ABSTRACT

Azoospermia, which is the absence of spermatozoa in the ejaculate, is not a rare cause of male infertility. Inducible nitric oxide synthase (iNOS) is a calcium-independent NOS, which is present in the testis and involved in spermatogenesis, and apoptosis of Sertoli and germ cells. Twenty idiopathic infertile men presenting nonobstructive azoospermia were enrolled in this study, and testicular sperm extraction procedures were performed. Tissue extracts were dissected, and the fluid samples were investigated to determine the presence of spermatozoa. Histologic evaluation of the spermatozoa-present samples revealed that seminiferous tubules were normal and were lined by Sertoli cells and spermatogenic cells. However, in the spermatozoa-absent samples, the diameter of the seminiferous tubules was small, and Sertoli-cell-only syndrome was determined in most of the tubules. iNOS expression was very weak in Sertoli cells, germ cells, and in Leydig cells in the spermatozoa-present group. In the spermatozoa-absent group, the immunostaining was very intense in Sertoli and Leydig cells. Electron microscopy findings were supported the histologic results. In conclusion, complete germ cell loss and intense expression of iNOS in the Sertoli and Leydig cells in the spermatozoa-absent groups of azoospermic human testis suggest an essential role of iNOS in spermatogenesis.

ABSTRACT

BACKGROUND: Omeprazole inhibits gastric acid secretion by blocking the proton pump of the gastric parietal cell. Nitrendipine is a derivative of the dihydropyridine group of calcium channel blockers and administrated for angina and hypertension. Famotidine is one of the newer histamine H2-receptor antagonists and heals gastric and duodenal ulcers by reducing gastric acid output. OBJECTIVES: The healing effects of omeprazole, nitrendipine and famotidine on stress-induced gastric ulcers were investigated in rats. METHODS: Forty male Wistar-albino rats were separated into five groups (n = 8), a control (non-stress) and four experimental (stress) groups. Experimental rats were treated with omeprazole, nitrendipine, famotidine or a placebo after the stresses of starvation and cold-restraint. RESULT: Macroscopically, the mean area of the affected lesional mucosa was 1/4 of the total gastric mucosa in the famotidine treated group and 1/5 of the total gastric mucosa in the nitrendipine treated group. A considerably decrease was observed in the omeprazole treated group in which the mean area of the lesional mucosa was only in 1/8 of the total gastric mucosa. On microscopic examination, congestive vessels and chronic inflammatory cell infiltrates were significantly reduced in the omeprazole treated group. Tissue regeneration was more prominent in the omeprazole group than the other groups. CONCLUSION: Omeprazole was found to be superior in terms of the effect on the healing process to nitrendipine and famotidine. Although therapeutic effects of nitrendipine and famotidine were observed, those were less than omeprazole.

ABSTRACT

Whipple's disease was diagnosed in a 42-year-old woman by the histological examination of a mesenteric lymph node taken at laparotomy, and the electron microscopic examination of a peroral intestinal biopsy specimen. She responded well to tetracycline therapy, but succumbed to fatal hepatitis ten weeks after the diagnosis of Whipple's disease
