ABSTRACT

BACKGROUND: Reduced flexion after total knee arthroplasty (TKA) compared to the preoperative values can affect patient's activities of daily living and lead to patient dissatisfaction. The aim of this study was to determine preoperative risk factors for developing a decrease in knee flexion after a primary TKA. METHODS: We identified 37 knees diagnosed with osteoarthritis with a preoperative knee flexion ≥120° but a 12-month postoperative range of motion (ROM) ≤110°. A random sample of 111 patients (1:3) from the same database, whose knees had a preoperative and 12-month postoperative ROM ≥120°, based on a diagnosis of primary osteoarthritis and no previous open knee surgery, were selected as the controls. RESULTS: We found female gender, black ethnicity, and the low preoperative knee-specific Knee Society Score to be significant risk factors for developing a reduction of knee flexion after TKA with odds ratios of 3.48, 5.11, and 0.95, respectively. CONCLUSION: In the absence of the well-recognized preoperative risk factors for a limited postoperative ROM, female patients, minorities, and those with lower knee-specific Knee Society Scores are at an increased risk of developing a decrease in flexion after an elective primary TKA.

ABSTRACT

La lesión de Monteggia consiste en la fractura del tercio proximal del cúbito asociada a la luxación anterior de la articulación radiocubital proximal. Es muy poco frecuente en niños (el 2% de las fracturas de la región del codo) y los casos inveterados son más inusuales. Se han descrito muchas estrategias terapéuticas para su tratamiento, sin que esté clara la necesidad de reconstrucción del ligamento anular. A continuación, describimos la técnica de Bell Tawse modifi cada a propósito de un caso de una paciente de 5+6 años con una lesión de Monteggia inveterada


Monteggia’s injury is a fracture of the proximal ulna which is associated with anterior dislocation of the proximal radioulnar joint. It is not a common injury in children (it only represents 2% of fractures of the elbow) and inveterate cases are even more unusual. Many surgical treatments have been proposed for this injury. Nevertheless, there is not a general agreement regarding the necessity of the annular ligament reconstruction. Here we describe the modifi ed technique of Bell-Tawse in a 5 and a half years patient with inveterate Monteggia injury

ABSTRACT

La cirugía artroscópica de reconstrucción del ligamento cruzado anterior en la rodilla es una técnica ampliamente utilizada en la actualidad. A las complicaciones propias de la técnica artroscópica, hay que sumar las de la extracción de la plastia, siendo las complicaciones vasculares las más raras y graves de todas ellas. Presentamos el caso de un paciente con un pseudoaneurisma arterial acontecido durante este procedimiento y discutimos el manejo y el tratamiento revisando la bibliografía actual (AU)


Today, reconstructive arthroscopic knee surgery of the anterior cruciate ligament is a widely used technique. In addition to the usual complications of arthroscopic techniques, the extraction of the piece must be taken into consideration, with vascular complications being the rarest and most serious of them all. We present the case of a patient who suffered an arterial pseudo aneurysm during this procedure. The management and treatment are discussed and the current bibliography reviewed (AU)

ABSTRACT

Se presenta el caso de un varón de 27 años con rotura meniscal en asa de cubo bicompartimental asociada a rotura del LCA en la rodilla derecha. El tratamiento realizado fue la ligamentoplastia del LCA con autoinjerto de tendones isquiotibiales y la meniscectomía parcial de ambos meniscos mediante técnica artroscópica, obteniendo unos excelentes resultados clínicos y funcionales. Aprovechamos este caso para revisar la bibliografía existente y exponer las opciones de tratamiento de las que se disponen según los hallazgos artroscópicos encontrados (AU)


The case of a 27 year old male with a displaced bucket-handle meniscal tear with associated anterior cruciate ligament rupture of the right knee is presented. Treatment included a ligamentoplasty of anterior cruciate ligament with hamstring autograph and partial meniscectomy of both meniscuses by means of arthroscopy, to obtain excellent clinical and functional results. We took advantage of this case to review existing bibliography and explain the various treatment options available based on the arthroscopic findings encountered (AU)
