ABSTRACT

OBJECTIVES: To compare the effect of 5 desensitizing agents on the hydraulic conductance (Lp) of dentin and to analyze its surface under a Laser Scanning Confocal Microscope (LSCM) before and after an erosive challenge. METHODS: Lp was analyzed in the following sequence: in the presence of smear layer (pMin), after a 15-second acid etching (pMax), after treatment with a desensitizing agent (pTreat) and after a 1-minute erosive challenge (6% citric acid - pEro). Fifty 1.0 ± 0.2 mm-thick dentin disks were prepared from sound human third molars and were randomly distributed into 5 groups (n = 10): FG-Fluoride gel (control), SA-Sensiactive, PR-Sensitive Pro-Relief, NP-Desensibilize Nano-P and EV-Enamel Pro Varnish. Data were analyzed by two-way ANOVA and Tukey tests (α<0.05). Additional specimens were analyzed under a Laser Scanning Confocal Microscope (LSCM) and by Energy Dispersive X-ray Spectroscopy (EDS). RESULTS: All materials reduced the Lp in some extent, except for FG and EV. After the erosive challenge, SA was the only material effectively able to keep the same Lp of the post-treatment phase, while the other products showed lower resistance upon an erosive challenge. Under the LSCM, the SA and NP materials were more effective to obliterate the opened dentin tubules and demonstrated higher resistance upon an erosive challenge. The EDS analysis evidenced levels of Ca, O, P, Si, Na and S. CONCLUSIONS: Product containing potassium oxalate was the most effective in reducing Lp of dentin samples before and after an erosive challenge. Under a LSCM, products containing potassium oxalate or hydroxyapatite crystals seemed to be occluding the dentin tubules after an erosive challenge.

METHODS: A total of 102 volunteer patients (68 female) aged 19 to 86 years, who sought medical care in health clinics of the university and were evaluated in the period from September to December 2013. The subjects were examined according to the Research Diagnostic Criteria for Temporomandibular Disorders, using a mechanical algometer (Palpeter®) with standardized pressure of 0.5 and 1.0kg, and Medical Outcomes Study 36-Item Short Form Health Survey (SF-36) questionnaire, to assess quality of life. The data were tabulated for statistical analysis and the variables were correlated with the clinical findings of the temporomandibular disorders and quality of life. RESULTS: Fifty percent of patients were positive for temporomandibular disorders and 39.2% classified as myofascial pain group. The temporomandibular disorder group was significantly associated with uncomfortable bite (p=0.0000), temporomandibular joint clicking (p=0.0001) and tooth clenching (p=0.0001). The Mann Whitney test used to analyze the SF-36 revealed that the domains of pain (mean score of 47.80%; p<0.0001) and mental health (62.67%; p<0.05) were strongly associated with temporomandibular disorders. CONCLUSION: The quality of life of individuals with temporomandibular disorders was negatively affected by the presence of pain and mental health disorders.

RESUMO Objetivo Determinar o impacto das disfunções temporomandibulares na qualidade de vida. Métodos Foram incluídos 102 pacientes voluntários (68 mulheres) com idades entre 19 e 86 anos, que buscaram atendimento médico nas clínicas de saúde da universidade e foram avaliados no período de setembro a dezembro de 2013. Os pacientes foram examinados segundo os Critérios Diagnósticos para Pesquisa em Disfunções Temporomandibulares, usando um algômetro mecânico (Palpeter®) com pressão padronizada de 0,5 e 1,0kg e o questionário Medical Outcomes Study 36-Item Short Form Health Survey (SF-36), para avaliação da qualidade de vida. Os dados foram tabulados para análise estatística, e as variáveis foram correlacionadas com os achados clínicos das disfunções temporomandibulares e da qualidade de vida. Resultados Metade dos pacientes foi positiva para disfunções temporomandibulares, sendo 39,2% deles classificados como grupo de dor miofascial. O grupo de disfunções temporomandibulares foi significativamente associado ao desconforto ao morder (p=0,0000), à crepitação da articulação temporomandibular (p=0,0001) e ao apertar dos dentes (p=0,0001). O teste de Mann-Whitney usado para analisar o SF-36 revelou que os domínios da dor (pontuação média de 47,80%; p<0,0001) e saúde mental (62,67%; p<0,05) estavam fortemente associados às disfunções temporomandibulares. Conclusão A qualidade de vida de indivíduos com disfunções temporomandibulares foi afetada negativamente pela presença da dor e de transtornos de saúde mental.

ABSTRACT

PURPOSE: To evaluate the effect of aqueous solutions of chlorhexidine digluconate (CHX) in different concentrations on bond strength to eroded dentin up to 6 months, using normal dentin as a control. METHODS: Exposed flat dentin of extracted third molars was only ground with 600-grit SiC paper/1 minute (normal dentin - N), or subsequently eroded by a regular-cola soft-drink (eroded dentin - E). N and E were acid-etched, washed, dried and rehydrated with 1.5 µL, respectively, of distillated water (control - NC / EC); of 0.004% CHX (N0.004% / E0.004%); or of 2% CHX (N2% / E2%). Adper Single Bond 2 was applied in all specimens and resin composite buildups were constructed with Filtek Z350. Specimens were sectioned in beams, which were tested (µTBS) immediately or after 6 months of aging. RESULTS: Microtensile bond strength to eroded dentin was always significantly lower than that to normal dentin. Application of tested CHX solutions did not exert a significant effect immediately; however, after aging, the 2% CHX prevented abrupt bond strength loss both to eroded and normal dentin.

ABSTRACT

PURPOSE: To assess the long-term effect of a 2% aqueous chlorhexidine (CHX) solution on bond strength to artificially eroded dentin compared to sound dentin. MATERIALS AND METHODS: Flat mid-coronal dentin surfaces of extracted third molars (n = 28) were subjected only to grinding with a 600-grit SiC paper for 1 min (sound dentin S, n = 14) or additionally to erosive pH cycling with a cola-based soft-drink (eroded dentin E, n = 14). After acid etching, rinsing, and air drying, S and E were rehydrated with 1.5 µl of 2% CHX (S2%, n = 7; E2%, n = 7) or of distilled water (control SC, n = 7; EC, n = 7). Composite buildups were incrementally constructed with Filtek Z350 following Adper Single Bond 2 application. Specimens were sectioned into beams, which were subjected to microtensile testing immediately or after 6 or 12 months of aging. Fractured surfaces were observed under a digital microscope (50X magnification). Microtensile bond strength (µTBS) (MPa) was analyzed by three-way ANOVA and Tukey's tests (α = 0.05) and failure mode by the Kruskal-Wallis test (α = 0.05). RESULTS: Compared to sound dentin, eroded dentin was consistently related to lower µTBS. Immediately and after 12-month aging, the effect of CHX was insignificant, but it was significant after 6-month aging, when it conserved the bond strength to both eroded and sound dentin. The percentage of adhesive and mixed failures were equivalent, and significantly more frequent than cohesive failures, whether in dentin or in composite. CONCLUSION: The 2% CHX effect on bond strength conservation to both eroded and sound dentin was not found to be persistent.

ABSTRACT

PURPOSE: To evaluate the effect of bleaching techniques on dentin permeability. The hypothesis was that the bleaching agent associated or not to etching and/or energy source would be able to increase dentin permeability (conductance). METHODS: Fifty 1 mm-thick disks of mid-coronal dentin were obtained from human third molars, which were mounted in a filtration chamber, allowing exposure of a standardized area of 0.282 mm2. All specimens were treated with 35% liquid phosphoric acid for 15 seconds to maximize dentin permeability. Then they were randomly allocated to five different conditions: C: placebo gel (negative control); HP: Whiteness HP Maxx (WM)-35% hydrogen peroxide; PA-HP: WM preceded by phosphoric acid etching for 15 seconds. HP-E: WM associated to a light source and PA-HP-E: WM preceded by phosphoric acid etching for 15 seconds and associated to a light source. The bleaching agent was applied for 10 minutes with intermediate agitation at 5 minutes. In the groups associated with a light source, it was activated for 30 seconds every 5 minutes. Afterwards, the permeability (Lp) was measured using the Flodec device. The data were collected and analyzed with one-way ANOVA and Tukey's test (P< 0.05). RESULTS: Mean and standard deviation of Lp were: C = 1.92 (1.04); HP = 1.14 (0.29); PA-HP = 14.40 (8.62); HP-E = 4.18 (5.14); PA-HP-E = 27.32 (13.24). Data revealed that phosphoric acid etching could increase the dentin permeability, while the bleaching agent or light curing alone did not.

ABSTRACT

Paciente do sexo feminino, 25 anos de idade, procurou tratamento estético queixando-se do escurecimento do dente 21. Foram realizados exames clínicos e readiográficos, comprovando a alteração de cor e o tratamento endodôntico satisfatório. O plano de tratamento foi proposto para estabelkecer a função e a estética do dente 21 (técnica mediata), associado ao clareamento externo, em consultório, dos dentes #15 a #25 e #35 a #45 co peróxido de hidrogênio a 35 por cento, ativado com luz híbrida, a fim de que fossem uniformizadas as tonalidades de cor dos dentes. Posteriormente, um pino de fibra de vidro foi fixado no canal radicular, com cimento de ionômero de vidro convencional, seguido pela restauração da abertura coronária com resina composta. As etapas do tratamento revelaram que o planejamento correto combinado com o conhecimento das técnicas disponíveis e das propriedades dos materiais são essenciais para a obtenção da excelência estética e funcional dos dentes em questão.


A 25-year-old woman was seeking for aesthetic treatment for her #21 tooth. Clinical and radiographic examinations were done confirming color change and satisfactory endodontic treatment. Thus, a treatment was proposed to reestablish function and aesthetics to the compromised teeth. The #21 tooth received internal bleaching and #15 to #25 and #35 to #45 teeth received external bleaching with a 35% hydrogen peroxide gel activated by hybrid light. This was performed in order to standardize the tonalities of the color of the teeth. Subsequently, a glass-fiber post was fixed inside the radicular conduct with conventional glass-ionomer cement and the endodontic coronal aperture was restored with composite resin. Treatment steps showed that the correct planning associated with the knowledge of available techniques and material properties are essential for reaching functional and aesthetic excellence.

A proposta deste trabalho foi analisar qualitativa e semiquantitativamente, em M.E.V. - Técnica Convencional, por Retroespalhamento de Elétrons (BSE) e por Espectroscopia de Dispersão de Raios-X (E.D.S.) - a microestrutura dentinária de dentes em diferentes condições clínicas. Formaram-se 7 grupos, com 5 dentes cada grupo: I- inclusos jovens (IJ); II- hígidos jovens (HJ); III- hígidos adultos (HA); IV- com lesão cariosa oclusal (LCO); V- com lesão cariosa cervical (LCC); VI- com exposição radicular (ER) e VII- com lesão cervical não cariosa (LCNC). Após as extrações, os dentes foram imediatamente fixados em Karnovsky. As porções radiculares e o esmalte coronário foram seccionados, preservando-se o terço cervical dos dentes. Os espécimes dentários foram divididos em duas metades, ambas contendo as superfícies vestibulares quanto as linguais, totalizando 70 espécimes. Cuidado especial foi tomado nos grupos com LCO, LCC e LCNC, garantindo que as lesões fizessem parte de suas duas secções. A análise morfológica foi realizada sobre área de dentina fraturada, em três regiões: próximo à superfície externa, no terço médio e próximo à polpa. Os espécimes foram desidratados em concentração ascendente de etanol e secados em ponto crítico. Metade dos espécimes foi metalizada com ouro, para análise em M.E.V., e a metade homóloga com carbono, para observação em E.D.S. e BSE. Os grupos IJ e HJ não mostraram alterações morfológicas, tanto na lingual quanto na vestibular. As dentinas peritubular e intertubular e os prolongamentos odontoblásticos (POs) estavam evidentes por toda a extensão dentinária, assim como fibrilas de colágeno margeando toda a superfície interna dos túbulos dentinários (TDs), muitas vezes lembrando a lâmina limitans (LL). Os grupos LCC e LCO mostraram grande quantidade de cristais e estruturas tubuliformes (ETfs) na dentina abaixo das lesões, além das demais estruturas vistas no grupos anteriores. The purpose of this study was to analyze, qualitatively and semiquantitavely, with SEM, BSE and EDS, the dentin microstructure in teeth with different clinical conditions. The teeth were divided in 7 groups (n=5 each one): I- Young non erupted teeth (YNET), II- Young sound teeth (YST), III- Old sound teeth (OST), IV- Occlusal carious lesion (OCL), V- Cervical carious lesion (CCL), VI- Root exposure (RE) and VII- Non carious cervical lesion (NCCL). After extraction the teeth were immediately fixed in Karnovsky. Crows and root were cut off in such a way to preserve the cervical third for the purposes of this investigation. The cervical specimens were fractured in two halves, both containing the lingual and buccal cervical surfaces, totaling 70 specimens. Special care was taken with OCL, CCL and NCCL groups during the fracture to ensure that both fragments enclosed part of their respective lesions. The morphological analysis was performed on the fractured dentin surfaces of both fragments, in three regions: near the external surface, on the middle third and near the pulp chamber. The specimens were dehydrated in ascending series of ethanol concentrations and critical point dried. Half of the specimens were gold sputtered for SEM analysis and the homologous half carbon coated for BSE and EDS observation. The YNET and YST groups showed no morphological changes in both buccal and lingual surfaces. The PD and ID and the OPs were evident throughout the dentin extension. The collagen fibrils were observed layering the walls of dentin tubules in their entire length, resembling the LL. The CCL and OCL groups showed a great amount of Crs and TSs, together with the other same features seen in the previous groups. The lingual surface of the CCL was distinct from the buccal surface and the OPs were limited to the third near the pulp on both surfaces. The RE group did not reveal intratubular structures (ITS). ABSTRACT

El recubrimiento pulpar directo e indirecto, desde el punto de vista biológico y clínico, representa importantes mecanismos para el mantenimiento de la vitalidad pulpar; sin embargo aún es considerado un tema controversial en la clínica odontológica, debido a la complejidad del diagnóstico y a la delicada conducta terapéutica necesaria para obtener éxito en el tratamiento clínico. En tal sentido, este artículo realizará un abordaje biológico y clínico del Recubrimiento Pulpar Directo e Indirecto con la intención de resaltar la importancia fundamental del diagnóstico clínico y radiográfico de la condición pulpar, enfatizando las indicaciones y contra indicaciones de ambos tratamientos. Adicionalmente serán materia de discusión los materiales comúnmente utilizados para la protección del complejo dentinopulpar y las nuevas perspectivas en esta área


Direct and indirect pulp capping represents important mechanisms, from the point of clinical and biological aspects to maintain the pulp vitality. However, it still a controversy issue at dental field, once the complexity diagnosis and the accurately therapeutics are need to achieve the success of clinical treatment. Seeing that, this article outlines the broad of the biological and clinical approach of the direct and indirect pulp capping. Emphasizing fundamentally the importance of the corrected diagnose of clinical and radiographic pulp condition, further the indications and contra-indications of both treatments. Furthermore, it will be discussed regarding the classical dental materials used to dentin-pulp protection and contemporary perspectives in this area

ABSTRACT

O avanço da Odontologia rumo à ciência foi iniciado por Black³ que, há mais de 100 anos, desenvolveu e classificou os preparos cavitários de forma compatível com as lesões cariosas e com os materiais restauradores existentes na época. Naquele período, o amálgama predominava como material restaurador, o que exigia um maior envolvimento da estrutura dentária sadia e a confecção de preparos cavitários retentivos para a adaptação desse material. Com o conhecimento do mecanismo de ação do flúor no processo de remineralização das lesões cariosas e o advento dos materiais adesivos, os preparos cavitários passaram a ser mais conservadores, preservando a estrutura dentária hígida. Contudo, esse novo conceito da Dentística Contemporânea ainda requer a obediência de uma série de princípios biológicos e mecânicos durante a confecção dos preparos cavitários, tornando-se necessário adequá-los às condições atuais

ABSTRACT

Objetivos: Avaliar a compatibilidade de união pela interação dos adesivos Single Bond (3M), Prime-Bond 2.1 (Dentsply) e Excite (Ivoclar/Vivadent) com as resinas compostas Z100 (3M), Esthet-X (Dentsply) e Tetric Ceram (Ivoclar/Vivadent), por meio do teste de resistência à tração. Métodos: Utilizou-se um conjunto de matrizes de aço inoxidável, composto de duas partes, sendo que cada parte apresentava um orifício central cônico com 8mm de diâmetro maior, 5mm de diâmetro menor e 1 mm de espessura. Uma das partes da matriz foi preenchida com uma das resinas compostas e fotoativada por 20s, aplicando-se uma camada de um dos sistemas adesivos na área de união e subseqüente fotoativação, por 20s. A segunda parte da matriz foi posicionada sobre a primeira inserindo o material de acordo com passos descritos para a primeira porção. Foram estabelecidos nove grupos, no total de 45 corpos de prova, sendo cinco espécimes para cada grupo experimental. Após uma hora, os corpos de prova foram submetidos ao teste de tração na máquina de ensaios Kratos. Resultados: Os resultados de união, em Kgf/cm2, entre a resina Esthet-X com os adesivos Single Bond, Prime-Bond 2.1 e Excite foram: 12.34, 12.00 e 9.80; da resina Tetric Ceram: 8.82, 10.91 e 8.05; da resina Z100, 11.44, 11.40 e 8.99, respectivamente. Conclusão: A análise estatística de variância a dois critérios não mostrou diferença significante nas interações dos adesivos simplificados com as diferentes resinas compostas, evidenciando que a compatibilidade química entre eles constitui uma alternativa viável na prática restauradora.
