ABSTRACT

Retinoblastoma (Rb), the most frequent malignant intraocular tumor in childhood, is caused by mutations in the retinoblastoma gene (RB1) situated on chromosome 13q14.2. The incidence of retinoblastoma is approximately 1 in 17,000 live births with approximately 8,000 new cases diagnosed each year worldwide. Rb is the prototypical hereditary cancer in humans. Autosomal dominant inheritance is seen in 30-40% of cases whereas the non-inherited sporadic type accounts for the remaining 60-70%. Rb arises due to inactivation of both alleles of the Rb tumor suppressor gene, which results in a defective Rb protein (pRB) with subsequent cell cycle impairment and uncontrolled cell proliferation. Patients with Rb have survival rates higher than 95-98% in industrialized countries but mortality remains high in developing countries. For example, the mortality rate in Africa is 70%. In all cases of intraocular and extraocular retinoblastoma, there is a need for new therapies that are more effective and carry less risk of toxicity. The Bruckner test is a practical and easy test for the detection of Rb, this test consists of assessing the fundus reflex through the pupil (red reflex) in both eyes simultaneously with a bright coaxial light produced with the direct ophthalmoscope. Rb can be detected by the Bruckner test showing a pupil that shines white or "Leukocoria". Although the diagnosis of Rb remains essentially clinical, the newly identified biomarkers could contribute to early molecular detection, timely detection of micrometastases and establish new therapeutic options for Rb.

ABSTRACT

Serjania triquetra is a medicinal plant widely used in traditional medicine for the treatment of urinary tract diseases, renal affections, and its complications. The population can buy this plant in folk markets as a raw material mixed with several herbal remedies or as a health supplement. On the market, two commercial presentations were found for the vegetal material; one had a bulk appearance and the other was marketed wrapped in cellophane bags (HESt-2, HESt-3). Nevertheless, the plant has not been exhaustively investigated and quality control techniques have not been developed. This research aimed to realize a phytochemical study using an authentic, freshly collected sample as a reference for S. triquetra (HESt-1), using the compounds identified. A method for the determination of preliminary chromatographic fingerprinting was developed. Additionally, the vasorelaxant effect from three samples was evaluated with ex vivo rat models. Thus, three hydroalcoholic extracts (HESt-1, HESt-2, and HESt-3) were prepared by maceration. A total of nine compounds were fully identified from HESt-1 after the extract was subjected to open-column chromatography. Seven metabolites were detected by gas chromatography, while ursolic acid (UA) and allantoin were isolated and identified using UPLC-MS and NMR, respectively. Three extracts were analyzed for their chromatographic fingerprint by UPLC-MS. Biological activity was explored by ex vivo rat aorta ring model to evaluate vasorelaxant activity. All extracts showed a vasorelaxant effect in a concentration-dependent and endothelium-dependent manner. S. triquetra vascular activity may be attributed to UA and allantoin compounds previously described in the literature for this activity.

Abstract: In recent decades, the growing obesity epidemic in Mexico has become one of the most important public health challenges faced by the country. With support from the World Obesity Federation, we formed a working group in 2021 to identify and summarize priority actions that Mexico can take to face this epidemic. More than 1 000 health professionals joined the development and discussion process. Recommendations from previously published, high-level documents and guidelines were taken into account. In commemoration of World Obesity Day 2022, this statement is presented as input for health care professionals to develop actions to address obesity. The statement includes 10 recommendations that include population-level and individual-level actions. It emphasizes the importance of social participation, comprehensive interventions with a person- centered perspective, planetary sustainability, on improving education and communication campaigns, as well as fostering a built environment that promotes active living, and shielding prevention and control efforts from conflicts of interest. The statement calls for obesity to be treated seriously, based on scientific evidence, in a timely and comprehensive manner, employing a life-course and ethical approach that does not perpetuate weight stigma in society.

INTRODUCTION: Transplants and organ donation are greatly aided by future medical professionals having adequate knowledge of this topic. This study aimed to elucidate the level of Mexican medical students' knowledge in the field of transplants and organ donation. MATERIALS AND METHODS: The evaluation instrument was designed and validated. The design used simple sampling with replacement, selecting a random sample of 5 universities from among the institutional members of the Mexican Association of Departments and Schools of Medicine (Asociación Mexicana de Facultades y Escuelas de Medicina [AMFEM]). The sample was composed of 3214 medical students. Measures of central tendency were determined, and the mean scores obtained across the different universities were compared using a Kruskal-Wallis test. The odds ratio was calculated for the students whose school or department included instruction on transplants and donation within their curriculum. Kendall correlation was used for the students' academic grade level and score. All analyses considered a threshold of P < .05. RESULTS: A questionnaire was administered to a sample of 2563 students to evaluate their knowledge of transplants and organ donation. The average score was 4.02 on a scale of 0 to 10 (standard deviation 0.03), with a 95% confidence interval (3.96-4.08). Students whose school or department taught the subject of transplants and donations within their curriculum obtained an odds ratio of 1.44 (P = .0000822). CONCLUSIONS: The findings of this study suggest that medical students in Mexico do not have sufficient knowledge of transplants and organ donation.

RESUMEN En el presente trabajo se compilan los hallazgos de diferentes publicaciones sobre las conductas autolesivas sin intención suicida en adolescentes. En relación a su conceptualización y tipología, se define que las conductas autolesivas son un acto de expresión y autorregulación de emociones de forma intencional, a través de las cuales los sujetos se autoinflingen daño físico sin ningún deseo consciente de morir, por lo que se excluye el intento suicida. Hay una amplia clasificación que circunscribe a las conductas autolesivas desde una perspectiva de enfermedad psiquiátrica, hasta la expresada solo como una forma de identificación y necesidad de aceptación y seguridad en la etapa de la adolescencia dentro de su entorno, caracterizada comúnmente por cortarse la piel y quemarse. Desde una perspectiva de la salud pública es importante reconocer este comportamiento en los adolescentes. Respecto a aspectos epidemiológicos, se reporta la prevalencia de esta conducta en un 13-29 % de la población adolescente, quienes la presentan al menos una vez en su vida, siendo más frecuente en las mujeres. Por último, el artículo refiere los factores determinantes para el desarrollo de dicha conducta en lo biológico, lo psicológico y lo social. La investigación supone un aporte para la intervención de la salud pública en el fenómeno, a partir de las funciones de promoción y prevención que tiene esta.

There is a broad classification of the this type of behavior describing it like a psychiatric illness or an expression and identification form in teenagers in order to fit into their environment, usually characterized by skin cutting and burns. Epidemiology reported a prevalence of this behavior in 13-29 % of the adolescent population, presented at least once in their lives. This behavior is reported as more common in women. Finally, references of the determining factors on biological, psychological and social aspects that may influence the presence of this behavior are presented, finding within them a great contribution to develop public health interventions to comply with essential functions of promotion and prevention.

ABSTRACT

Objetivo: determinar la relación entre la manifestación del síndrome de lipodistrofia y la terapia antirretroviral de gran actividad en pacientes con VIH/sida. Métodos: se realizó un estudio descriptivo, transversal y correlacional en pacientes con VIH/sida que reciben terapia antirretroviral de gran actividad, atendidos entre marzo y diciembre de 2007 en el Centro Ambulatorio de Prevención y Atención del Sida e Infecciones de Transmisión Sexual (CAPASITS) de Tepic, Nayarit, México. La definición y diagnóstico del síndrome se realizó mediante el método de The National Centre in HIV Epidemiology and Clinical Research (NCHECR) de Australia. Se utilizó la prueba de chi cuadrado para evaluar la dependencia entre el síndrome y la terapia. Se evaluaron 175 pacientes (128 hombres y 47 mujeres), de 19 a 72 años de edad. Resultados: se diagnosticaron 141 pacientes (80,6) con síndrome de lipodistrofia (IC95% 74,7-86.4 %); el 82,6 % correspondió a hombres y el 74,5 % a mujeres. Según la severidad, el porcentaje fue de 17 % de grado 1, 3 % de grado 2, 10 % de grado 3 y 51 % de grado IV. Las pruebas de chi cuadrado para evaluar dependencia entre el síndrome y la terapia resultaron no significativas. Conclusiones: el síndrome de lipodistrofia severo resulta un serio problema para la apariencia de pacientes de VIH/sida que reciben o no la terapia antirretroviral y que agrega un riesgo cardiovascular importante que debe ser considerado para intentar su prevención o tratamiento. Aunque los resultados pueden presentar sesgos y limitaciones, aportan una aproximación importante para sustentar y planificar intervenciones sanitarias que disminuirían el impacto del síndrome en la salud de los pacientes con VIH/sida.


Objective: to establish the relationship between lipodystrophy syndrome and highly active anti-retroviral therapy in HIV/AIDS patients. Methods: a cross-sectional, descriptive and co-relational study was conducted in HIV/AIDS patients, who had received highly active antiretroviral therapy in Centro Ambulatorio de Prevención y Atención del Sida e Infecciones de Transmisión Sexual (CAPASITS) de Tepic in Nayarit, Méjico from March to December, 2007. The definition and diagnosis of this syndrome was based on the method developed by the National Center in HIV Epidemiology and Clinical Research (NCHECR) method in Australia. The Chi square test evaluated the dependence between syndrome and therapy. One hundred seventy five patients (128 men and 47 women) aged 19-72 years were evaluated. Results: one hundred forty one patients (80.6 %) were diagnosed with lipodystrophy syndrome (CI95%74,7-86,4 %); 82.6 % corresponded to men and 74.5 % to women. According to severity, 17 % classified as grade 1; 1.3 % in grade 2; 10 % in grade 3 and 51 % as grade IV. The Chi square tests for the evaluation of dependence between syndrome and therapy were not significant. Conclusions: severe lipodystrophy syndrome is a serious health problem for HIV/AIDS patients receiving antiretroviral therapy or not. It adds a significant cardiovascular risk that must be considered for prevention or treatment. Although the results may be biased or restricted, they represent an important approach to planning and performing health interventions that will reduce the impact of the syndrome on the health of HIV/AIDS patients.

Con el objetivo de comparar el aprendizaje de habilidades clínicas en estudiantes de la Universidad Autónoma de Nayarit que desarrollaron su internado de pregrado en tres instituciones hospitalarias del Sistema Nacional de Salud, se realizaron pruebas objetivas al inicio y fin del periodo y cuestionario con preguntas abiertas y semicerradas. Se efectuaron análisis no paramétricos para comparación de grupos y análisis discriminante para identificar las variables más útiles para diferenciar las instituciones en relación al ambiente. Se obtuvo diferencia en al menos una de las comparaciones entre instituciones; la mayoría de los estudiantes en las tres instituciones percibió que sus tutores tuvieron de regular a mala disposición para la enseñanza o para permitirles participar en las actividades académicas y fue el jefe de enseñanza el encargado de coordinar las sesiones académicas. El análisis discriminante demostró que las variables más influyentes en la discriminación resultaron el papel del coordinador y la calidad de las actividades académicas, las horas de sueño y el tamaño de los grupos de internos asignados a las guardias. En las tres instituciones existió un incremento significativo del aprendizaje de habilidades clínicas entre el inicio y el fin del internado, con un nivel mayor en el aprendizaje en la sede del IMSS


A comparative study of the learning of clinical skills was carried out in Universidad Autónoma de Nayarit in Mexico. The participating students, who developed their undergraduate internship in three different hospitals of the national health care system of that country, answered a questionnaire including open and semi-close questions. The non-parametric analysis compared the groups whereas the discriminating analysis identified the most useful variables, differentiated the institutions with respect to the environment and revealed the opinions of the medical students about the undergraduate internship period. Some differences were noted at least in one of the comparisons among the institutions; the majority of students from the three institutions perceived that their tutors´willingness to teach them or to allow them participate in the academic activities was not good in general, so it is the head of education who is in charge of coordinating the academic sessions. The most influential variables in the discriminatory analysis were the role of the coordinator and the quality of the academic activities, the sleeping hours and the size of the groups of interns assigned to the medical calls. Significant rise of the clinical skill learning was observed in the three institutions when comparing the results from the beginning to the end of the internship period, being the level of learning higher in the Instituto Mexicano del Seguro Social (IMSS)
