literatura, y dado el alto consumo de estas bebidas, es importante tenerlo presente frente a pacientes con esta sintomatología.


Abstract: Hypokalemia is defined as serum potassium concentration less than 3.5 mEq/l. Hypokalaemia less than 3 mEq/l can present with cardiovascular and neurological symptoms. A case of myopathy secondary to hypokalaemia due to excessive consumption of sugary cola is presented. Although this etiology is rare, it is described in the literature, and given the high consumption of these beverages, it is important to keep it in mind in patients with these symptoms.


Resumo: A hipocalemia é definida como concentração sérica de potássio inferior a 3,5 mEq/l. Hipocalemia inferior a 3 mEq/l pode apresentar sintomas cardiovasculares e neurológicos. Apresenta-se um caso de miopatia secundária à hipocalemia devido ao consumo excessivo de cola açucaradas. Embora essa etiologia seja rara, é descrita na literatura e, devido ao alto consumo dessas bebidas, é importante tê-la em mente em pacientes com esses sintomas.

Cancer is associated with immunodeficiency, while allergies result from immune system hyperactivity mediated by cytokines and immunoglobulins. The purpose of this study was to determine the relationship between immune environment of specific cancers and allergies, emphasizing cytokines related to Th1 and Th2 responses associated with IgE. 80 adults were distributed into two groups: control (n = 20) and cancer (n = 60), distributed in three subgroups (n = 20), head and neck, stomach, and prostate cancers. This study compared Th1 (IL-2) and Th2 (IL-4) parameters, anti-inflammatory, pro-inflammatory, or regulatory profile regarding both IgE levels and reported allergies, by means of clinical manifestations and IgE, IL-1ß, IL-2, IL-4, IL-17, and TGF-ß serum concentration. Clinically allergies were observed in 50% of the control group and in 20% of the cancer group (p = 0.009). IL-2 cytokine and TGF-ß concentrations were higher in the patients with cancer as compared to the control (p < 0.005). However, there were IL-4, IL-17, and IL-1ß decreases in the patients with cancer (p < 0.05). No correlation was observed between the cytokines studied and IgE and clinically proven allergies in both investigated groups. There was an inverse association between cancer and clinical allergy manifestations. In head and neck, stomach, and prostate cancers, an immunosuppressive serum tumor environment was predominant. There was no difference in cytokines related to Th1 and Th2 parameters in relation to IgE. No correlation was found between clinically proved allergies and immunity markers related to the same allergens.

OBJECTIVE: This review is to compare patient-satisfaction with ball-type overdenture attachment systems with others attachment systems. MATERIAL AND METHODS: This study was registered in PROSPERO (CRD42018097234) and adheres the PRISMA guidelines. Electronic searches on PubMed/MEDLINE, Scopus, Cochrane, and Web of Science databases for published articles were performed before October 2020. The PICO question was: "Do patients with a ball-type overdenture retention system have greater satisfaction, when compared to other attachment systems?" The evaluation of risk of bias was performed using the Cochrane risk of bias tool. RESULTS: After searching the databases, seven articles were selected out of a total of 2583. A total of 312 implants were placed in 139 patients, with a mean age of 65.9 years. The risk of bias in the included studies varied according to the different domains in a risk of uncertain bias or low bias risk. No difference was found between the ball attachment systems and the others systems, with respect to patient-satisfaction. The meta-analysis revealed no statistically significant difference between the ball systems and another systems (P = 0.11; MD: 10.90; 95% CI: -2.55 to 24.35). CONCLUSIONS: Regarding patient general satisfaction, it was not possible to determine differences between the ball system and another types of attachment system for overdenture. The ball-type system was statistically superior only to the magnet system.

BACKGROUND: Bariatric surgery can have several oral repercussions, including tooth erosion due to decreased salivary flow associated with the action of acidic pH and behavioral modifications in the diet that lead to the dissolution of mineralized dental tissues. OBJECTIVES: This systematic review aimed to evaluate whether bariatric surgery presented a greater risk of dental erosion. SETTING: Dentistry School, Pernambuco University, Camaragibe, Pernambuco, Brazil. METHODS: This review was based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyzes (PRISMA) and registered at the International Prospective Registry of Systematic Reviews (CRD42019124960). A search was performed in the PubMed/MEDLINE, Web of Science, and Cochrane Library databases using the following descriptors: "dental erosion OR tooth wear OR oral health OR dental wear OR tooth erosion OR salivary flow AND bariatric surgery OR gastrectomy OR obesity surgery". RESULTS: The review included 553 articles (after exclusion of duplicates) submitted for title and abstract reading, of which 24 were selected for full text analysis. Five articles fulfilling the eligibility criteria were included in the qualitative and quantitative analysis review. Because of high heterogeneity of the studies, meta-analysis could not be performed. CONCLUSIONS: Based on the review, we concluded that patients undergoing bariatric surgery had a higher incidence of dental erosion. All studies presented a high degree of dental erosion in patients submitted to bariatric surgery (P < .05).

ABSTRACT

The objective of this systematic review is to evaluate the effect of the incorporation of zirconia (ZrO2) particles on the mechanical properties of PMMA (polymethyl methacrylate), and to establish which characteristics of this material yield the best results aiming their biomedical applicability. This study was carried out in accordance with the Items of Preferred Reports for Systematic Reviews and Meta-Analyzes (PRISMA) and is registered in PROSPERO under registration number: CRD42018095801. The searches were carried out in the PubMed, MEDLINE, Scopus and Cochrane Library databases for articles published up until April 2018. After the different stages of the article selection process, eight articles were selected for qualitative and quantitative analysis. All were in vitro studies, totaling 536 evaluated samples. The concentrations of zirconia ranged from 0.5% to 20% and the particle sizes were between 15 nm and 10 µm. The incorporation of zirconia particles did not increase the flexural strength of PMMA in only one study. While the concentration of zirconia influences PMMA, the type of acrylic resin, size, and silanization of zirconia particles did not influence the results. Thus, the addition of zirconia particles showed a positive effect on PMMA enhancing their use in the medical and dental field, especially when certain anatomical areas requires higher strength of the materials, providing longevity for the rehabilitation.

Resumen: Introducción: un porcentaje de las infecciones del sistema nervioso central permanece sin diagnóstico etiológico. Las técnicas de amplificación de ácidos nucleicos mediante reacción en cadena de la polimerasa en tiempo real pueden disminuir este porcentaje. Objetivo: describir la etiología de las neuroinfecciones y valorar la utilidad de las técnicas de biología molecular en el diagnóstico y su impacto en el tratamiento antimicrobiano. Metodología: estudio observacional, descriptivo, retrospectivo a partir de registros clínicos. Se incluyeron mayores de 18 años asistidos en un hospital público de Montevideo durante un período de 32 meses, a quienes que se les realizaron técnicas de biología molecular en líquido cefalorraquídeo por sospecha clínica de neuroinfección. Resultados: se incluyeron 109 pacientes. En pacientes sin infección por VIH ni antecedentes neuroquirúrgicos (67%), se identificó microorganismo responsable en 16 casos, 8 bacterias y 9 virus. Todos identificados por técnicas de biología molecular modificando el tratamiento antimicrobiano empírico en 25 casos (34,2%). En portadores de VIH (25,7%), se detectaron microorganismos en 14 pacientes (50%). Seis virus, 5 bacterias y 7 hongos (Cryptococcus neoformans). El estudio por técnicas de biología molecular determinó el diagnóstico de 17 microorganismos y modificó el plan antimicrobiano inicial en 12 casos (42,9%). En pacientes con antecedente de neurocirugía reciente (7,3%), se aislaron seis microorganismos, tres de ellos exclusivamente mediante cultivo. Se modificó el tratamiento en tres casos (37,5%). Conclusiones: las técnicas de biología molecular deben considerarse complementarias. El impacto que generan en el diagnóstico y tratamiento justifica el uso de estas técnicas a pesar de su mayor costo.


Summary: Introduction: a certain percentage of infections of the central nervous system have no etiological diagnosis. Nucleic acids amplification techniques by means of a polymerase chain reaction in real time may reduce this percentage. Objective: to describe etiology of neuroinfectious diseases and assess the usefulness of molecular biology techniques in their diagnosis, as well as its impact on antimicrobial treatment. Method: observational, descriptive, retrospective study based on clinical records which included patients older than 18 years old, who had been assisted in a public hospital in Montevideo for over 32 months and had undergone molecular biology techniques with cerebrospinal fluid (CSF) given the clinical suspicion of neuroinfection. Results: 109 patients were included in the study. Among non-HIV infected patients who had not undergone neurosurgeries the responsible microorganism was identified in 16 cases (8 bacteria and 9 virus). They were all identified by molecular biology techniques by modifying the empiric antimicrobial therapy in 25 cases (34.2%). In carriers of HIV (25.7%), microorganisms were identified in 14 patients (50%). Six virus, 5 bacteria and 7 fungi (Cryptococcus neoformans). Molecular biology techniques defined the diagnosis of 17 microorganisms and modified the initial antimicrobial plan in 12 cases (42.9%). In patients with a history of recent neurosurgery (7.3%), 6 microorganisms were isolated, 3 of them exclusively through cultures. Treatment was modified in 3 cases (37.5%). Conclusions: molecular biology techniques need to be regarded as a complement. The impact that have in diagnosis and therapy justify their use despite its higher cost.

ABSTRACT

The aim of the present study was to perform a systematic review and meta-analysis to assess the influence of bariatric surgery on the clinical periodontal conditions in patients with obesity. This review was based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses and registered at the International Prospective Registry of Systematic Reviews (CRD42018099313). A search was conducted by 2 investigators in the PubMed/MEDLINE, Web of Science, and Cochrane Library databases for relevant articles published up to May 2018. The inclusion criteria were randomized controlled trials, prospective and retrospective studies, observational studies, longitudinal studies, and cohort studies with at least 3 months of follow-up. No language restrictions were imposed. The exclusion criteria were studies that did not evaluate or report the periodontal measurements, cross-sectional studies (without follow-up after surgery), studies that performed periodontal treatment, and those with insufficient periodontal data. The meta-analysis was based on the Mantel-Haenszel method and inverse variance. The quantitative analysis revealed no statistically significant differences with regard to bleeding on probing (P = .9; mean deviation: -.70; confidence interval = -11.43 to 10.04) or probing pocket depth (P = .41; mean deviation: -.46; confidence interval = -1.55 to .63) before and after intervention. Clinical attachment loss showed a statistically significant difference (P = .0002; mean deviation: .18; confidence interval = .07-.30). It can be concluded that bariatric surgery does not influence bleeding on probing or probing pocket depth, but leads to a worsening of clinical attachment loss.

Introdução: Vários estudos verificaram que pacientes com câncer apresentam menor incidência de alergia, enquanto outros não encontraram tal associação. Esses achados conflitantes decorrem de diferentes métodos utilizados e conceitos para definir alergia. Objetivos: Verificar se existe relação entre câncer e alergia. Métodos: Este estudo caso-controle incluiu pacientes maiores de 18 anos com diagnóstico de câncer, comparados com indivíduos sadios, pareados por sexo e idade. Realizou-se anamnese, testes percutâneos para alergia e dosagem sérica de eosinófilos. Ambos os grupos foram pareados quanto às variáveis de interesse e ajustaram-se eventuais desvios por análise multivariada. Resultados: Relato de alergia ocorreu em 30% dos pacientes no grupo com câncer e em 53% no grupo sem câncer (p = 0,001), já para os testes cutâneos ocorreu maior positividade no grupo com câncer (69%) do que no sem câncer (51%), (p = 0,01). Não houve diferença entre os valores dos eosinófilos. Conclusão: Pessoas com câncer apresentaram menor relato de alergia. Por outro lado, em indivíduos com câncer houve testes cutâneos positivos para alergia sem correspondência clínica. Portanto, em cancerosos, o teste cutâneo positivo para alergia não corresponde à presença de alergia clínica. (AU)


Introduction: Many studies have found patients with cancer present less allergy, while others did not found this association. The conflicting findings from prior studies are due to differences in the research methods, and in definitions of allergy. Objective: To identify a relation between cancer and allergy. Methods: This case-control study included patients older than 18 years of age, who presented a diagnosis of cancer, as compared to healthy individuals, paired by sex and ages. Anamnesis, allergy percutaneous test and a blood sample was collected to evaluate the eosinophils. Both groups were paired according to interest variables, and eventual deviations were adjusted by means of multivariate analysis. Results: Reports of allergies occurred in 30% of the patients with cancer, as compared to 53% in the non-cancer group (p = 0.001), whereas in the cutaneous tests, a greater positivity occurred in the cancer group (69%) than in the non-cancer group (51%), p = 0.01. No difference in the eosinophil values was observed within these groups. Conclusion: Patients with cancer present lower level of allergies. By contrast, in these cases, positive cutaneous tests detected allergies with no correspondence to clinical findings. Therefore, in presence of cancer, the positive cutaneous test for allergies is not related with clinical manifestation of allergy. (AU)

Introdução: a prótese dentária total é um órgão artificial que deve promover a saúde e a qualidade de vida. O modo de uso, conservação e higienização é parte fundamental para o sucesso e longevidade da mesma. Uma ferramenta que parece ser eficaz na detecção e disseminação desse tipo de informação é a extensão universitária. Objetivo: analisar o nível de conhecimento sobre o uso, conservação e higienização de próteses totais em uma população de baixa renda, através de estudo epidemiológico transversal. Métodos: a amostra de conveniência foi composta por 37 usuários de próteses totais atendidos em uma ação do programa de extensão universitária Resgatando Sorrisos da Universidade de Pernambuco, que promove ações terapêuticas e informativas sobre as áreas de prótese dentária, dentística, patologia e pacientes com necessidades especiais. Após assinatura do termo de consentimento livre e esclarecido (TCLE), foram coletadas informações sociodemográficas e sobre hábitos de uso e higiene das próteses, através de um questionário aplicado por pesquisadores em ambiente reservado. Os dados foram interpretados por meio de análise descritiva e analítica entre as variáveis com nível de significância considerado, p < 0,05. Resultados e Discussão: dos pacientes, 70,27% dormem com as próteses; sobre frequência de higienização, 43,2% relatam fazer pelo menos 3 vezes ao dia, sendo o uso da escova e creme dental o método mais utilizado (83,78% - 31 entrevistados); características como bases e dentes desgastados, presença de biofilme/tártaro foram encontradas em mais 50% das próteses totais. Não foi observada nenhuma correlação estatisticamente significativa entre as variáveis (p>0,05). Conclusão: o nível de conhecimento sobre o uso, conservação e higienização de próteses totais da população estudada precisa ser melhorado, uma vez parte da população ainda apresenta hábitos de uso e higiene de próteses totais inadequados.(AU)


Introduction: the dental total prosthesis is an artificial organ that should promote health and quality of life. The way of use, conservation and hygiene is fundamental for the success and longevity of it. One tool that seems to be effective in detecting and disseminating this type of information is activities of University extension. Objectives: to analyze the level of knowledge about the use of complete sets of dentures and their conservation in a low-income population through a cross-sectional epidemiological study. Methods: This research had a convenience sample of 37 users of complete sets of dentures. Datawas collected using a questionnaire applied by two trained examiners. The data were interpreted via descriptive and statistical analysis (tests of Kruskal-wallis e Kolmogorov ­ Smirnov/ p =0,05). Results: The average income of the sample was R$ 668.00. The average time of denture use was 7.3 years; 70.27% of the patients sleep with their dentures, and 83.78% of them sanitize the dentures with a toothbrush and toothpaste. Conclusion: Regarding the variables association, we noticed tendencies yet to be explored in the habits of the complete denture wearer. Moreover, we conclude that the population's level of knowledge on the subject needs to be improved. Thereby, once these conditions are detected and transmitted to government officials, the extension project through the research tool will be able to assist government's course of action regarding oral health.(AU)

ABSTRACT

Este estudo teve como objetivo analisar o impacto do projeto de extensão universitária, Resgatando Sorrisos, na formação de estudantes do curso de Odontologia da FOP - UPE, nos anos de 2013 e 2014. Foram utilizados 02 questionários em momentos distintos, tendo como amostra 71 alunos. Foi utilizado o teste de hipótese estatística Wilcoxon para comparar as respostas de antes e depois da ação. Diante da elevada frequência de respostas, como "muitíssimo" e "bastante", observamos um alto grau de aceitação e entendimento da importância do projeto na formação do aluno, assim como no que se refere à capacidade de mudança que o projeto pode causar na vida dos pacientes e sua possível aplicabilidade no sistema público de saúde, dentre outras questões. Contudo, observa-se no ano de 2014 em quesito referente ao grau de expectativa inicial e satisfação final do projeto, uma diferença estatística entre a ida e a volta, p=o,o1. Conclui-se que os alunos apresentam um entendimento positivo em relação à ação extensionista, considerando-a importante para sua formação profissional e pessoal... (AU)


The aim of this study was to analyze the impact of the University Extension Project, ResgatandoSorrisos, in the graduation process of students of Dentistry course FOP - UPE in the years of 2013 and 2014. There were 02 questionnaires used, at different times, with a sample size 71 students. Wilcoxon statistical hypothesis test was used to compare responses before and after the action. Such high frequency responses of "vital" and "enough", a high degree of acceptance and understanding of the importance of the project in the formation of the student was observed, as well as the capacity of a change that the project may cause in the lives of patients and its possible applicability in the public health system, among other issues. However, in 2014, there was a statistical difference between the outward and return (p = o, o1) in a question referring to initial degree of expectation and ultimate satisfaction of the project. It was concluded that students have a positive understanding about the extension action, and they consider it important for their professional and personal training... (AU)

ABSTRACT

Rectal leiomyosarcomas are rare tumors originated from smooth muscle cells. Differential diagnosis includes gastrointestinal stromal tumors (GIST), leiomyomas or schwannomas, and the differentiation of these tumors is usually made through immunohistochemistry. Due to its rarity, the standard treatment has not been defined. The purpose of this study was to present the follow-up of a patient with leiomyosarcoma of medium rectum submitted to exclusive operative treatment. The tumor size was 6 cm and it had a high mitotic index. The patient remains with good urinary function and good sphincter function, and free of the disease after a three-year follow-up. (AU)


Os leiomiossarcomas retais são tumores raros e originários de células de músculo liso. O diagnóstico diferencial inclui tumores estromais do trato gastrointestinal (GIST), leiomiomas ou schwannomas, e a diferenciação desses tumores normalmente é feita por imunohistoquímica. Devido a sua raridade, o tratamento-padrão ainda não está bem definido. O objetivo deste relato foi mostrar o seguimento de um paciente com leiomiossarcoma de reto médio submetido a tratamento operatório exclusivo. O paciente se manteve com bom controle urinário e boa função evacuatória. O tumor tinha alto índice mitótico e tamanho de 6 cm, mas não há sinais de recidiva após três anos da operação. (AU)
