ABSTRACT

The elastic and retractile nature of young skin allows glandular reductions to be performed without skin excision for the treatment of mammary hypertrophy in very young girls. These reductions in volume prevent secondary ptosis and stretch marks and are performed via an isolated inframammary incision. The devascularisation induced blocks glandular growth and no recurrence of hypertrophy has been observed in our series of 34 cases with a mean follow-up of 16 months.

ABSTRACT

Myeloma is a malignant hematopathy with multiple lesions, mainly in bones. The authors report a case in which the myeloma was present in the mandibuloparotid region only. The atypical nature and rarely observed location (mainly parotid) of the lesion raised diagnostic, prognostic, and therapeutic problems that were resolved by close cooperation with a team of hematologists. Therapy was by surgery only, in this particular case, because of the early stage of the lesion, but regular close surveillance is essential.

ABSTRACT

The authors, after studying the various types of microvascular sutures performed by participants at the Microsurgical Course (Dr Ph. Ploncard) from the Brussel University during a chosen period of over one year, draw several conclusions concerning the results in order to explain the main failures that may be found--aneurysms, stenosis, thrombosis--after surgery. The indications of this type of training are pointed out-extra-intracranial by pass, coronary by pass, flap revascularisation, etc... A good technique gained after an intense training together with an obvious individual predisposition are according to the authors the two main reasons of success in this meticulous surgery.

ABSTRACT

Antibiotic therapy for paronychia has seen its day. It is indicated only rarely and justified only when on the day following infection or during the next few days there are signs of regional or systemic spread. The surgical treatment of infections of the hand is not difficult but should be known, if not learned. A paronychia which has been opened but not cured should be reoperated upon rapidly. An "old" paronychia is a major catastrophe: small fistula, great damage. No surgical quarter for evil organisms!

ABSTRACT

Triangular dermal-fat flaps can be used in the excision of adherent vertical scars in the lower abdomen to prevent a recurrence of the depressed adhesion. Also, several such flaps can be used on the upper margin of an abdominoplasty, and one on the lower edge (above the pubis), to equalize the thickness and the lengths of the two edges. This results in a better final appearance. Finally, the use of a single triangular dermal-fat flap on the lower margin when doing a lipectomy for "subtrochanterric lipodystrophy" (or a "plastic on buttock") gives the buttock a more normal shape--round in the medial two-thirds and almost flat in the lateral one-third.

ABSTRACT

Repair of loss of substance of the lower lip has been the object of numerous studies. Each author has used his own ingenuity to describe a new type of flap aimed at restoring a particular form of loss of substance. The techniques suggested here are capable of solving a majority of problems on the basis of simple and original solutions. These flaps make it possible to resolve the problems posed by most cases of loss of substance of the lower lip. The choice of these plastic procedures permits restoration which is at one and the same time functional and anesthetic.
