ABSTRACT

OBJECTIVES: We aimed to analyse the clinical epidemiology and genetic diversity of invasive pneumococcal disease (IPD) episodes attributed to the Spain(23F)-ST81 (PMEN1) clone. METHODS: Fifty-eight (2.7%) of 2117 invasive pneumococci isolated from adult patients during the 1990-2012 period shared a PFGE pattern related to the PMEN1 clone. The genotype was confirmed by multilocus sequence typing. The pbp2x, pbp1a, pbp2b and pspA genes were PCR-amplified and sequenced. Polymorphisms in the pspC gene were identified by PCR restriction fragment length polymorphism. The presence of transposons with erythromycin and tetracycline resistance determinants was detected by PCR. RESULTS: The prevalence of the PMEN1 clone increased from 0.8% in 1991 to 6.2% in 2001, and decreased to 0% in 2010-12, concomitant with the introduction of the seven-valent pneumococcal conjugate vaccine for children. A total of 93.1% of patients had pneumonia, meningitis or peritonitis; 87.9% of patients had associated underlying diseases, mainly cancer, chronic obstructive pulmonary disease and diabetes. Two closely related sequence types (STs) (ST81, n = 52; ST85, n = 6) were detected, with different serotypes: 23F (n = 42), 19A (n = 9) and 19F (n = 6). All the isolates were resistant to penicillin, co-trimoxazole and chloramphenicol. All the isolates also shared the same pbp1a allele, whereas multiple alleles of pbp2b, pbp2x, pspA and pspC were detected. Of the isolates, 89.7% were tetracycline resistant and 60.3% (n = 35) were macrolide resistant, and resistance was associated with different Tn916-like transposons. CONCLUSIONS: Adult IPD caused by this clone was mainly detected in patients with underlying conditions, and genetic variability was observed among PMEN1 isolates collected in our area over the past 20 years.

ABSTRACT

Streptococcus pneumoniae plays an important role in causing acute exacerbations in patients with chronic respiratory disease. However, few data are available regarding pneumococcal persistence in adult patients with chronic respiratory diseases. Fifty pneumococci recovered from sputum samples (1995 to 2010) from 13 adult patients with ≥ 3 episodes of acute exacerbation or pneumonia, with the same serotype and pulsed-field gel electrophoresis (PFGE) pattern, were studied. Multilocus sequence typing (MLST) loci, penicillin-binding protein (PBP) genes (pbp2x, pbp1a, pbp2b), and the quinolone-resistant determining regions (QRDRs) of parC, parE, and gyrA were PCR amplified and sequenced. The average time between the first and last episode was 582 days (standard deviation [SD], ± 362). All but two patients received multiple courses of ß-lactam treatment, and all persistent strains were resistant to penicillin; however, the PBP sequences were stable over time apart from one variable nucleotide in pbp2x, observed among pneumococci isolated from three patients. In contrast, 7/11 patients treated with fluoroquinolones had fluoroquinolone-resistant pneumococci. In three patients, the initially fluoroquinolone-susceptible strain developed resistance after fluoroquinolone therapy, and in the remaining four patients, the persistent strain was fluoroquinolone resistant from the first episode. QRDR changes involved in fluoroquinolone resistance were frequently observed in persistent strains after fluoroquinolone treatment; however, the PBP sequences and MLST genotypes of these strains were stable over time.

ABSTRACT

During the winter period 2010/11 27 epidemiologically unlinked, confirmed cases of oseltamivir-resistant influenza A(H1N1)2009 virus infection have been detected in multiple, geographically dispersed settings. Three of these cases were in community settings, with no known exposure to oseltamivir. This suggests possible onward transmission of resistant strains and could be an indication of a possibility of changing epidemiology of oseltamivir-resistant influenza A(H1N1)2009 virus.

ABSTRACT

OBJECTIVES: To assess the risk factors, antibiotic therapy and outcomes of multidrug-resistant Gram-negative bacilli (MDRGNB) bacteraemia in hospitalized patients with cancer. METHODS: Episodes of MDRGNB bacteraemia were compared with a susceptible control group in a 4 year prospective study. RESULTS: Of 747 bacteraemias, 372 (49.7%) were caused by a Gram-negative bacilli (GNB). Fifty-one of these (13.7%) were caused by a multidrug-resistant (MDR) strain. Previous antibiotics [odds ratio (OR) 3.57; 95% confidence interval (CI) 1.63-7.80] and urinary catheter (OR 2.41; 95% CI 1.01-5.74) were identified as independent risk factors for MDRGNB acquisition. The most frequent mechanism of resistance was extended-spectrum ß-lactamase (ESBL) production (45%), mainly by Escherichia coli, followed by Amp-C cephalosporinase hyperproduction (24%). Patients with MDRGNB bacteraemia more frequently received inadequate initial antibiotic therapy (69% versus 9%; P < 0.001) and time to adequate therapy was longer in this group (41% versus 4%; P < 0.001). Patients in the resistant group more frequently required intensive care unit (ICU) admission (14% versus 5%; P = 0.023), had greater need for mechanical ventilation (14% versus 3%; P = 0.005) and had a higher overall case-fatality rate (41% versus 21%; P = 0.003). Risk factors for mortality were solid tumour (OR 5.04; 95% CI 2.49-10.19), current corticosteroid use (OR 4.38; 95% CI 2.39-8.05), ICU admission (OR 11.40; 95% CI 3.19-40.74) and MDRGNB bacteraemia (OR 3.52; 95% CI 1.36-9.09). CONCLUSIONS: MDRGNB bacteraemia was common among cancer patients, especially in those exposed to antibiotics and urinary catheter. The most frequent mechanism of resistance was ESBL production. Patients with MDRGNB more frequently received inadequate empirical antibiotic therapy and presented poorer outcomes with a higher overall case-fatality rate (within 30 days).

ABSTRACT

The purpose of this study was to assess the risk factors for, and the clinical relevance of, faecal carriage by extended-spectrum ß-lactamase producing Escherichia coli (ESBL-EC) in neutropenic cancer patients (NCP). An observational prospective multicentre cohort study was conducted over 2 years at two teaching hospitals. Patients with acute leukaemia or undergoing stem cell transplantation were included during neutropenia episodes. Rectal swabs were obtained at hospital admission and weekly thereafter until discharge or death. ESBL-EC colonized episodes were compared with non-colonized episodes. ESBL-EC strains were studied by PCR and isoelectric focusing, and molecular typing was performed by pulsed field gel electrophoresis (PFGE). Among 217 episodes of neutropenia, the prevalence of ESBL-EC faecal carriage was 29% (14% at hospital admission). Multivariate analysis identified previous antibiotics as the only independent risk factor for ESBL-EC faecal colonization (OR 5.38; 95% CI 2.79-10.39). Analysis of ESBL-EC isolates revealed a polyclonal distribution with CTX-M predominance (81.3%). E. coli bacteraemia was mainly caused by non-ESBL producing strains and its rate was similar in both groups (13% vs. 11%). We found no association between ESBL-EC carriage and an increased risk of ESBL-EC bacteremia or a negative influence on other clinical outcomes, including length of hospitalisation, early and overall mortality rates. ESBL-EC faecal colonization is frequent in NCP but difficult to identify by epidemiological or clinical features on presentation. Prior antibiotic therapy is the major associated risk factor. In this setting colonization does not appear to have a significant clinical relevance. Thus, routine testing for ESBL-EC faecal carriage does not seem to be beneficial.

ABSTRACT

Objetivo: Existen estudios en animales con cáncer de mama y estudios retrospectivos en humanos que sugieren una reducción de riesgo de metástasis tras realizar una anestesia regional en la cirugía de mama. Hemos estudiado si la realización de bloqueo paravertebral torácico (BPVT) asociado a una anestesia general comparado con una anestesia general sin BPVT reduce la incidencia de metástasis a corto plazo en las pacientes sometidas a cirugía oncológica de mama. Métodos: Se recogieron retrospectivamente 138 pacientes sometidas a cirugía de mama (bien cirugía conservadora, o bien mastectomía, en ambos casos con o sin linfadenectomía) en el periodo entre enero de 2008 hasta junio de 2009. Se consideraron las siguientes variables: edad, peso, antecedentes personales, tipo de tumor, grado histológico, TNM, índice de Nottingham, si recibió o no terapia sistémica, tipo de cirugía, tipo de anestesia, metástasis a 6 meses y a 12 meses, y la supervivencia libre de enfermedad. Resultados: En 40 pacientes se practicó un BPVT más anestesia general, y en 98 anestesia general solamente. En cuanto al grado histológico, clasificación TNM, índice de Nottingham y terapia sistémica no presentaban diferencias entre los dos grupos. La aparición de metástasis a 6 meses fue de 2,5% en el grupo de anestesia con BPVT y de un 6,1% en el grupo de anestesia general (p = 0,673), y a 12 meses, un 2,5% en el grupo de anestesia general con BPVT, y un 9,2% en el de anestesia general (p = 0,281). El consumo intraoperatorio de fentanilo y remifentanilo y de analgesia postoperatoria fue mayor en el grupo con anestesia general. Conclusiones: En este estudio retrospectivo el porcentaje de metástasis fue menor tanto a 6 como a 12 meses en las pacientes en las que se realizó un bloqueo paravertebral con respecto a las que se realizó anestesia general exclusivamente, sin que la diferencia fuera estadísticamente significativa(AU)


Objectives: A reduction in risk of metastasis after performing regional anesthesia in breast surgery has been suggested in both animal studies and retrospectives human studies with breast cancer. We studied whether thoracic paravertebral block (TPVB) associated with general anesthesia compared with general anesthesia reduces the metastases incidence in short term in patients undergoing breast cancer surgery. Methods: 138 patients undergoing breast surgery (either conservative breast surgery or mastectomy, both of them with or without lymphadenectomy) were retrospectively examined between January 2008 and June 2009. The following variables were recorded: age, weight, medical history, type of tumor, histological grade, TNM, Nottingham Index, adjuvant therapy, type of surgery, type of anesthesia, metastasis at 6 and 12 months and disease-free survival. Results: In 40 patients a TPVB combined with general anesthesia were performed, and 98 patients had general anesthesia alone. There were no differences in histological grade, TNM classification, Nottingham index and adjuvant therapy between the two groups. Metastasis at 6 months was 2.5% in the group of anesthesia combined with TPVB and 6.1% in the General Anesthesia group (p = 0.673). At 12 months was 2.5% and 9.2%, respectively (p = 0.281). The intraoperative consumption of fentanyl and remifentanil and postoperative analgesia requirements were higher in the group with general anesthesia. Conclusions: In this retrospective study, the rate of metastasis was lower at both 6 and 12 months in patients who underwent the paravertebral block combined with general anesthesia compared to general anesthesia. However, differences between both groups were not found to be significant(AU)

ABSTRACT

On 29 April 2009, an imported case of pandemic (H1N1) 2009 virus infection was detected in a London school. As further cases, pupils and staff members were identified, school closure and mass prophylaxis were implemented. An observational descriptive study was conducted to provide an insight into the clinical presentation and transmission dynamics in this setting. Between 15 April and 15 May 2009, 91 symptomatic cases were identified: 33 were confirmed positive for pandemic (H1N1) 2009 virus infection; 57 were tested negative; in one the results were unavailable. Transmission occurred first within the school, and subsequently outside. Attack rates were 2% in pupils (15% in the 11-12 years age group) and 17% in household contacts. The predominant symptoms were fever (97%), respiratory symptoms (91%), and sore throat (79%). Limited spread in the school may have been due to a combination of school closure and mass prophylaxis. However, transmission continued through household contacts to other schools.

ABSTRACT

Fecal colonization by extended-spectrum-beta-lactamase-producing Escherichia coli in 912 stool samples collected from 154 neutropenic patients with cancer, hospitalized at two teaching institutions, was prospectively studied. Forty-nine (31.8%) patients were colonized, 22 of them at hospital admission. Most strains were clonally unrelated and carried a CTX-M-9 group enzyme.

ABSTRACT

We reported our experience with the usefulness and interpretation of the significance of cultures at the second-stage exchange of infected arthroplasty in a prospective, 3-year follow-up study. When such intraoperative cultures were negative, patients received no therapy; when at least two cultures showed the same microorganism, results were interpreted as infection and patients were treated with antibiotics for 6-8 weeks with no more surgical procedures. Genotypic analysis (pulsed-field gel electrophoresis) was performed to analyse coagulase-negative Staphylococcus (CoNS) infections. Among 25 patients, 18 had negative cultures at the second-stage (group 1) and seven had positive cultures (group 2) receiving glycopeptides. Follow-up medians were 30 months for group 1 and 35 months for group 2; no patients in either group had persistence or recurrence of infection. All patients from group 2 had infection by CoNS at the second-stage; in six cases CoNS were also responsible for the initial infection. Genetic studies confirm that second-stage strains show different clonal identity than first-stage ones suggesting a superinfection rather than a real persistence of initial infection. Our results support the role of intraoperative cultures at the second-stage to identify patients at risk of recurrent infection who could benefit from early antibiotic therapy. The persistence of initial infections and the presence of new superinfections should be better defined according to genotypic studies.

We performed a retrospective matched-cohort study to determine the risk factors for mortality among patients with Escherichia coli infections. From January 1996 to December 2003, 100 hospitalised patients with extended-spectrum beta-lactamase (ESBL)-producing E. coli infections were compared with patients not infected with ESBL-producing E. coli. These patients were selected according to the same site of infection and the closest date of admission. Comparison of the two groups showed that empirical antibiotic therapy was more often inadequate in patients infected with ESBL-producing E. coli (44% vs 15%; P<0.01), and that early mortality (16% vs 6%; P=0.02) and overall mortality (25% vs 11%; P=0.01) were also significantly higher in patients with ESBL-producing E. coli infections. A multivariate model identified the urinary tract focus as the only independent risk factor influencing early mortality for E. coli infections [odds ratio (OR): 0.1; 95% confidence interval (CI): 0.03-0.7; P=0.01]. All 12 patients with ESBL-producing E. coli urinary tract infections treated initially with an oxyimino-beta-lactam survived. Subsequent analysis of the factors influencing early mortality in the cohort of 130 patients with a non-urinary E. coli infection found inadequate empirical antibiotic therapy as an independent risk factor for mortality only for non-urinary E. coli infections (adjusted OR: 3.0; 95% CI: 1.0-8.6; P=0.03). The study showed that hospitalised patients with ESBL-producing E. coli infections more often receive inadequate empiric antibiotic therapy and have a higher mortality rate than those infected with non-ESBL-producing strains. The site of infection strongly influences mortality. The administration of inadequate empirical antibiotic therapy is independently associated with higher mortality only among patients with non-urinary tract infections.

ABSTRACT

Between 1998 and 2003 the rate of erythromycin resistance among pneumococci in Spain was 34.4%. Although the MLS(B) phenotype was prevalent (94.7%), the rate of the M phenotype increased from 3.3% to 8.9% (P < 0.01). Clonal dissemination of mef(E)-carrying strains of serotype 14 variant of the Spain(9V)-3 clone was the major contributor to this increase.

ABSTRACT

Between 1996 and 2002, 103 hospitalised patients yielding one or more clinical isolates of extended-spectrum beta-lactamase-producing Escherichia coli (ESBL-EC) were identified. A significant increase was observed in the incidence of ESBL-EC colonisation or infection during the study period (1.65 episodes/100 000 patient-days in 1996 to 12.6 episodes/100 000 patient-days in 2002; p 0.01). Infection developed in 70 (68%) patients (75 episodes), with surgical site (44%) and urinary tract (17%) infections being the most frequent. Pulsed-field gel electrophoresis showed extensive clonal diversity among the isolates. A case-control study and multivariate analysis identified female gender (OR 2.1; p 0.01), use of a nasogastric tube (OR 3.5; p 0.001) and previous antibiotic therapy (OR 3.9; p < 0.001) as independent variables associated with acquisition of ESBL-EC. The study demonstrated a progressive increase in the number of ESBL-EC isolates in a non-epidemic setting. Most cases of ESBL-EC colonisation or infection occurred in hospitalised patients exposed to invasive procedures and antibiotic pressure.

INTRODUCTION: Nitric oxide (NO) is an important mediator of both physiological and pathological responses. Its dual role in the ischemia-reperfusion syndrome is still a matter of controversy. The aim of this study was to analyze the effect of NO on apoptosis and cell necrosis associated with heterotopic small bowel transplant. METHODS: Sprague-Dawley rats underwent heterotopic small bowel transplants with 3 hours of cold ischemia and 5 hours of reperfusion. Animals were assigned to the following study groups: Sham; bowel transplant (Trp); bowel transplant + NO donor (Trp + NONOS); bowel transplant + NO synthesis inhibitor (Trp + L-NAME). We studied histological changes and bacterial translocation in mesenteric nodes, liver and spleen as parameters of cell necrosis and caspase-3 activity as a parameter of apoptosis. RESULTS: Histological changes and bacterial translocation showed that exogenous administration of NO protected the transplant. Simple bowel transplant, with or without inhibition of NO synthesis, did not display this protective effect. Significantly greater levels of apoptosis were observe in grafts among the group administered NO at pharmacological doses. CONCLUSIONS: In experimental bowel transplantation rats administered exogenous NO show less necrosis but at the same time stimulation of apoptosis.
