Por ello realizamos una valoracion de suó distintas indicaciones médicas y paramédicas con el fin de intentar acercarnos a tomar las decisiones que supongan un mayor beneficio materno-fetal. In the past 20 years we are attending to a continuous increase in the rate of caesarean section. Because of this we try to analize the different clinical and non clinical indications for caesarean section in order to decide the best for mother and fetus. La cesárea de Misgav Ladach, descrita por Stark, y que debe su denominación al hospital en el que ha sido desarrollada, aporta varias novedades (incisión de Joel-Cohen modificada, cierre uterino con una sola sutura y ausencia de peritonización) y sigue los principios de la cirugía mínimamente agresiva, eliminando, por innecesarios, algunos de los pasos quirúrgicos de la cesárea convencional. La revisión bibliográfica constata las numerosas ventajas del procedimiento. Aporta rapidez, sencillez, seguridad y ahorro económico. También reduce el sangrado intraoperatorio, la morbilidad postoperatoria y el riesgo de adherencias, haciendo factible una recuperación más rápida. Sin embargo, en los últimos años, diferentes autores han propuesto una serie de modificaciones, que, sin reducir su eficacia, pueden añadir ventajas adicionales. En este sentido, se presenta la técnica que, después de una experiencia de cuatro años, se emplea en la actualidad en el Hospital Universitario La Paz, de Madrid
The Misgav Ladach cesarean section was described by Stark. Its name is due to the hospital where this new technique was initiated and applied. This method contributes with some developments as the Joel-Cohen modified incision, one single suture in the histerorrafy and lack of close of the peritoneum. Besides, this procedure follows the principles of the minimaly aggressive surgery avoiding several inherent setps of conventional cesarean section. Bibliographic review verifies the multiple advantages of this technique, offering us quickness, simplicity, safety and economic saving. Moreover, it reduces the intrasurgical bleeding, the postoperatory morbidity and the risk of adherences, helping the patient in her best and faster recovery. However, in the last years, different authors have introduced some modifications to this procedure, adding advantages with the same effectiveness. Breech delivery is one of the principal problems in the actual gynaccology. Nowadays gynaecologists tend to practice caesarean section in breech presentations. In this article we try to review delivery ways that present lower mother and perinatal morbidity and mortality in preterm as well as term pregnancies and single or multiple pregnancies. Moreover we try to study the utility of external podalic version and underline the importance of obstetric experience when deciding the way of delivery
