ABSTRACT

El monóxido de carbono es conocido como " el asesino silencioso" por tratarse de un gas incoloro e inodoro. De acuerdo con estas características, su toxicidad pasa desapercibida, lo cual dificulta arribar al diagnóstico. En la mayoría de los casos, las épocas frías y la intoxicación grupal hacen sospechar su presencia, debido a que se utilizan formas inapropiadas de calefacción tanto en el ámbito domiciliario como público. Nuestro objetivo es dar a conocer una intoxicación masiva con monóxido de carbono en un salón de fiestas infantiles en el que se utilizó una fuente de combustión instalada, no con el fin de calefaccionar, sino como abastecimiento de luz (grupo electrógeno). Se recalca que también puede ocurrir en cualquier época del año.

La "jarra loca" es una mezcla de bebidas alcohólicas y medicamentos que utilizan los jóvenes como modalidad de abuso. En los últimos años, además de psicofármacos, los jóvenes mezclan hipoglucemiantes, lo cual puede generar secuelas neurológicas o producir la muerte, dependiendo de la dosis ingerida y el tiempo de duración de la hipoglucemia. Objetivo. Estimar la prevalencia de consumo de alcohol y medicamentos en adolescentes de 12 a 18 años de edad que concurren a cuatro escuelas del partido de Tres de Febrero, de la provincia de Buenos Aires. Material y métodos. Estudio observacional, descriptivo y transversal. Se realizó una encuesta autoadministrada sobre consumo de alcohol y medicamentos en adolescentes de 12 a 18 años, de cuatro escuelas de la provincia de Buenos Aires. Se realizaron talleres para intercambio de conocimientos y experiencias con los alumnos y se entregó material gráfico informativo. Resultados. Se realizaron 977 encuestas. El 49% de los alumnos encuestados refirió consumir bebidas alcohólicas al salir con sus amigos. El 8% (n= 78) de los alumnos refirieron consumir "jarra loca" y el 49% refirieron que sus amigos lo hacían. Veintiún alumnos requirieron atención médica, al menos, en una oportunidad luego de haber tomado bebidas alcohólicas y 17 de estos habían consumido "jarra loca". El 55% de los alumnos desconocían el uso de las pastillas que mezclan en la "jarra loca". El 48% de los padres desaprueba el consumo de alcohol. Conclusión. El 49% refirió tomar bebidas alcohólicas. En nuestro trabajo, el 8% de los alumnos encuestados consumieron o probaron alguna vez "jarra loca", aunque el 49% afirmaron que sus amigos sí lo hacían.(AU)

La mariposa nocturna Hylesia nigricans es responsable de brotes epidémicos en nuestro país. Se denomina erucismo al contacto con las larvas u orugas y lepidopterismo, si el contacto se produce con las mariposas. Realizamos una actualización de estos lepidópteros de importancia médica

ABSTRACT

The iron is the main component of hemoglobin and is also part of myoglobin and enzymes. Its deficit is the most common cause of nutritional anemia in humans. The use of iron salts is very common in children, because they are indicated for prophylaxis and treatment of iron deficiency anemia. The availability in households and the emergence of flavored formulations that promote adherence to treatment, greatly assist in this type of poisoning. Usually, the overdose is considered a low hazard, because it is a mineral supplement added in many of the baby food. Iron in free state is able of producing toxicity, disrupting multiple cellular processes by catalyzing redox reactions with lipid peroxidation and free radical formation. We report a case of serious toxicity by iron salts, in which early intervention and management with specific chelator, allowed a favorable evolution.

ABSTRACT

El hierro es el componente principal de la hemoglobina y forma también parte de la mioglobina y enzimas. Su déficit es la causa más común de anemia nutricional en el hombre. El uso de sales de hierro en pediatría es muy frecuente, ya que se indican como profilaxis y tratamiento de la anemia ferropénica.La disponibilidad en los hogares y la aparición de formulaciones saborizadas que favorecen la adhesión terapéutica, colaboran en gran medida en este tipo de intoxicación. Habitualmente, la sobredosis se considera de baja peligrosidad, por tratarse de un suplemento mineral adicionado en muchos de los alimentos infantiles. El hierro en estado libre es capaz de producir toxicidad y alterar múltiples procesos celulares al catalizar reacciones rédox con peroxidación de lípidos y formación de radicales libres. Presentamos un caso clínico de intoxicación grave, donde la intervención precoz y la administración del quelante específico permitieron una evolución favorable.(AU)

El uso de sales de hierro en pediatría es muy frecuente, ya que se indican como profilaxis y tratamiento de la anemia ferropénica.La disponibilidad en los hogares y la aparición de formulaciones saborizadas que favorecen la adhesión terapéutica, colaboran en gran medida en este tipo de intoxicación. Habitualmente, la sobredosis se considera de baja peligrosidad, por tratarse de un suplemento mineral adicionado en muchos de los alimentos infantiles. El hierro en estado libre es capaz de producir toxicidad y alterar múltiples procesos celulares al catalizar reacciones rédox con peroxidación de lípidos y formación de radicales libres. Presentamos un caso clínico de intoxicación grave, donde la intervención precoz y la administración del quelante específico permitieron una evolución favorable.

ABSTRACT

"Jarra loca" is a mixture of alcohol and prescription drugs. Sometimes, these are added to alcohol without the knowledge of the user. In recent years, adolescents mixed psychotropic drugs and hypoglycemic agents to reach the drowsiness and anxiety of hypoglycemia. We present five patients, one adolescent and four young adults, from Buenos Aires and the metropolitan area, followed up in the National Poisoning Center. They consumed this mixture with serious toxic manifestations and a high mortality rate.

ABSTRACT

The Euphorbiaceae family are plants that have in common latex production. There are over 8000 species. They have worldwide distribution. We can find trees, shrubs or herbs. Many important species belong to this family and they are used in our country for industrial and medical purposes. There are also ornamental plants with attractive appearance that stimulate children curiosity, intake or manipulation. In the National Poison Center consultations for plants represent a small number (0.2%) of the total accidents in children, however they can be serious. We report the case of a 4 years old girl who manipulated and swallowed part of an Euphorbiaceae family plant (Synadenium grantii).

Las euforbiáceas son una familia de plantas que tienen en común la producción de látex. Existen más de 8000 especies.Tienen amplia distribución mundial, y adoptan la forma de árboles, arbustos o hierbas. Pertenecen a esta familia numerosas especies de importancia y, en nuestro país, se emplean con fines industriales y medicinales. También existen plantas ornamentales, cuya atractiva apariencia estimula la curiosidad de los niños, quienes pueden ingerirlas o manipularlas.En el Centro Nacional de Intoxicaciones, las consultas por plantas representan un escaso número (0,2 por ciento) dentro del total de accidentes en pediatría; sin embargo, pueden revestir gravedad.Presentamos el caso de una niña de 4 años, quien manipuló e ingirió parte de una planta de la familia Euforbiáceas(Synadenium grantii).

La ¶jarra loca÷ es una mezcla de bebidas alcohólicas y medicamentos. En algunas ocasiones, los adolescentes desconocen los riesgos de consumirla. En los últimos años, los jóvenes mezclan psicofármacos e hipoglucemiantes para lograr el efecto de obnubilación y ansiedad de la hipoglucemia. Presentamos cinco pacientes, un adolescente y cuatro adultos jóvenes, provenientes de la Ciudad de Buenos Aires y del conurbano, seguidos telefónicamente en el Centro Nacional de Intoxicaciones (CNI), que consumieron esta mezcla con consecuencias clínicas graves y alta mortalidad.(AU)
