ABSTRACT

Esta investigación examinó las diferencias en la activación muscular en los músculos de la cadera y muslo en corredores y corredoras con y sin el síndrome de la banda iliotibial (SFBI). Se registró la actividad neuromuscular en 21 corredores durante la carrera (14 SFBI y 7 sanos). No se han encontrado diferencias significativas en la actividad muscular media entre los corredores y corredoras lesionados. Sin embargo, en el caso de las corredoras lesionadas, se han encontrado diferencias entre el vasto lateral y el tensor fascia lata, y entre el vasto lateral y el bíceps femoral (p<0,05 en ambos casos). En el caso de los corredores hombres lesionados, se han encontrado diferencias entre el glúteo mayor y el tensor fascia lata, y entre el glúteo mayor y el bíceps femoral (p<0,05 en ambos casos). Estos hallazgos proporcionan un mayor entendimiento de la lesión y ayudarían a un tratamiento más específico. (AU)


A study was performed to examine differences in hip and thigh muscle activation in male and female runners with and without iliotibial band syndrome (ITBS). The neuromuscular activity of 21 runners was recorded during run (14 ITBS and 7 healthy). No significant differences were observed in mean muscle activity in injured male and female runners. In contrast, in female runners with ITBS, there were differences between the vastus lateralis and the tensor fasciae latae and between the vastus lateralis and the femoral biceps (p<0.05 in the two cases). With regard to male runners with ITBS, differences in activity were observed between the gluteus maximus and the tensor fasciae latae, and between the gluteus maximus and the femoral biceps (p<0.05 in the two cases). These findings contribute to a better understanding of iliotibial band syndrome and may be useful for the design of targeted treatments. (AU)

ABSTRACT

Objetivo: La diabetes mellitus tipo 2 (DM2) y la osteoporosis son enfermedades asociadas con un entorno pro-inflamatorio, cuya prevención mediante nuevas estrategias terapéuticas podría evitar su desarrollo. Sin embargo, existe un escaso número de estudios que evalúen el perfil inflamatorio de la osteoporosis en pacientes con DM2.El objetivo de este estudio se centró en evaluar la respuesta inflamatoria inmunitaria mediante concentraciones séricas de nueve citocinas, dos de ellas de carácter anti-inflamatorio (IL-10, IL-5) y seis pro-inflamatorias (IL-2, IL-6, IL-12 (p70), IL-17A, TNFα e IFNɣ) en 163 individuos con DM2 y 47 controles. Una subpoblación, formada por 43 pacientes DM2 sin osteoporosis, y 33 con osteoporosis, fue analizada en más profundidad a nivel de parámetros óseos. Además, hemos evaluado las hormonas calciotropas, los marcadores de remodelado óseo, densidad mineral ósea y fracturas vertebrales en la población, y hemos analizado la relación de las citocinas ensayadas con la DM2, la osteoporosis y las fracturas vertebrales prevalentes.Los pacientes con DM2 tenían concentraciones séricas significativamente más altas de IL-10 en comparación con el grupo control (0,5±1 vs. 0,14±0,3 pg/ml; p=0,016) y los niveles de IL-12 p70 se mostraron más bajos en pacientes con DM2 respecto a los controles (2,9±1,6 vs. 3,9±3,1 pg/ml; p=0,027).En el grupo de pacientes con DM2 y osteoporosis, los niveles de la citocina IL-6 resultaron elevados respecto al grupo de DM2 sin osteoporosis (10,9±14,6 vs. 4,5±7,0; p=0,017). También se observó una asociación de IL-5, siendo sus niveles más bajos en el grupo DM2 con osteoporosis (1,7±0,2 vs. 3,8±0,6; p=0,032). Además, la IL-5 mostró una correlación directa con los niveles del biomarcador de formación ósea fosfatasa alcalina ósea (r=0,277, p=0,004) en la subpoblación de pacientes con DM2. El resto de citocinas no mostraron diferencias significativas. (AU)

ABSTRACT

Resumen El diagnóstico diferencial de las lesiones destructivas de la línea media es amplio y complejo debido a su presentación clínica poco específica, pudiendo abarcar desde cuadros infecciosos, neoplásicos y patologías autoinmunes. Una entidad a considerar son las lesiones destructivas de la línea media inducidas por cocaína (CIMDL), la que se caracteriza por una extensa destrucción de estructuras a nivel medio-facial. Se presenta el caso de un paciente con antecedentes de consumo de cocaína con una gran lesión destructiva endonasal asociada a una perforación palatal, fiebre, cervicalgia y meningismo. Se realizó un estudio nasofibroscópico y exámenes de laboratorio donde destaca un leve aumento de parámetros inflamatorios y serología positiva para anticuerpos anticitoplasma de neutrófilo (ANCA). Los exámenes imagenológicos destacan una trombosis de carótida interna derecha, osteítis del clivus y paquimeningitis retroclival. Se decide ingresar a pabellón para realizar un aseo y tomar biopsias. Al descartar los diagnósticos diferenciales de CIMDL, el paciente inició terapia antibiótica y antifúngica endovenosa evolucionando en forma satisfactoria. La CIMDL es una condición infrecuente cuyo principal diagnóstico diferencial es la granulomatosis con poliangeitis. Dada similitud en las presentaciones clínicas y la gran diferencia en el manejo de ambas patologías la necesidad de un diagnóstico certero es fundamental.


Abstract Differential diagnosis of destructive midline injuries is complex due to its nonspecific clinical presentation, and the broad variety of conditions that share the same signs and symptoms, such as infections, neoplasms and autoimmune diseases. One of the etiologies that should be considered is cocaine-induced destructive midline lesions (CIMDL), cha- racterized by extensive destruction of midface structures. The following case describes a patient with history of cocaine abuse with a large destructive endonasal lesion asso- ciated with a palatal perforation, fever, cervicalgia and meningism. Clinical assessment included a nasofibroscopy and laboratory tests. Results revealed a slight increase in in- flammatory parameters and positive antineutrophil cytoplasmic antibody serology. Ad- ditionally, imaging revealed a thrombosis of the right internal carotid, clivus osteitis, and retroclival pachymeningitis. Surgical debridement was performed, and biopsies of the nasal cavity were taken. Once the other possible diagnoses were ruled out, the patient was started on intravenous antibiotic and antifungal therapies. The patient showed a positive response to treatment and was successfully discharged after two months. CIMDL is a rare condition whose main differential diagnosis is granulomatosis with polyangiitis. Given the similarity in clinical presentations and the important differences in the management of both pathologies, the need for an accurate diagnosis is essential.

Resumen Introducción: Debido a la pandemia por SARS-CoV-2, las consultas presenciales y cirugías electivas fueron suspendidas o disminuidas para evitar la propagación viral hacia pacientes y personal de salud. En nuestro país se ha implementado la telemedicina como canal alternativo de atención. Debido a su reciente implementación en nuestra especialidad a nivel nacional, es importante conocer la percepción del paciente y proveedor sobre ella, con el fin de evaluar su factibilidad como nueva modalidad de atención en el futuro. Objetivo: Evaluar la satisfacción del paciente y del proveedor sobre el uso de telemedicina en Otorrinolaringología en nuestro hospital. Material y Método: Estudio prospectivo observacional. Se adaptaron dos instrumentos de evaluación de satisfacción reportados en la literatura, para el paciente y proveedor de salud. Las encuestas fueron administradas luego de la consulta de telemedicina a través de correo electrónico para el paciente, y como parte de la plataforma de telemedicina para el proveedor y respondidas en forma voluntaria. Las respuestas fueron analizadas utilizando medidas de frecuencia (porcentajes). Resultados: Se recopilaron 51 encuestas de pacientes y 69 de proveedores. En los pacientes, en un 98% la telemedicina facilitó el acceso a atención, siendo del mismo nivel que una presencial (91%). Un 98% volvería a usarla y la recomendaría a familiares o amigos. En los proveedores, el 98% estuvo satisfecho con la teleconsulta y 89% con la plataforma. Conclusión: La atención por telemedicina es satisfactoria para usuario y proveedor en nuestro hospital. Esta es una herramienta valiosa para complementar la atención clínica otorrinolaringológica.


Abstract Introduction: Due to the SARS-CoV-2 pandemic, clinical consults and elective surgeries were suspended or decreased to avoid spreading the disease to patients and health personnel. In our country telemedicine has been implemented as an alternate way to obtain medical attention. Given that this attention paradigm has only been recently implemented in our specialty at the national level, it is important to know the perception of the patient and health care provider in order to evaluate its feasibility as a new modality of care in the future. Aim: To assess the patients and healthcare providers satisfaction regarding the use of telemedicine in the Otorhinolaryngology Department. Material and Method: Prospective observational study. Two satisfaction surveys previously reported in the literature were adapted for the patient and health provider. The surveys were sent after each telemedicine consultation via email to the patient, and as part of the telemedicine platform for the provider and answered voluntarily. The responses were analyzed using proportions. Results: answered surveys were obtained from 51 patients and 69 healthcare providers. In 98% of patient's telemedicine facilitated access to care, which was found to be as good as a clinical consult in 91% of the telehealth visits. 98% would use it again and recommend it to family or friends. In 98% of providers were satisfied with the teleconsultation and 89% with the platform. Conclusion: teleconsultation is satisfactory for both patients and providers in our hospital. This new attention modality is a valuable tool to complement the standard otolaryngologist clinical assessment.

Resumen Introducción: La infección tardía es una posible complicación del implante de un esfínter urinario artificial (método más aceptado para tratar la incontinencia urinaria de esfuerzo secundaria a prostatectomía radical). Si sucede, debe retirarse la prótesis y colocar una sonda vesical hasta su reinserción meses más tarde. El paciente se convierte en portador domiciliario de sonda vesical, que puede adquirir una infección de tracto urinario por un mal cuidado. Objetivo: Desarrollar sesiones educativas para que un paciente adquiera destreza en el manejo de sonda vesical y consecuentemente, prevenir las infecciones asociadas. Desarrollo: Se presenta el caso de un varón de 80 años, portador de sonda vesical por retirada temporal de su esfínter urinario artificial. Al realizar la valoración de enfermería, se identifica el diagnóstico enfermero Conocimientos deficientes en relación con los cuidados del dispositivo. Posteriormente, se seleccionan los resultados esperados y se planifican las intervenciones educativas. Tras llevarlas a cabo identificamos que el paciente incrementó sus conocimientos sobre el manejo de la sonda vesical. Conclusiones: La educación sanitaria es una herramienta de prevención del riesgo de infección asociado al mantenimiento de la sonda vesical en el domicilio, ya que, si el paciente no es capaz de afrontar su autocuidado, el tratamiento pierde efectividad. Es imprescindible identificar las necesidades formativas de los pacientes, abordarlas mediante intervenciones educativas reforzadas con documentación escrita durante el ingreso, para proporcionar la información necesaria y fortalecer su comprensión, por lo que puede mejorar el nivel de autocuidado, así como prevenir complicaciones tras el alta hospitalaria.


Abstract Introduction: Late infection is a possible complication an artificial urinary sphincter´s implant the most accepted method to treat urinary incontinence secondary to radical prostatectomy. If this happens, the prosthesis must be removed, and a urinary catheter was placed. Then, the patient becomes a urinary catheter home carrier prone to acquire urinary tract infections as a result of carelessness. Objective: To develop educational interventions so that these patients acquire dexterity to manage the urinary catheter and thus, prevent the possible associated infections. Development: A case of an 80 years old male carrying a urinary catheter temporarily substituting an artificial urinary sphincter is addressed. The nursing diagnosis assessment is performed. The patient demonstrates Insufficient knowledge related to the management of the device. Educational interventions addressing the situation are planned. After the implementation of the strategy, the patient improves his knowledge of urinary catheter management. Conclusions: Health education is an infection risk prevention tool that can address the management of urinary catheters in-home patients, improving the effectiveness of the related treatments. It is essential to identify the needs of the patients and help address them through educational interventions to enhance self-care and prevent possible complications.

Se acontecer, deve retirar-se a prótese e colocar uma sonda vesical até sua reinserção meses mais tarde. O paciente torna-se portador domiciliar de sonda vesical, podendo adquirir uma infeção de tracto urinário por um mal cuidado. Objetivo: Desenvolver sessões educativas para que um paciente adquira destreza no manejo de sonda vesical e consequentemente, prevenir as infecções associadas. Desenvolvimento: Presenta-se o caso de um homem de 80 anos, portador de sonda vesical por retirada temporal de seu esfíncter urinário artificial. Realiza-se a valoração de enfermagem, onde se identifica o diagnóstico de enfermagem Conhecimentos deficientes ao respeito dos cuidados do dispositivo. Posteriormente, selecionam-se os resultados esperados e planificam-se as intervenções educativas. Após levá-las a cabo, consegue-se que o paciente incremente seus conhecimentos sobre o manejo da sonda vesical. Conclusões: A educação sanitária é uma ferramenta de prevenção do risco de infeção associado à manutenção da sonda vesical na residência, já que, se o paciente não for capaz de afrontar seu autocuidado, o tratamento perde efetividade. É imprescindível identificar as necessidades formativas dos pacientes e combatê-las mediante intervenções educativas reforçadas com documentação escrita durante o ingresso, para proporcionar a informação necessária e reforçar sua compreensão, pelo que pode melhorar o nível de autocuidado e prevenir complicações após a alta hospitalar.

El diagnóstico diferencial de la macroglosia es amplio y puede estar en el contexto de una patología localizada o una enfermedad sistémica. Dentro de las enfermedades sistémicas que se manifiestan con macroglosia se describe la amiloidosis, caracterizada por un depósito irreversible de una proteína amorfa y fibrilar El compromiso de esta patología en el territorio de cabeza y cuello es infrecuente y su depósito en la lengua corresponde a menos del 9% de los casos. Se presenta el caso de un paciente que consultó por dolor en hemilengua derecha, disfagia y baja de peso. Se estudió inicialmente con exámenes de laboratorio y nasofibroscopía, sin hallazgos concluyentes. Dado persistencia de síntomas se realizó resonancia nuclear magnética (RM) que mostraba signos sugerentes de enfermedad de depósito, confirmándose mediante estudios histopatológicos una amiloidosis sistémica secundaria. El paciente fue derivado a hematología para completar estudio e iniciar el tratamiento. La amiloidosis sistémica es un diagnóstico infrecuente, que debe ser considerado por su mal pronóstico vital. El diagnóstico en etapas iniciales puede mejorar sustancialmente la sobrevida y calidad de vida de aquellos que padecen la enfermedad. Por lo anterior, es necesario completar un estudio acabado de la patología, apoyándose en métodos no invasivos como la RM.


The differential diagnosis of macroglossia is broad, and it may be present in the context of a localized pathology or a systemic disease. One of the systemic diseases that present macroglossia is amyloidosis, which is characterized by an irreversible deposit of an amorphous and fibrillar protein. The manifestation of this pathology in head and neck territory is infrequent, and its deposit in the tongue represents less than 9% of all types of amyloidosis. We present the case of a patient, who consulted with pain in the tongue, dysphagia, and weight loss. He was initially studied with laboratory tests and a nasofibroscopy with no conclusive findings. Given the persistence of symptoms, magnetic resonance imaging (MRI) was performed, showing signs of an infiltrative disease, which was confirmed as secondary systemic amyloidosis through histopathological studies. Given the above, the patient was referred to hematology to start treatment. Systemic amyloidosis is an uncommon diagnosis that should be suspected since it implies a poor vital prognosis. Moreover, an early diagnosis can substantially improve the survival rate and quality of life of those who suffer this disease. Therefore, a comprehensive study of this condition is needed, complementing with non-invasive methods such as MRI.

ABSTRACT

OBJETIVOS: Investigar el uso de la 18F-FDG(flúor-18 fluorodesoxiglucosa) PET/TC en la estadificación del cáncer de cabeza y cuello (CCC) y su repercusión en la decisión terapéutica y planificación de tratamiento radioterápico. MATERIAL Y MÉTODOS: Se incluyen 100 pacientes con CCC y la siguiente localización tumoral: 18% cavidad oral, 20% orofaringe, 12% hipofaringe, 11% nasofaringe, 37% laringe, 2% senos paranasales. La estadificación tumoral según la AJCC (American Joint Committee of Cancer, 7th) es: 5%-I, 7%-II, 14%-III, 61%-IVA, 7%-IVB, 6%-IVC. Se les realiza una TC y una 18F-FDG PET/TC en condiciones de simulación para comparar la reclasificación del estadiaje. Además, se analizan los cambios de actitud terapéutica. RESULTADOS: La 18F-FDG PET/CT detecta 6 pacientes metastásicos que requieren tratamiento paliativo y 8 tumores sincrónicos, siendo uno paliativo. Se produce una reclasificación del estadiaje en 27 pacientes. La extensión tumoral varía en 28 (14% sobre-estadificación, 14% infra-estadificación), implicando una variación en el contorneo del GTV (Gross Tumor Volume). La estadificación ganglionar cambia en 47: 8 pacientes son infra-estadificados (N2C cambia a N2A/N2B/N1) y 2 son falsos positivos. Diecinueve pacientes son falsos negativos y 5 con afectación ganglionar unilateral (N1/N2A/N2B) muestran actividad metabólica bilateral. Estos cambios de estadificación implican una adaptación del volumen ganglionar a irradiar. CONCLUSIONES: La 18F-FDG PET/TC produce una reclasificación superior al 10% en casi todas las categorías estudiadas (cTNM, extensión tumoral, enfermedad ganglionar) y detecta más estadíos metastásicos y tumores sincrónicos que los estudios convencionales, lo que genera un impacto en el manejo del paciente y contorneo de los volúmenes de radioterapia


PURPUSE: The aim is to investigate the use of 18F-FDG (fluorine-18 fluorodeoxyglucose) PET/CT in head and neck cancer (HNC) staging and its effect on the therapeutic strategy and radiotherapy (RT) planning. METHODS AND MATERIALS: One hundred patients with HNC were included. Primary tumor sites: 18% oral cavity, 20% oropharynx, 12% hypopharynx, 11% nasopharynx, 37% larynx, 2% paranasal sinuses. Patients were staged according to the American Joint Committee of Cancer 7th edition. Stage: 5% stage I, 7% stage II, 14% stage III, 61% stage IVA, 7% stage IVB and 6% stage IVC. A contrast-enhanced CT and a 18F-FDG PET/CT acquired under RT position were performed. Both exams were compared to analyze patients' staging reclassification. Changes in therapeutic strategy were analyzed. RESULTS: 18F-FDG PET/CT detected 6 distant metastases and treatment intention changed to palliative. Eight synchronous tumors were detected; one received palliative treatment.18F-FDG PET/CT reclassified cTNM staging in 27patients. Tumor extension changed in 28 (14% up-staged; 14% down-staged), implying a change in GTV (Gross Tumor Volume) delineation. Nodal detection was reclassified in 47 PATIENTS: 8 patients down-staged (N2C to N2A/N2B/N1) and 2 were false positive. Nineteen patients were false negatives and 5 staged as N+(N1/N2A/N2B) turned out into N2C. These staging modifications imply adapting the nodal volume to be irradiated. CONCLUSIONS: 18F-FDG PET/CT reclassification was higher than 10% in almost all categories studied (cTNM, tumor extension and nodal disease) and detects more metastases and synchronous tumors than conventional studies, which has an impact on the therapeutic patient management and RT planning

ABSTRACT

RESUMEN La otomastoiditis tuberculosa es una presentación extremadamente rara de la forma extrapulmonar de la enfermedad y puede ser difícil llegar a su diagnóstico. Presentamos el caso de una paciente de 35 años con otomastoiditis tuberculosa bilateral acompañado de vértigo, hipoacusia mixta bilateral y paresia del nervio facial bilateral, como debut de una tuberculosis. Cultivos de Mycobacterium tuberculosis (MTB) y prueba de reacción en cadena de la polimerasa (PCR) de otorrea fueron inicialmente negativos. La tomografía computarizada de oídos y resonancia magnética mostraron cambios inflamatorios otomastoídeos bilaterales sin evidencia de erosión ósea ni extensión a partes blandas. Se realizó una mastoidotomía, las muestras del tejido obtenido evidenciaron osteomielitis crónica, bacterias ácido-alcohol resistentes y PCR positiva para MTB. La paciente recibió tratamiento con drogas antituberculosas durante 12 meses logrando una recuperación completa de la otalgia y vértigo, y mejoría parcial de audición y paresia facial. En resumen, los hallazgos clínicos e imagenológicos de la otomastoiditis tuberculosa son inespecíficos por lo cual se requiere de un alto índice de sospecha clínica para lograr el diagnóstico adecuado e iniciar el tratamiento de la infección subyacente.


ABSTRACT Tuberculous otomastoiditis is an extremely rare form of extrapulmonary disease that can be easily misdiagnosed. We hereby report the case of a previously healthy 35-yearold female with bilateral tuberculous otomastoiditis associated with vertigo, bilateral mixed hearing loss, and bilateral facial nerve palsy as the initial clinical presentation. Repeated Mycobacterium tuberculosis (MTB) culture and molecular testing of otorrhea aspirates were initially negative. High-resolution temporal bone computed tomography and magnetic resonance imaging showed partial opacification of the mastoid air cells without signs of bone erosion. A mastoidotomy was performed with mastoid tissue showing chronic osteomyelitis, positivity in acid-fast staining and MTB PCR. The patient was treated with a 12 month antituberculous treatment, with complete recovery of otalgia and vertigo, and improvement in hearing levels and facial nerve palsy. In summary, clinical and imaging findings for tuberculous otomastoiditis are non-specific, hence a high degree of suspicion is required in order to diagnose and promptly treat the underlying infection.

RESUMEN La voz hipernasal y la regurgitación nasal son síntomas de disfunción velofaríngea. Ésta puede tener múltiples causas: anatómicas, neurológicas o funcionales. Se describe el caso de una paciente de sexo femenino, de 13 años, que se presenta con voz hipernasal y regurgitación nasal aguda. Al examen físico se evidencia inmovilidad del velo del paladar derecho sin otros hallazgos neurológicos. El estudio con resonancia nuclear magnética de cerebro y punción lumbar fueron normales. Se diagnosticó una incompetencia velofaríngea aguda transitoria, de probable etiología viral. La paciente evolucionó de forma favorable con mejoría clínica progresiva. La incompetencia velofaríngea a causa de una paresia o parálisis del nervio vago y/o nervio glosofaríngeo es una causa poco frecuente de disfunción velofaríngea.


ABSTRACT Hypernasal speech and nasal regurgitation are symptoms of velopharyngeal dysfunction. This may have multiple causes, including velopharyngeal incompetence due to paresis or paralysis of the vagus nerve and/or glossopharyngeal nerve. We describe the case of a 13 year-old female patient, with hypernasal speech and acute nasal regurgitation, with a physical examination showing immobility of the right palate with no other neurological findings. Magnetic resonance imaging of the brain and lumbar puncture was normal. Transient acute velopharyngeal incompetence was diagnosed, probably of viral etiology. The patient evolved favorably with progressive clinical improvement. Velopharyngeal incompetence due to paresis or paralysis of the vagus and/or glossopharyngeal nerves is a rare cause of velopharyngeal dysfunction.

ABSTRACT

PURPOSE: The aim is to investigate the use of 18F-FDG (fluorine-18 fluorodeoxyglucose) PET/CT in head and neck cancer (HNC) staging and its effect on the therapeutic strategy and radiotherapy (RT) planning. METHODS AND MATERIALS: One hundred patients with HNC were included. Primary tumor sites: 18% oral cavity, 20% oropharynx, 12% hypopharynx, 11% nasopharynx, 37% larynx, 2% paranasal sinuses. Patients were staged according to the American Joint Committee of Cancer 7th edition. Stage: 5% stage I, 7% stage II, 14% stage III, 61% stage IVA, 7% stage IVB and 6% stage IVC. A contrast-enhanced CT and a 18F-FDG PET/CT acquired under RT position were performed. Both exams were compared to analyze patients' staging reclassification. Changes in therapeutic strategy were analyzed. RESULTS: 18F-FDG PET/CT detected 6 distant metastases and treatment intention changed to palliative. Eight synchronous tumors were detected; one received palliative treatment. 18F-FDG PET/CT reclassified cTNM staging in 27patients. Tumor extension changed in 28 (14% up-staged; 14% down-staged), implying a change in GTV (Gross Tumor Volume) delineation. Nodal detection was reclassified in 47 patients: 8 patients down-staged (N2C to N2A/N2B/N1) and 2 were false positive. Nineteen patients were false negatives and 5 staged as N+(N1/N2A/N2B) turned out into N2C. These staging modifications imply adapting the nodal volume to be irradiated. CONCLUSIONS: 18F-FDG PET/CT reclassification was higher than 10% in almost all categories studied (cTNM, tumor extension and nodal disease) and detects more metastases and synchronous tumors than conventional studies, which has an impact on the therapeutic patient management and RT planning.

ABSTRACT

RESUMEN La enfermedad de Rosai-Dorfman (ERD) o histiocitosis sinusal con linfadenopatía masiva es una enfermedad infrecuente, de etiología desconocida caracterizada por linfadenopatías cervicales masivas bilaterales. El compromiso extraganglionar puede ocurrir en diferentes sitios, incluida la vía aérea, en la cual la localización nasosinusal es la más frecuente, pero puede comprometer también otros sitios de la vía aérea superior. Dentro de éstos, el compromiso laríngeo es muy poco frecuente. Se presenta el caso de una paciente de 82 años con antecedentes de ERD localizada en los ganglios linfáticos cervicales en remisión espontánea, que desarrolló posteriormente una obstrucción subaguda de las vías respiratorias. La tomografía computarizada y la nasofaringolaringoscopía mostraron lesiones subglóticas que obstruían severamente las vías respiratorias. Se realizó una traqueostomía de emergencia y biopsia de las lesiones, confirmando el diagnóstico de ERD extranodal.


ABSTRACT Rosai-Dorfman disease or sinus histiocytosis with massive lymphadenopathy is a rare disease of unknown etiology characterized by bilateral massive cervical lymphadenopathy. Extranodal involvement can occur in different sites including airway where nasosinusal involvement is common but this disease may occur in other sites of the upper airway. Laryngeal involvement is rare. We present the case of an 82-year-old female patient with a history of Rosai-Dorfman disease located in cervical lymph nodes in spontaneous remission that subsequently develops airway obstruction. Computed tomographic scan and nasopharyngolaryngoscopy showed subglottic lessons that severely obstructed the airway. Emergency tracheotomy and biopsy were performed, confirming the diagnosis of extranodal RDD. Patient was observed and tracheostomy was maintained with good tolerance.

ABSTRACT

Background: Atrophic gastritis (AG) and intestinal metaplasia (IM) are stages that appear in the process of gastric carcinogenesis. Their presence requires programmed endoscopic vigilance. Objectives: To determine the frequency of AG and IM in gastric biopsies (GB) taken according to Sydney Protocol and to correlate them with endoscopic findings. Methods: Retrospective descriptive analysis of 233 upper gastrointestinal endoscopies with GB per Sydney Protocol. OLGA (Operative Link for Gastritis Assessment) and OLGIM (Operative Link for Gastric Intestinal Metaplasia Assessment) scores were calculated based on the GB description. Endoscopic findings were analyzed for atypical findings and compared to the GB report. Statistic analysis for Kappa and ANOVA was performed via Stata 12. Results: Mean age of patients was 58 ± 12 years. 69% were women. The frequency of AG and IM was 44% and 33% in the antrum, 31% and 20% in the angular incisure and 14% and 9% in the body, respectively. AG and IM were more frequent in the antrum (p < 0.05). AG and IM were more severe in the angular incisure and body (p < 0.05). We were unable to calculate OLGA and OLGIM in 6% and 9% of cases, respectively, due to absence of severity description in GB. 53% were OLGA 0, 42% OLGA I-II and 5% OLGA III-IV. 70% were OLGIM 0, 25% OLGIM I-II and 5% OLGIM III-IV. Agreement between endoscopic and histological findings was best for IM in the antrum (75.5%, Kappa 0.4). Sensitivity and specificity of endoscopic findings were 39% and 70% for AG, and 30% and 85% for IM, respectively. Conclusion: AG and IM are frequent findings in our patients. Due to the low endoscopic sensitivity for AG and IM, we suggest a systematic GB sampling using Sydney Protocol in patients over 40 years old.


Introducción: La gastritis crónica atrófica (GCA) y la metaplasia intestinal (MI) son etapas en el proceso de carcinogénesis gástrica, su presencia requiere control endoscópico programado. Objetivos: Determinar la frecuencia de GCA y MI en biopsias gástricas (BG) por protocolo de Sydney y relacionarlas con el hallazgo endoscópico. Métodos: Estudio descriptivo mediante revisión de 233 endoscopias digestivas altas con BG por Protocolo Sydney. Se graduó puntaje OLGA (Operative Link for Gastritis Assessment) y OLGIM (Operative Link for Gastric Intestinal Metaplasia Assessment) según la descripción de la BG. Se definió el hallazgo endoscópico según su informe y se comparó con BG como patrón de referencia. Estadística: Stata 12 para Kappa y ANOVA. Resultados: Edad promedio 58 ± 12 años, 69% mujeres. La frecuencia de GCA y MI en antro fue de 44 y 33%, en ángulo 31 y 20% y en cuerpo 14 y 9%, respectivamente. Hubo mayor frecuencia de GCA y MI en antro (p < 0,05). La graduación de GCA y MI fue mayor en ángulo y cuerpo (p < 0,05). No se obtuvo OLGA en 6% y OLGIM en 9% por ausencia de graduación. La frecuencia de OLGA 0 fue de 53%, OLGA I-II 42%, OLGA III-IV 5%, OLGIM O 70%, OLGIM I-II 25% y OLGIM III-IV 5%. La mejor correlación se observó entre la MI antral endoscópica con la histológica (75,5%, Kappa 0,4). La sensibilidad y especificidad endoscópica fue de 39 y 70% para GCA y 30 y 85% para MI. Conclusión: GCA y MI son hallazgos frecuentes en nuestros pacientes. Por la baja sensibilidad endoscópica en la identificación de GCA y MI sugerimos la toma sistemática de BG por protocolo de Sydney en pacientes mayores de 40 años.

ABSTRACT

Thaumatin-like proteins (TLPs) are enzymes with important functions in pathogens defense and in the response to biotic and abiotic stresses. Last identified olive allergen (Ole e 13) is a TLP, which may also importantly contribute to food allergy and cross-allergenicity to pollen allergen proteins. The goals of this study are the characterization of the structural-functionality of Ole e 13 with a focus in its catalytic mechanism, and its molecular allergenicity by extensive analysis using different molecular computer-aided approaches covering a) functional-regulatory motifs, b) comparative study of linear sequence, 2-D and 3D structural homology modeling, c) molecular docking with two different ß-D-glucans, d) conservational and evolutionary analysis, e) catalytic mechanism modeling, and f) IgE-binding, B- and T-cell epitopes identification and comparison to other allergenic TLPs. Sequence comparison, structure-based features, and phylogenetic analysis identified Ole e 13 as a thaumatin-like protein. 3D structural characterization revealed a conserved overall folding among plants TLPs, with mayor differences in the acidic (catalytic) cleft. Molecular docking analysis using two ß-(1,3)-glucans allowed to identify fundamental residues involved in the endo-1,3-ß-glucanase activity, and defining E84 as one of the conserved residues of the TLPs responsible of the nucleophilic attack to initiate the enzymatic reaction and D107 as proton donor, thus proposing a catalytic mechanism for Ole e 13. Identification of IgE-binding, B- and T-cell epitopes may help designing strategies to improve diagnosis and immunotherapy to food allergy and cross-allergenic pollen TLPs.

OBJECTIVE: We aimed to compare quality of life in two clinical conditions treated with immunosuppressants: cadaveric liver transplant recipients and multiple sclerosis patients. We also assessed the clinical significance of these results regarding a representative age-adjusted sample of the general Spanish population. METHODS: Using a cross-sectional design, the SF-36 Health Survey was used to evaluate 62 patients with these chronic conditions (31 in each group) who were matched for gender. An analysis of covariance was performed to control for the influence of time from multiple sclerosis diagnosis and liver transplantation surgery until assessment. Student t test of covariate-adjusted mean values was used as the statistical test and Cohen's d effect size index, to assess the magnitude of intergroup differences and assess clinical significance. RESULTS: Significantly worse scores were observed among the neurological patients compared with transplant recipients regarding role-physical (P = .038), general health (P = .003), vitality (P = .034), and physical functioning (P = .049), with medium effect sizes (Cohen's ds from -0.511 to -0.785). Against normative values, liver transplant recipients displayed relevant differences in all SF-36 subscales (Cohen's ds from -0.569 to -0.974) except for mental health (small effect size). Likewise, multiple sclerosis patients showed much greater differences versus the general population (Cohen's ds from -0.846 to -1.760). CONCLUSIONS: Liver transplant recipients showed better quality of life than multiple sclerosis patients (medium effect sizes) in physical quality-of-life dimensions. Interestingly, despite having controlled for time from diagnosis/transplantation, both medical conditions showed clinically significant impairments (large and medium effect sizes) in physical and psychosocial quality-of-life domains. We concluded that transplant recipients belong to a population that still requires special health care because, even after having undergone their treatment of choice, they do not achieve normal levels of biopsychosocial functioning.

OBJECTIVE: To compare the biopsychosocial functioning among liver transplantation and cirrhotic patients as a function of self-perceived pain level. METHODS: We selected two groups of liver patients of the same gender (men) and disease etiology (alcoholic): there were 39 liver transplant recipients and 34 severe cirrhotic patients. The Hospital Anxiety and Depression Scale and the SF-36 Health Questionnaire were used. We applied analysis of covariance, with age and model end-stage liver disease (MELD) scores as covariates to assess the influence of two independent factors: (1) group (liver transplant recipients and cirrhotic patients), and (2) self-perception of pain (mild and high). We also calculated Cohen's d as an effect size index. RESULTS: No interactive effects were found between factors group and self-perceived pain in any of the variables studied. With regard to the main effects, we found statistically significant differences in the following variables between: a) liver transplant recipients and cirrhotic patients: anxiety (P = .000), depression (P = .003), role-physical (P = .001), mental health (P = .016), general health (P = .000), vitality (P = .000), and physical functioning (P = .000); and b) liver patients with mild and high self-perceived pain: anxiety (P = .008), depression (P = .000), role-physical (P = .002), mental health (P = .000), vitality (P = .000), and physical functioning (P = .001). In all the indicated variables, with medium and large effect sizes (Cohen's ds from 0.58 to 1.64), the cirrhotic patients and patients with a high level of self-perceived pain had much poorer mental health and quality of life. CONCLUSION: Alcoholic cirrhotic male patients with a high level of self-perceived pain had the greatest biopsychosocial impairment, even exceeding the clinical threshold in the anxiety and depression scales; and the highest biopsychosocial well-being was associated with liver transplant recipients with a mild level of self-perceived pain.

OBJECTIVE: To determine whether differences in the coping strategies used by liver patients during the pretransplantation phase were a function of their relatives' level of anxiety. MATERIALS AND METHODS: We assessed 75 pre-liver transplantation patients and 75 relatives (one per patient). To assess relatives' anxiety status, we used the Hospital Anxiety and Depression Scale (HADS), and the Questionnaire of Coping with Stress in Cancer Patients (CAEPO) to study patients' coping strategies. Three subgroups of relatives were established as a function of their scores on the HADS anxiety subscale: normal anxiety (G(1)), dubious anxiety (G2), and clinical anxiety (G(3)). To verify intergroup differences in the coping strategies used by the patients, we used the nonparametric Kruskal-Wallis H test. We also performed pairwise comparisons with nonparametric Mann-Whitney U test (with Bonferroni's correction) and Cohen's d as an effect size index. RESULTS: Focusing on the most relevant effect sizes, the pairwise contrasts indicated the following differences: a) normal anxiety (G(1)) and dubious anxiety (G(2)): seeking social support (d = 0.502); b) normal anxiety (G(1)) and clinical anxiety (G(3)): coping and active fighting (d = 0.607), self-control and emotional control (d = 0.658), and seeking social support (d = 0.944); and c) dubious anxiety (G(2)) and clinical anxiety (G(3)): coping and active fighting (d = 0.743), self-control and emotional control (d = 0.722), and seeking social support (d = 0.515). CONCLUSION: In general, during the pre-liver transplantation study, the liver patients whose relatives showed clinical levels of anxiety used these three healthy coping strategies to a lesser extent: coping and active fighting, self-control and emotional control, and seeking social support.

OBJECTIVE: We aimed to determine whether there were differences with regard to anxiety and depressive symptomatology between liver transplant recipients with better (G(1)) versus worse (G(2)) self-perceptions of general health compared with pre-liver transplantation cirrhotic patients (G(3)). METHODS: The groups of patients included 168 recipients including 85 and 83 with better or worse self-perceptions of general health, respectively, and 75 cirrhotic pre-liver transplantation patients. For the psychological assessment we used the Hospital Anxiety and Depression Scale and the general health dimension of the SF-36 Health Questionnaire. The following analyses were used: Analysis of variance (ANOVA) with post hoc pairwise comparisons by means of Tukey's test and Cohen's d, an effect size index. RESULTS: Significant differences were observed among the three groups for the variables of anxiety (P = .000) and depression (P = .000). Specifically, liver transplant recipients with better self-perceptions of general health displayed lower scores (better mental health) compared with those showing worse self-perceptions or cirrhotic patients. There were no differences between the latter two groups. The differences in these variables were relevant (large effect sizes) for anxiety (Cohen's d(1-2) = -1.075, Cohen's d(1-3) = -1.155) and for depression (Cohen's d(1-2) = -1.145, Cohen's d(1-3) = -1.158). CONCLUSION: The anxious-depressive status was not necessarily better among liver transplant recipients. There was great variability among them as a function of self-perceived general health. Transplant recipients with worse self-perception of general health presented the same anxiety-depressive levels as patients with severe liver disease in the pretransplantation phase; the latter groups reach the clinical threshold on the depression scale.
