ABSTRACT

AIMS: To compare the dental caries prevalence using the International Caries Detection and Assessment System (ICDAS) and the caries risk by Caries Management by Risk Assessment (CAMBRA) in individuals with cerebral palsy (CP) and normoactives (NAs). METHODS AND RESULTS: Sixty children and adolescents aged 6-12 years (30 CP/30 NA) were clinically evaluated by one calibrated examiner using two-digit ICDAS criteria and converted into components of dmf/DMF indices: d2mf2/D2MF2 (enamel and dentin lesions) and d3mf3/D3MF3 (dentin lesions). An adapted CAMBRA was used for risk classification. The mean d2mf2s/d2mf2t and D2MF2S/D2MF2T for CP were 17.0 ± 16.8/7.5 ± 4.3 and 10.7 ± 17.6/5.3 ± 5.8, respectively, and for NA were 17.2 ± 16.9 /6.9 ± 4.8 and 11.1 ± 11.7/5.5 ± 4.7, respectively. The mean d3mf3s/d3mf3t and D3MF3S/D3MF3T for CP were 10.1 ± 16.7/3.0 ± 4.1 and 4.9 ± 15.6/0.2 ± 0.4, respectively, while for NA the mean values were 9.8 ± 13.0/3.5 ± 3.8 and 2.1 ± 5.7/0.9 ± 2.0, respectively. There were no statistically differences for caries prevalence and risk in both groups (p > 0.05). CONCLUSIONS: Dental caries was highly prevalent in CP and NA children and adolescents. Enamel and dentin lesions and high caries risk were the most common condition.

ABSTRACT

BACKGROUND: The outbreak of microcephaly between 2015 and 2017 in Brazil created an international public health emergency. AIM: This study sought to evaluate the chronology, sequence of eruption, and the presence of anomalies amongst children with microcephaly compared with normoreactive children. DESIGN: A comparative study of 108 children aged 0-2 years attending public services was conducted. The microcephaly group (MG; n = 54) and the nonmicrocephaly group (NMG; n = 54) were matched by age, gender, and family income at a 1 : 1 ratio. Erupted teeth and the presence of anomalies were evaluated and compared. Chi-square tests, Pearson's correlations, Fisher's exact tests, and Student's t-tests were applied (α = 0.05). RESULTS: No between-group differences were found with regard to gender (P = 0.560) or age (P = 0.529). The MG presented with a higher percentage of alterations in eruption sequence, opacity, self-injury, and delayed eruption compared with the NMG (P < 0.05). The MG was more likely to exhibit changes in tooth eruption sequence (OR = 12.23, 95% CIs = 3.25-46.03, P < 0.001), delayed eruption (OR = 23.12; 95% CIs = 7.09-75.40, P < 0.001), and opacity (n = 12.19, 95% CIs = 1.44-103.28, P = 0.022) than the NMG. CONCLUSION: Children with microcephaly present with greater delays in chronology, alterations in eruption sequence, and enamel defects than normoreactive children.

Introdução: Lesões dentárias por trauma constituem experiências angustiantes em crianças, que promovem alterações tanto físicas quanto emocionais e psicológicas. Objetivo: Avaliar a prevalência de traumatismos dentários em pacientes com distúrbio neuropsicomotor e comparar a indivíduos normorreativos. Material e método: Avaliaram-se 120 indivíduos, sendo 60 com alterações neuropsicomotoras (grupo de estudo) e 60 normorreativos (grupo controle), de ambos os sexos, de 2 a 15 anos de idade, assistidos na Fundação de Apoio ao Deficiente do Governo do Estado da Paraíba. Os dados foram coletados por meio de questionário estruturado e exame clínico. Foi realizada análise descritiva e inferencial (teste t-student; teste Exato de Fisher), adotando-se um nível de significância de 5%. Resultado: A prevalência de traumatismos dentários observada em pacientes com distúrbio neuropsicomotor foi de 20,0%, enquanto que no grupo controle foi de 16,6% (p>0,05); no grupo controle a ocorrência foi maior no sexo masculino. Os grupos diferiram quanto ao tipo de atividade no momento do trauma (p<0,05) em relação à etiologia (p<0,05) e em relação ao local de ocorrência (p<0,05). Para ambos os grupos, os dentes mais afetados foram os incisivos centrais superiores. As fraturas de esmalte, seguidas pelas de esmalte e dentina sem exposição pulpar foram as lesões mais comuns nos dois grupos. Conclusão: A prevalência de traumatismos dentários em indivíduos com alteração neuropsicomotora é similar à de indivíduos normorreativos, com maior ocorrência no sexo feminino, em fase anterior à adolescência, durante atividades de rotina.


Introduction: Traumatic dental injuries are distressing experiences in children, which promote both physical, emotional and psychological changes. Objective: To evaluate the prevalence of dental trauma in patients with neuropsychomotor disorder and to compare to normoreactive individuals. Material and method: 120 individuals, 60 neuropsychomotor changes (study group) and 60 normorreatives (control group), of both sexes, from 2 to 15 years old, assisted in the Foundation of Support to the Disabled of the Government of the State of Paraíba. Data were collected through a structured questionnaire and clinical examination. Descriptive and inferential analysis (t-student test; Fisher's exact test) was performed, adopting a significance level of 5%. Result: The prevalence of dental trauma observed in patients with neuropsychomotor disorder was 20.0%, whereas in the control group it was 16.6% (p>0.05); in the control group the occurrence was higher in males. The groups differed according to the type of activity at the moment of the trauma (p<0.05) in relation to the etiology (p<0.05), and in relation to the place of occurrence (p<0.05). For both groups, the most affected teeth were the maxillary central incisors. Enamel fractures, followed by enamel and dentin fractures without pulp exposure were the most common lesions in both groups. Conclusion: The prevalence of dental trauma in individuals with neuropsychomotor alterations is similar to that of normorreative individuals, with a higher occurrence in females, in preteen phase, during routine activities.

ABSTRACT

Objetivo: Avaliar as manifestações bucais e o uso de serviços odontológicos de indivíduos com Doença Renal Crônica (DRC) em tratamento hemodialítico. Materiais e Métodos: Participaram doestudo 130 indivíduos, de ambos os sexos e com idade superior a 18 anos em atendimento no Serviço de Hemodiálise de referência da cidade de João Pessoa-PB. Foram coletados dados socioeconômicos,saúde geral, higiene bucal, autopercepção dos problemas bucais e acesso ao serviço odontológico. Um único examinador realizou o exame clínico intrabucal. Resultados: A maioria dos participantes era do sexo masculino (60,8%), com baixos níveis de escolaridade (56,9%) e renda(61,5%). Houve o predomínio da hipertensão arterial sistêmica (86,2%) na doença de base. Em relação à autopercepção dos problemas bucais, os participantes relataram com maior frequência axerostomia (51,5%), seguida de mau hálito (30,0%). Dentre as manifestações bucais, a gengivite e a presença de cálculo dentário foram as mais prevalentes, com 66,2% e 56,2%, respectivamente.A maioria dos indivíduos buscava atendimento odontológico na rede pública de saúde e afirmava que o cirurgião-dentista não estava preparado para atender e explicar o tratamento que seria realizado. Conclusão: As principais alterações bucais identificadas foram xerostomia, mau hálito,cálculo dentário e gengivite, e que a maioria dos indivíduos tinha dificuldade no acesso ao serviço odontológico público de saúde.


Objective: To assess oral manifestations and use of dental care assistance by individuals with Chronic Kidney Disease (CKD) in hemodialysis treatment. Materials and Methods: Participated inthe study 130 individuals of both genders and aged over 18 years old attending the Hemodilalysis Reference Service in João Pessoa - PB. Data collection for socioeconomic status, oral hygiene, self--awareness of oral problems and access to dental care was carried out. A single examiner ran a clinical intraoral exam. Results: Most participants were male (60.8%) with low levels of education(56.9%) and income (61.5%). There was a predominance of systemic arterial hypertension (86.2%)as the underlying disease. In relation to the self-awareness of oral problems, participants reporteddry mouth most frequently (51.5%) followed by bad breath (30.0%). Among oral manifestations, gingivitis and the presence of tartar were the most prevalent, 66.2% and 56.2%, respectively. Mostindividuals sought dental care in the public health system and stated that the dental surgeon wasnot prepared to attend and explain the treatment to be provided. Conclusion: The main oral alterationsidentified were dry mouth, bad breath, tartar as well as gingivitis, and that most participantsfound it difficult to have access to dental care assistance in the public health system.

Este trabalho objetivou analisar a opinião dos profissionais da UTI (Unidade de Terapia Intensiva) (médicos, enfermeiros, fisioterapeutas e fonoaudiólogos) sobre a inclusão do cirurgião dentista na equipe, avaliar o conhecimento destes profissionais sobre a associação da condição oral com a PAV (Pneumonia Associada à Ventilação Mecânica) e analisar a existência de protocolos de higiene oral para o paciente entubado na UTI do Hospital Regional do Cariri-CE (HRC). Foi realizado um estudo transversal, no qual foi aplicado um questionário para os profissionais de saúde que atuavam na UTI do mesmo Hospital. Sobre o conhecimento dos profissionais a respeito do projeto de lei 2.766/2008, constata-se que aproximadamente 40% dos pesquisados desconheciam a lei. Entretanto, ao se questionar sobre a PAV, 95,7% dos entrevistados conhecem as causas de ocorrência do evento e 87% concordam que a inclusão do cirurgião dentista (CD) beneficiaria a equipe que atua na UTI. Quando questionados sobre a higienização bucal dos pacientes durante a permanência dos mesmos na UTI, todos os entrevistados responderam que indicam o uso de Clorexidina a 0,12%. Ao se questionar qual a função do CD na equipe, 95,7% assinalaram prevenção e controle dos eventos da microbiota bucal; 82,6% atendimento de emergência, tratamento de traumatismo e 78,3% limpeza bucal. Conclui-se a presença do CD é importante na UTI para reduzir os riscos da PAV e realizar procedimentos específicos em sua área (AU).


This study aimed to analyze the opinion of professionals from the Intensive Care Unit (doctors, nurses, physiotherapists, and speech therapists) about the inclusion of dentists on staff; assess the knowledge of these professionals about the association of oral status with PAV (pneumonia associated with mechanical ventilation); and analyze the existence of protocols of oral hygiene for patients intubated in the ICU of the Hospital Regional Cariri (HRC). A cross-sectional study was conducted using questionnaire answered by health professionals who worked in the ICU at HRC. About the knowledge of professionals in relation to the bill 2766/2008, 40% of respondents were unaware of the law. However, when questioned about the PAV, 95.7% of respondents know the causes of occurrence of the event and 87% agreed that the inclusion of the dentist (CD) would benefit the team working in the ICU. When asked about the oral hygiene of patients during their permanence in the ICU, all respondents indicated the use of chlorhexidine 0.12%. When questioned about the function of the CD on the staff of the ICU, 95.7% indicated prevention and control of the events of the oral microbiota; 82.6% emergency care, treatment of trauma; and 78.3% mouthwash. It was concluded that the presence of CD in the ICU is important to reduce the risk of VAP and perform specific procedures in your area (AU).

Alterações no fluxo e na composição da saliva têm sido descritas em indivíduos com paralisia cerebral (PC), e estas podem ser consideradas de risco para o desenvolvimento de doenças bucais. O objetivo deste estudo foi investigar a influência das alterações de osmolaridade e fluxo salivar na experiência da cárie dentária em indivíduos com PC. A amostra era composta por 63 crianças com PC (GE) e 50 crianças normorreativas (GC), de ambos os sexos, na faixa etária de 5 a 16 anos de idade. Para o GE foram coletados dos prontuários médicos … desordem do movimento, o tipo clínico da PC, uso de medicações e nível do Gross Motor Funnction Classification System (GMFCS). Ambos os grupos foram avaliados quanto … experiência de cárie, sendo registrado o número total de dentes cariados, perdidos e obturados para a dentição dec¡dua e permanente (ceo-d e CPO-D). A saliva de repouso foi coletada para a determinação do fluxo e da osmolaridade salivar com o emprego do Salivette© (Sarstedt, Nmbrecht, Alemanha). Os grupos GE e GC diferiram significantemente para as variáveis CPOD (p<0,001), osmolaridade (p<0,001) e fluxo salivar (p<0,001), apresentando o GE maiores valores de CPOD e osmolaridade e menores de fluxo salivar quando comparado a GC. A severidade do dano neurológico representado pelo padrão clínico tetraparesia, a falta de controle cervical e o uso de anticonvulsivante são fatores que influenciam negativamente nos parâmetros salivares e experiência de cárie e indivíduos com PC


Changes in the flow and composition of saliva have been described in patients with cerebral palsy (CP), what can be considered as a risk factor that may result on the development of oral diseases. The aim of this study was to investigate the influence of changes in osmolarity and salivary flow in dental caries experience in individuals with CP. The sample consisted of 63 children with CP (GE) and 50 normoreactive children (GC) of both sexes, aged 5-16 years old. For GE group information regarding movement disorder, the clinical type of PC, use of medications, and level of Function Gross Motor Classification System (GMFCS) were obtained through medical records. Both groups were assessed for caries experience, and were recorded the total number of decayed and lost teeth for deciduous and permanent dentition (dmft and DMFT). To determine the salivary flow and osmolality the resting saliva was collected using Salivette© (Sarstedt, Nmbrecht, Germany). The EG and CG have differed significantly for variables DMFT (p <0.001), osmolality (p <0.001), and salivary flow rate (p <0.001). GE has presented higher values of DMFT and osmolarity and inferior salivary flow when compared to GC. The severity of neurological injury represented by standard clinical tetraplegia, lack of neck control, and anticonvulsants are factors that influence negatively salivary parameters and caries in individuals with CP

ABSTRACT

Objetivo: Avaliar clinicamente a condição odontológica de pacientes internados em Unidade de Terapia Intensiva (UTI). Método: Foi realizado estudo exploratório, descritivo e transversal com avaliação clínica odontológica de 50 pacientes internados em UTI do Hospital São Vicente de Paula (João Pessoa/PB). Foram coletados os dados dos prontuários médicos relativos à idade, sexo, fator causal da internação e comorbidades relacionadas a problemas pulmonares. A análise da condição bucal foi qualificada em três categorias: boa (condição aceitável de higiene sem sangramento ao toque ou focos de infecção aparentes); regular (paciente apresentava pouco sangramento ao toque e/ou candidíase observada clinicamente como placas esbranquiçadas sobre mucosa inalterada ou avermelhada, removidas mecanicamente); e ruim (presença de abscessos e candidíase disseminada incluindo orofaringe). Resultados: A idade média dos examinados era de 53,8 (±18,5) sendo 23 (46%) do sexo masculino. As causas das internações de 24 (48,0%) pacientes foram neurológicas e de 26 (52%) por problemas renais, cardíacos, metabólicos, respiratórios e septicemia. Foram registrados quadros de pneumonia em 56% dos pacientes. Quanto à distribuição das condições de saúde bucal, observaram-se porcentagens significativamente maiores (p menor que 0,001) da condição de higiene ruim, comparadas às outras condições.Conclusões: A maioria dos pacientes internados em Unidade de Terapia Intensiva apresentava condição odontológica ruim, e a candidíase foi a infecção mais prevalente.


Objective: To evaluate clinically the oral conditions of the patients admitted to an Intensive Care Unit (ICU). Method: An exploratory, descriptive and transversal study was developed with clinical oral evaluation of 50 patients admitted to the ICU of the São Vicente de Paula Hospital (João Pessoa, PB, Brazil). The data were collected from the medical files concerning age, gender, cause of admission and comorbidities relative to lung problems. The oral condition was classified into three categories: good (acceptable hygiene, no gingival bleeding to touch or visible infection foci); regular (little gingival bleeding to touch and/or clinically visible candidiasis as white plaques on healthy or reddish mucosa, removed mechanically) and poor (presence of abscesses and widespread candidiasis, including the oropharynx). Results: The mean age of the patients was 53.8 (±18.5) years, 23 (46%) of them being male. Twenty-four (48%) patients were hospitalized due to neurological causes and 26 (52%) due to kidney, cardiac, metabolic or respiratory problems and septicemia. Pneumonia was present in 56% of the patients. As for the oral health conditions, there were significantly higher percentages (p less than 0.001) of poor conditions compared with the other conditions. Conclusion: Most patients admitted to the ICU presented poor oral condition and candidiasis was the prevalent infection.
