ABSTRACT

The objective of the study was to investigate the screening utility of a questionnaire for cerebral visual impairment (CVI) by correlating the questionnaire with diagnostic tools such as the L94, the Test of Visual Perceptual Skills - Revised and the Visual Perception subtask of the Beery test of VisuoMotor Integration.The questionnaire consisted of 46 items, exploring different characteristics of CVI. We consecutively recruited 91 children. Parents filled out the questionnaire after which all children were seen for a diagnostic evaluation of CVI.There were 58 boys. Subjects' mean age was 6.10 years. A median of 12 items was ticked in the 45 children with CVI and 7 in the children without impairment. The domain 'visual attitude' scored positive most frequently. A logistic regression model using individual items, yielded Receiver Operating Curves for the questionnaire with good areas under the curve of 0.81 against the L94, 0.78 against the TVPS-R and 0.84 against the VP subtask. The sum score of the 6 domains was found to be an easy-obtainable score with a good sensitivity and specificity profile.This CVI questionnaire is a viable tool that has the potential of being implemented as part of a routine screening procedure for CVI.

ABSTRACT

PURPOSE: Two clinical cases of internal herniation through the foramen of Winslow are presented. The first case concerns herniation of the cecum and ascending colon. In the second patient, the ileum was herniated through the foramen epiploicum. METHODS: Preoperative assessment of clinical and radiologic findings suggested diagnosis. RESULTS: Final diagnosis was only made at laparotomy. Reduction of hernia could be performed by pulse traction, and no signs of intestinal necrosis were seen. CONCLUSION: Herniation through the foramen of Winslow should be taken into account when making a differential diagnosis for upper gastrointestinal tract obstruction.

ABSTRACT

From 1985 till 1990 twenty-three children aged seven weeks to six years presented with a retrolental white mass. Six children had retinoblastoma, six Coats' disease, five retinopathy of prematurity, four persistent hyperplastic primary vitreous, one cysticercosis and one retinal detachment. Retinoblastoma and Coats' disease was only seen in children over eight months, whereas persistent hyperplastic primary vitreous and retinopathy of prematurity all presented before the age of six months. Differentiation between Coats' disease and retinoblastoma is not always easy. Most helpful for the diagnosis is to perform funduscopy under general anaesthesia with special attention to the vasculature. Ultrasounds are helpful, but can give confusing results. Cytological analysis of subretinal fluid is useful in Coats' disease.

ABSTRACT

Optic neuritis is a clinical entity with specific characteristics. Many disease can be associated with the symptomatology. We retrospectively selected the files of 16 persons out of 60 optic neuritis patients. Selection was based on the detection of a non-common etiology. As underlying etiology for "optic neuritis" we found e.g.: histiocytosis, intracerebral aneurysm, spongioblastoma.
